氯吡格雷、低分子肝素与阿司匹林联用治疗不稳定型心绞痛临床观察

Clinical observation of clopidogrel combined with low molecular mass heparin and aspirin on unstable angina pectoris

目的 探讨小剂量氯吡格雷、低分子肝素和肠溶阿司匹林联用治疗不稳定型心绞痛的临床疗效及安全性。方法 选择诊断明确的不稳定型心绞痛患者 6 4例 ,随机分为治疗组和对照组 ,对照组采用低分子肝素、肠溶阿司匹林及抗心绞痛药物 ,治疗组在以上治疗基础上加用氯吡格雷 ,观察两组对心绞痛的疗效 ,监测出、凝血指标 ,并评价其安全性。结果 治疗组能明显减少心绞痛发作频率 (P <0 .0 1 ) ,改善临床症状 ,对改善心电图缺血性S T段压低明显优于对照组 ,且对出、凝血指标影响不大 (P >0 .0 5 )。结论 小剂量氯吡格雷、低分子肝素和阿司匹林联合应用治疗不稳定型心绞痛疗效肯定且安全 。

Objective To evaluate the efficiency and safety of clopidogrel combined with low molecular mass heparin and aspirin on unstable angina pectoris(UAP). Methods Sixty four patients with UAP were divided randomly into treatment group and control group. The patients in control group were treated with low molecular mass heparin, aspirin and the routine therapy of angina pectoris. The patients in treatment group were added with clopidogrel besides theabove medications. The effect of the drugs on bleeding and coagulating time were observed. Results Compared with the control, the attack frequency of angina pectoris was reduced dramaticly, the clinical symptoms and electrocardiology(ECG) were improved as well in treatment group. The drugs had no effects on bleeding and coagulating time. Conclusion Low dosage of clopidogrel combined with low molecules mass heparin and aspirin in treatment of UAP has shown more effective and safety than molecules mass heparin and aspirin.
