摘要 血管性认知障碍(vascular cognitive impairment, VCI)是指脑血管病变及其危险因素导致的临床卒中或亚临床血管性脑损伤,涉及至少1个认知域受损的临床综合征,涵盖了从轻度认知障碍(mild cognitive impairment, MCI)到痴呆,也包括合并阿尔茨海默病(Alzheimer’s disease, AD)等混合性病理所致的不同程度的认知障碍。VCI概念的提出,有助于优化认知领域的诊疗和研究,而临床对VCI的认识和应用还有很大不足。本综述分别从概念、病理机制、辅助检查及诊疗方法进行分析,更加全面了解VCI的研究进展,从而对VCI患者的危险因素进行积极管控,对VCI的早期识别及早期治疗有重要的意义。 Vascular cognitive impairment (VCI) is predominately caused by vascular risk factors and cerebro-vascular disease. VCI includes at least one cognitive disorders, from mild cognitive impairment to vascular dementia caused by ischemic or hemorrhagic stroke, and vascular factors alone or in a combination with neurodegeneration including Alzheimer’s disease (AD) and AD-related dementia. The concept of VCI is helpful to optimize the diagnosis, treatment and research in the cognitive field, but the clinical understanding and application of VCI still leave much to be desired. In this review, the concept, pathophysiological mechanisms, supplementary examinations and therapeutic meth-ods of VCI were systematically examined, which is helpful to control risk factors and have taken early detection and proper treatment for the patients with VCI.
