椎体后凸成形术配合唑来膦酸治疗骨质疏松性胸腰椎压缩性骨折疗效分析 被引量:14

The clinical analysis of percutaneous kyphoplasty combined zoledronic acid in the treatment of 100 cases of osteoporotic vertebral compression fracture

摘要 目的 :探讨椎体后凸成形术配合唑来膦酸治疗骨质疏松性胸腰椎压缩性骨折的疗效。方法 :本研究选取了100例骨质疏松性胸腰椎压缩性骨折患者,按是否使用唑来膦酸分为两组,观察组(51例)行椎体后凸成形术配合唑来膦酸治疗,对照组(49例)行体后凸成形术治疗。观察并记录术前,术后6个月,12个月的伤椎椎体高度比,Cobb’s角,VAS评分,JOA评分,腰椎骨密度值及随访期间并发症发生情况,椎体后凸成形术配合唑来膦酸治疗骨质疏松性胸腰椎压缩性骨折的疗效。结果 :术后6个月,12个月,两组伤椎椎体高度比均明显升高,Cobb’s角均明显降低,且观察组伤椎椎体高度比高于对照组,Cobb’s角值低于对照组,术后6个月,12个月,两组VAS评分均明显降低,且观察组VAS评分明显低于对照组,术后12个月,观察组有效率明显高于对照组,术后6个月,12个月,两组腰椎骨密度值均明显升高,且观察组腰椎骨密度值明显高于对照组,随访期间,两组并发症发生率无明显差异。结论 :综上所述,PKP配合唑来膦酸治疗骨质疏松性胸腰椎压缩性骨折,能减轻老年患者疼痛,改善骨质疏松症状的同时有效预防患者发生二次骨折,值得临床推广使用。 Objective Discuss the efficacy of percutaneous kyphoplasty (PKP) combined zoledronic acid in the treatment of osteoporotic vertebral compression fracture.Methods In this study,100 patients with osteoporotic vertebral compression frac-ture were selected, they were divided into two groups according to whether use the zoledronic acid or not. The observation group (51 cases) was given PKP combined zoledronic acid. The control group (49 cases) was given PKP. The efficacy of PKP combined zoledronic acid in the treatment of osteoporotic vertebral compression fracture was evaluated by the injured vertebral height ra-tio, Cobb's angle, VAS score, JOA score, lumbar bone density score and complications during follow-up period.Results After 6 months and12 months surgery, the injured vertebral height ratio were significantly increased in two groups. The Cobb's angle were significantly decreased in two groups. The injured vertebral height ratio of the observation group was higher than that of the control group. The Cobb's angle of the observation group was lower than that of the control group. After6 months and12 months surgery, the VAS score of two groups was decreased. The VAS score of observation group was lower than that of the con-trol group. After12 months, the effective rate of observation group was higher than that of the control group. After 6 months and 12 months surgery, the lumbar bone density of two groups were increased. The lumbar bone density of observation group was higher than that of the control group. During the follow-up, there were no statistical significance on the complications between two groups.Conclusion Adopting PKP combined with zoledronic acid in the treatment of osteoporotic vertebral compression fracture, it could relieve pain and cure the symptoms of osteoporosis, prevent the secondary fracture effectively. It was worthy of clinical use.
