不同联合用药方案治疗椎体骨质疏松性骨折的有效性及安全性评价

Efficacy and Safety Evaluation of Different Drug Combinations in the Treatment of Ver-tebral Osteoporotic Fractures

目的:椎体骨折是一种常见的骨质疏松性骨折,药物是其治疗基础,本研究探索四种不同联合用药方案治疗的有效性及安全性。方法:挑选2018年9月~2020年8月的椎体骨质疏松性骨折病人400例,将其随机性分成A、B、C、D四组,每组各100例,均以口服钙片及维生素D作为基础治疗。A组进行基础 + 唑来膦酸治疗,B组进行基础 + 鲑降钙素鼻喷雾剂治疗,C组进行基础 + 甲状旁腺素类似物(特立帕肽)治疗,D组进行基础 + 仙灵骨葆胶囊治疗。分别比较四组患者治疗前后的骨密度水平、临床疗效、疼痛程度、安全性及生活质量。结果:1) 治疗前骨密度差异没有统计学意义(P 】0.05);治疗6个月与12个月后,C组 】A组 】B组 】D组(P 0.05),治疗后显著高于治疗前(P 【0.05),且C组最为明显(P 【0.05)。结论:钙片及维生素D联合特立帕肽治疗椎体骨质疏松性骨折能够获得更加良好的临床效果,且安全性可靠,可以作为首选治疗方案。

Objective: Osteoporotic fracture of the vertebral body is a common osteoporotic fracture, and med-ication is the basis of its treatment. This study explored the efficacy and safety of four different drug combinations. Methods: A total of 400 patients with osteoporotic fractures of the vertebral body treated from September 2018 to August 2020 were selected, and the selected objects were divided into four groups A, B, C, and D according to the random number table method. There were 100 cases in each group. The four groups were treated with oral calcium tablets and vitamin D as the basic treatment. Group A was treated with basic treatment + zoledronic acid;group B was treated with basic treatment + salmon calcitonin nasal spray;group C was treated with basic treatment + para-thyroid hormone analog (teriparatide);group D was treated with basic treatment + Xianling Gubao capsule treatment. Bone mineral density, clinical efficacy, pain, safety and quality of life were com-pared before and after treatment in 4 groups. Results: 1) There was no significant difference in bone mineral density before treatment (P >0.05);the bone mineral density was compared between 6 months and 12 months after treatment in group C >group A >group B >group D (P 0.05);after treatment, it was higher than before treatment (P <0.05), and was most significant in C group (P <0.05). Conclu-sion: Calcium tablets and vitamin D combined with teriparatide in the treatment of vertebral oste-oporotic fractures has achieved better clinical effect, and is safe and reliable, which can be used as the first choice of treatment for patients.
