目的:探讨在治疗踝pilon骨折合并外踝骨折改良前外侧入路单侧切口的临床效果及其对术后恢复的影响。方法:报告一例53岁男性患者的双侧皮隆骨折病例。患者因从约2米的高度坠落而受伤。采用改良前外侧入路单侧切口进行手术,详细描述了该手术方法,并评估其优缺点以及如何最大限度地减少常规双侧入路带来的并发症。结果:手术成功完成,术中出血量少,手术时间相对较短。术后观察到软组织损伤较小,患者疼痛明显减轻,功能恢复良好。随访期间无明显并发症发生,骨折愈合情况良好,踝关节功能恢复满意。结论:改良前外侧入路治疗踝皮隆骨折是一种有效的方法,具有减少手术时间、降低软组织损伤、促进骨折愈合及减少并发症等优点,值得临床推广应用。Objective: To investigate the clinical outcomes and postoperative recovery effects of a modified anterolateral approach using a single incision in the treatment of ankle pilon fractures combined with lateral malleolus fractures. Methods: This study reports a case of bilateral pilon fractures in a 53-year-old male patient. The patient sustained injuries from a fall of approximately 2 meters. A modified anterolateral approach with a single incision was used for the surgery. The surgical technique is described in detail, and the advantages and disadvantages of this method, as well as ways to minimize complications associated with the conventional bilateral approach, are evaluated. Results: The surgery was successfully completed with minimal intraoperative blood loss and relatively short operation time. Postoperative observations showed minimal soft tissue damage, significant pain relief, and good functional recovery. No significant complications were observed during follow-up, with satisfactory fracture healing and ankle joint function recovery. Conclusion: The modified anterolateral approach for treating ankle pilon fractures is an effective method, offering advantages such as reduced operation time, minimized soft tissue damage, promotion of fracture healing, and reduction of complications, making it worthy of clinical application and promotion.
