心房扑动射频线性消融时局部电图改变的临床价值

摘要 目的 :探讨射频线性消融心房扑动 (房扑 )时局部电图改变的临床价值。方法 :对I型房扑患者行射频线性消融 ,采用峡部双向传导阻滞标准判断消融是否成功。同时观察消融电极处局部电图的改变。结果 :消融成功后局部电图发生了明显的变化 ,主要表现为双电位及A波幅度变小。消融后A波幅度平均为(0 3 3 7+0 2 79)mV ,消融前A波幅度平均为 (1 2 17+0 886)mV ,消融前后相比有极显著性差异 (P <0 0 1)。结论 :局部电图变化对快速判断消融是否成功具有重要意义。 Objective:The purpose of this study was to observe the changes of local electrogram in ablation electrodes and evaluate its clinical value. Methods:Radiofrequency ablation was performed in patients with atrial flutter.The criteria of conduction block in isthmus were adopted.Bipolar electrograms were recorded along the ablation line at the sites where radiofrequency ablation resulted in complete isthmus block. Results:①Double potentials were observed at all recorded sites when the block was complete. ②Lower amplitude of the bipolar electrogram was observed after ablation. Conclusions:The changes of local electrogram in ablation electrodes were valuable in evaluating a successful ablation.
