宫腔镜联合UAE在内生型瘢痕妊娠患者治疗中应用的优势及可行性

The feasibility and advantage of applying laparoscopic combined with UAE in the treatment of cesarean scar pregnancy type Ⅰ

目的 :探讨子宫动脉栓塞(UAE)联合宫腔镜手术治疗剖宫产切口瘢痕妊娠(CSP)内生型的临床效果。方法 :选取本院妇产科2011年1月~2014年2月手术治疗的82例CSP患者进行回顾性分析,其中42例患者采用UAE+宫腔镜检查+刮宫术联合手术治疗(观察组)、40例患者采用UAE+刮宫术治疗(对照组)。结果 :观察组的手术出血量、住院时间、术后阴道出血量、阴道出血时间、血β-HCG恢复时间显著的低于对照组患者,观察组的手术时间、术后月经恢复时间、术后1个月复查妊娠物残留率以及患者术后Ⅲ级疼痛程度发生率与对照组比较差异无统计学意义。结论 :UAE+宫腔镜检查+刮宫术联合手术治疗内生型瘢痕妊娠患者安全有效。

Objective To explore the clinical effective of combine hysteroscopy and UAE on treating cesarean scar preg-nancy (CSP) type I. S.Methods 82 CSP type I cases were selected in our hospital in January 2011 ~ February 2014 for sur-gical treatment,42 patients were treated with UAE + hysteroscopy + curettage combined with surgical treatment (observation group),40 cases were treated with curettage combined with UAE treatment (control group).Results Volume of blood lost during the operation, operation time, length of stay, postoperative vaginal bleeding, , and serumβ-HCG recovery time were lower than the control group significantly, the difference of recovery time of menstruation, residual rate assessed from ultrasound one month after operation, ⅲ grade pain incidence were not significant between the two groups.Conclusion UAE + hysteroscopy com-bined with curettage surgical treatment is safe and effective in patients with CSP I.
