硬膜外术后镇痛对慢性阻塞性肺病患者术后肺功能的影响

Effects of analgesic pump on respiratory function in upper abdominally operated patients with chronic obstruction pulmonary diseases

目的 探讨术后使用镇痛泵对慢性阻塞性肺病患者上腹部手术后肺功能的影响。方法 40例择期上腹部手术患者随机分为Ⅰ (对照组 )、Ⅱ (镇痛泵组 )两组 ,采用AS -3 0 0型电脑肺功能测定仪监测术前和术后 12、2 4、48h仰卧位时肺活量 (Vc)、潮气量 (VT)、1秒肺活量 (FEV1.0 )、最大呼气中期流速 (MMF)及呼吸频率 (RR) ,同时测定脉搏氧饱和度 (SPO2 )及术前和术后 8、12h的血气结果。结果 两组患者术后肺功能均有明显抑制 ,但Ⅱ组患者术后肺功能下降的幅度明显小于Ⅰ组 (P <0 .0 1)。结论 应用止痛泵能显著改善慢性阻塞性肺病患者上腹部手术后的呼吸功能。

ObjectiveTo investigate the effects of analgesic pump on respiratory function in 40 upper abdominally operated patients with chronic obstruction pulmonary diseases (COPD).MethodsThe patients subject to elective abdominal operation were randomly divided into the control group (Ⅰ group) and analgesic pump group (Ⅱ group). VC,VT,FEV1.0, MMF and RR were assessed before operation,12 h,24 h and 48 h after anesthesia,and pulmonary function was analyzed with AS-300 instrument.SPO 2 and PO 2 were also assessed before operation and after anesthesia.ResultsAfter operation,pulmonary function was suppressed significantly in the two groups,but the inhibitory degree of pulmonary function in group I was significantly greater than that in group Ⅱ(P<0.01).ConclusionApplication of analgesic pump after operation can improve pulmonary function in COPD patients markedly.
