摘要
目的 :探讨乌苯美司胶囊对慢性阻塞性肺疾病急性加重期(AECOPD)患者气道炎症、免疫失衡及肺功能的影响。方法 :120例AECOPD患者随机分为治疗组与对照组,各60例,对照组采取常规治疗,治疗组在对照组基础上加以乌苯美司胶囊口服,连续用药8周。测定两组治疗前后肺功能、炎症因子、氧化应激反应及免疫功能指标,并观察两组临床疗效及不良反应情况。结果 :治疗组治疗总有效率95.00%,比对照组的83.33%差异有统计学意义;与治疗前比较,两组治疗后第1s用力呼气容积(FEV1)、FEV1占预计值百分比(FEV1%)、FEV1/用力肺活量(FVC)、超氧化物歧化酶(SOD)、CD3+、CD4+、CD4+/CD8+均显著上升,血清白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、丙二醛(MDA)、CD8+均显著下降,差异有统计学意义,观察组治疗后上述指标均显著优于对照组;两组不良反应比较无显著差异。结论 :乌苯美司胶囊能显著改善AECOPD患者免疫功能,减轻气道炎症反应,促其肺功能明显恢复,安全可行。
Objective To investigate the effect of Ubenimex Capsules on airway inflammation, immune imbalance and pulmonary function in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Methods A total of 120 patients with AECOPD were randomly divided into the treatment group and the control group, 60 cases in each group. The control group received routine treatment, while the treatment group was treated with oral Ubenimex Capsules on the basis of treatment in the control group. All patients were treated continuously for 8 weeks. The pulmonary function, inflammatory factors, oxidative stress reactions and immune function indexes in the two groups were determined before and after treatment, and the clinical efficacy and adverse reactions were observed. Results The total effective rate of treatment in the treatment group was higher than that in the control group(95.00% vs 83.33%). After the treatment, the forced expiratory volume in 1 s(FEV1), the percentage of FEV1 in predicted value(FEV1%), FEV1/forced vital capacity(FVC), superoxide dismutase(SOD), CD3+, CD4+ and CD4+/CD8+ increased significantly, while serum interleukin-8(IL-8), tumor necrosis factor α(TNF-α), C reactive protein(CRP), malondialdehyde(MDA) and CD8+ decreased significantly in the two groups, and the above indexes were significantly better in the observation group than in the control group. There was no significant difference in adverse reactions between the two groups. Conclusion Ubenimex Capsules can significantly improve the immune function, reduce airway inflammation and promote the recovery of lung function in patients with AECOPD, safe and feasible.
