幽门螺杆菌感染、焦虑抑郁与口腔扁平苔藓发病及癌变的关系研究

Correlation of Helicobacter pylori infection, anxiety and depression with the pathogenesis and carcinogenesis of oral lichen planus

目的 :分析幽门螺杆菌(Hp)感染、焦虑抑郁与口腔扁平苔藓(OLP)发病及癌变的相关性。方法 :选取我院收治的OLP患者60例以及同期在我院进行体检的健康志愿者30例,分别设为OLP组和对照组。两组人员分别检测Hp、血清Hp-IgG抗体,CagA-IgG抗体和VacA-IgG抗体,采用医院焦虑抑郁量表(HADS)对两组人员进行焦虑抑郁测评。结果 :OLP组Hp强阳性和弱阳性表达数和对照组比较明显较高,差异有统计学意义。糜烂型OLP患者Hp阳性率和非糜烂型OLP患者比较,差异无统计学意义。OLP组中Hp-IgG抗体、CagA-HpIgG和VacAHpIgG抗体阳性率和对照组比较均明显较高,差异有统计学意义。糜烂型OLP患者Hp-IgG抗体、CagA-HpIgG和VacA-Hp IgG抗体阳性表达例数,差异无统计学意义。结论 :OLP与Hp感染及毒素CagA、VacA以及精神因素均具有一定相关性,进一步研究CagA、VacA这两种基因,对Hp感染引发的局部及全身疾病的治疗具有重要作用。此外,临床上还可通过对患者心理进行干预治疗,给予不同心理指导并配合适当药物,使治疗效果进一步提高。

Objective To analyze the correlation between Helicobacter pylori (Hp) infection, anxiety and depression, and the pathogenesis and carcinogenesis of oral lichen planus (OLP).Methods Sixty OLP patients an 30 healthy volunteers in our hospital were involved in this study, serving as OLP group and control group. Hp, serum Hp-IgG, CagA-IgG and VacA-IgG antibody of patients in two groups were detected. The anxiety and depression of the involved patients were measured by Hospital Anxiety and Depression Scale (HADS).Results The strong positive and weak positive expression of Hp in OLP group were sig-nificantly higher than those in control group. There was no significant difference in Hp positive rate between non-erosive OLP patients and erosive OLP patients. The positive rates of Hp-IgG, CagA-HpIgG and VacA-HpIgG antibodies in the OLP group were significantly higher than those in the control group. . The positive expression of Hp-IgG, CagA-HpIgG and VacA-HpIgG antibodies in erosive OLP patients was not statistically significant.Conclusion OLP is associated with Hp infection, toxin CagA, VacA and mental factors. Further study of CagA and VacA genes plays an important role in the treatment of local and systemic diseases caused by Hp infection. In addition, clinical psychological intervention is suggested for patients, different psychological guidance and appropriate drugs can improve the therapeutic effect.
