腹腔镜胆囊切除术致胆道损伤临床分析

The Clinical Analysis of Bile Duct Injury Caused by Laparoscopic Cholecystectomy

目的:探讨腹腔镜胆囊切除术致胆道损伤的临床特点和处理方法。方法:对1994～2002年间腹腔镜胆囊切除术1550例发生胆管损伤15例病人的临床资料进行回顾性分析。结果:15例中,死亡1例,余14例随访3月至5年,胆道狭窄2例,余12例恢复满意。结果:腹腔镜胆囊切除术致胆管损伤与开腹手术致胆囊损伤比较,有其特点,术中发现可立即修复,结果较好。应依据不同类型,选择不同的手术方式。

To explore clinical characteristics and management of bile duct injury caused by laparoscopic cholecystectomy. Methods:Clinical manifestations of 15 cases of bile duct injury caused by laparoscopic choleeystectomy from 1994 to 2000 in our hospital were analyzed retrnspeetively.Results:In 15 cases,1 case dead other 14 cases followed ranging from 3 months to 5 years.With biliary stricture occurred in 2 cases,other 12 cases recovered well.Conclusion:Comparing with injury caused by laparoscopic cholecysteetomy,the bile duct injury caused by laparoscopic cholecystectomy has its own characteristics.If the iojury could be found during operation the outcome of immediate repair would be satisfactory.Different treatment should be used according to its injury type.
