多普勒组织成像技术在评价糖尿病患者心脏功能方面的临床研究 被引量:1

Clinical Study on Doppler Tissue Imaging for Assessment of Left Ventricular Function in Patients with Type 2 Diabetes Mellitus

摘要 目的 探讨多普勒组织成像技术 (DTI)评价 型糖尿病患者左室整体功能的临床应用价值。方法 采用 HP Sonos 5 5 0 0型超声诊断仪 ,对 34例 型糖尿病患者和 1 6例正常人进行了二尖瓣环运动速度的测定 ,并用常规超声心动图法检测左室收缩舒张功能指标 ,对这两组参数对比分析。结果 发现糖尿病组 Em、 Am、Em/Am与对照组比较有显著性差异 ;同时 L Ad、 E、 A、 E/A、 EDT、 IVRT等指标在两组间亦有显著性差异 ;以Em/Am<1为标准 ,对糖尿病患者舒张功能异常的检出率为 2 7/34(79.4 1 % ) ,而以 E/A<1或 E/A>2为标准其检出率为 2 2 /34(6 4 .71 % )。结论  型糖尿病患者舒张功能异常早于收缩功能异常 ;DTI技术能敏感、准确评价左室舒张功能 。 Objective To evaluate left global ventricular function in type 2 diabetes mellitus using Doppler tissue imaging (DTI). Methods Thirty four patients with type 2 diabetes mellitus and sixteen normal subjects underwent Hewlett Packard Sonos 5500 echocardiographic system with DTI Mitral annular velocities were measured by DTI The cardiac functions were analysed using the conventional echocardiography The correlation and differences between TDI data and those conventional echcardiographic parameters were analysed. Results Em, Am,Em/Am were significantly different between the two groups,and LAd,E,A,E/A,EDT,IVRT were statistically different between the two groups,either Conclusions The left ventricular diastolic dysfunction happens earlier than the systolic dysfunction in patients with type 2 diabetes mellitus DTI can assess the left ventricular dysfunction in the patients with type 2 diabetes mellitus The sensitivity and the evaluation accurary of DTI is higher than those of the conventional echocardiography
