单侧多功能外固定支架结合有限内固定治疗胫腓骨开放性骨折

Treatment of Open Tibia Shaft Fractures with Unilateral Multifunctional External Fixator Combined with Limited Internal Fixation.

目的 探讨单侧多功能外固定支架结合有限内固定治疗胫腓骨开放性骨折的效果。方法 1 31例胫腓骨开放性不稳定性骨折 ,其中 4 6例采用外固定支架结合螺丝钉治疗 ,4 3例单纯采用外固定支架治疗 ,4 2例采用交锁髓内钉治疗。结果 采用Johner-Wruhs评分。单侧多功能外固定支架结合螺丝钉治疗组失血量、手术时间明显低于交锁髓内钉组 (P <0 0 5 ) ,骨愈合时间明显短于单纯用外固定支架组 (P <0 0 5 ) ,与交锁髓内钉组无明显差异性 (P >0 0 5 )。结论 该方法具有固定牢靠、创伤少。

Objective To study effect on treatment of open tibia shaft fractures with unilateral multifunctional external fixator combined with limited internal fixation.Methods One hundred thirty one cases of open unstable tibia shaft fractures were classified into the three groups.Forty six cases in the treatment group were treated with unilateral multifunctional external fixator combined with the screws.Forty three cases in the first contrast group were treated with mere unilateral multifunctional external fixator.And 42 cases were treated with intramedullary interlocking nails.Results Final results were evaluated according to Johner-Wruhs scoring system.Amount of operative bleeding and operative time in the treatment group decreased more significantly than the ones in the second contrast group (P<0 05).However,bone healing time in the treatment group shortened more significantly than that in the first contrast group (P<0 05),and it had not a significant difference compared with that in the second group (P>0 05).Conclusion This operative method of unilateral multifunctional external fixator combined with limited internal fixation has the advantages of stabe fixation,less operative time,less amount of operative bleeding,minimum tissue trauma and quicker bone union.
