肾上腺皮质腺瘤的CT、MRI表现对比分析(附22例报告)

A comparative study of CT and MRI in adrenal adenoma

目的 :通过CT、MRI诊断与手术病理对照 ,提高肾上腺皮质腺瘤影像学诊断与临床诊断符合率。材料与方法 :收集我们医院从 1993～ 2 0 0 2年间经手术、病理证实的 2 2例肾上腺皮质腺瘤的CT或 /和MRI检查资料进行对比分析。其中男性 9例 ,女性 13例 ,年龄为 2 1～ 6 2岁 ,平均年龄 4 3岁。检查设备为GE980 0型CT机 ,GEProspeedSXAdvantage型螺旋CT机 ,GE1 0TSignaHorizonLX型磁共振设备 ,行CT或 /和MRI平扫 ,然后行增强检查。结果 :2 2例均为单侧发病 ,其中右侧 15例 ,左侧 7例。最小病灶为 9×12mm ,最大病灶为 170× 12 0mm。CT表现为肾上腺区圆形或椭圆形低密度或等密度影 ,边缘部分清楚 ,增强可显示部分强化 ,MRI表现为T1WI类似肝实质信号 ,T2 WI接近或高于肝实质信号。结论 :①肾上腺皮质腺瘤CT、MRI表现有一定的特征性 ;②结合临床资料 ,肾上腺皮质腺瘤能与部分肾上腺病变鉴别。

Objective : To improve the accordance of imaging diagnosis and clinic diagnosis, the paper studied the CT and MRI findings of adrenal adenoma compared with pathology. Materials and methods∶22 cases (M∶9F∶13) adrenal adenoma from 1993 to 2002 which confirmed by pathology were retrospectively analysed. The age is from 21 to 62 years old (mean: 43).All the patients underwent CT scan (plain and enhanced CT) and MRI. GE 9800 CT, GE Prospeed SX Advantage spiral CT and GE 1.0T Signa Horizon LX MRI were used. Results∶22 cases were all unbilitary, 15 cases in the right and 7 cases in the left. The minimum lesion is 9×12mm. The maximum is 120×170mm. The main imaging appearance was ball oroval, iso or signal low density mass on adrenal, partly enhancement on CT, iso indensity singral to liver on T1WI and iso or high indensity signal to liver on T2WI. Results∶①Adrenal adenoma has characteristic imaging appearance on CT and MRI;② Combined with clinic date. Adrenal adenoma can be differentiated from other adrenal diseases.
