ABS-2000全自动血库系统对输血相容性检测进行全面质量控制的评价

摘要 目的 评价ABS 2 0 0 0全自动血库系统 (以下简称ABS系统 )在临床输血实验室全面质量控制中的意义。方法 使用人工设计的各种样本对ABS系统进行特异性、灵敏度及抗干扰实验的对比评估 ,并对临床 5 2 2 6例标本进行ABO正反定型、Rh(D)测定及抗体筛检测定 ,分析其结果。结果 检测标本在特异性、灵敏度和抗干扰实验中均能达到设计要求 ,并优于手工Polybrene方法 (以下简称凝聚胺法 )。ABO血型测定 5 2 2 6份 ,除 1 39份正反定型不符 (NTD)外 ,其他测定血型全部与报告血型相符 ;Rh(D)所测结果为 2份NTD ,5 2 2 6份结果全部相符。抗体筛检测定 5 2 2 6份 ,抗筛阳性 2 0 1份 ,使用同一种抗筛细胞进行凝聚胺法抗筛测定阳性仅为 1 4 8份 (P <0 .0 0 5 )。结论 ABS系统应用的固相检测技术具有较高的敏感度 ,能检测出较弱的IgG类不规则及自身抗体 。 Objective To evaluate the safty and applicability of the fully automated blood bank system (ABS 2000) and to compare it with standard manual method.Methods Microtiter plates were used for determining ABO and Rh(D) types,solid phase red cell adherence assays were used for irregular antibody detection in order to assess the specificity,sensitivity and anti disturb test.5226 patients' samples were tested by ABS 2000 and standard manual test tube methods.Results The speificity,sensitivity and anti disturb test of ABS 2000 were higher than standard manual test tube method.In 5226 patients' samples tested for ABO types were 100% concordant except 139 NTD result.Dtypes tests were 100% concordant except 2 patients' NTD results.201 were positive tested by contibody screen test,while 148 were positive tested by manual test tube methods,with statistically obvious difference ( P <0.005).Conclusions The ABS 2000 system using soloid phase system for testing has higher sensitivity than standard manual test tube method,the ABS 2000 offers a higher standard of blood testing for hospital transfusion services and it is also a valuable tool in total quality control of transfusion loboratory.
