作业治疗对不同等级手功能脑瘫儿童精细运动的影响 被引量:10

The Effection of Occupational Therapy to Fine Motor Skills of the Chrildren with Different Levels Hand Functional Cerebral Palsy

摘要 目的 :观察分析作业治疗对不同等级手功能脑瘫患儿精细运动的影响。方法 :选取2016年3月~2016年12月在我院儿童康复中心接受治疗的>4岁的脑瘫儿童194例,随机分为对照组96例和实验组98例,两组均采用运动疗法、言语治疗、引导式教育等综合治疗,实验组增加作业治疗,治疗时间为4个月;两组在治疗前后采用MACS进行手部功能分级,应用FMFM进行精细运动功能评估,用SPSS软件分析作业治疗对不同等级手功能脑瘫患儿精细运动功能评估数据进行分析。结果 :经4个月治疗后,实验组精细运动功能评分明显高于对照组;MACS分级不同的脑瘫患儿精细运动功能均得到了提高,其中手功能Ⅰ级、Ⅱ级、Ⅲ级患儿治疗后的FMFM评估得分显著高于治疗前,手功能Ⅳ级的患儿治疗后提高分数较低,手功能Ⅴ级患儿提高的分数最低。结论 :作业治疗可促进>4岁的不同等级手功能脑瘫患儿精细运动能力的提高;加强手功能训练可提高脑瘫患儿的自理能力,有助于脑瘫患儿全面康复和更快、更好地融入社会。 Objective Observing and analysing the effection of occupational therapy to fine motor skills of the chrildren with different levels hand functional cerebral palsy. Methods Choosing 194 over 4 years old children with cerebral palsy who were treated in our hospital between Mar.2016 to Dec.2016. The 194 patients were randomly divided into control group and experimental group. We used exercise therapy, speech therapy, conductive education and other methods to treat, and instructed the parents to cooperate the rehabilitation. The experimental group was added occupational therapy, Treatment time is 4 months. We used MACS to grade the hand function, applied FMFM to evaluate the fine motor skills function, and uses SPSS to analyse all the data. Results After 4 months treatment, The experimental group's score was apparently higher than the control group. The fine motor skills function of different levels cerebral palsy promoted. Children patients withⅠ, Ⅱ, Ⅲ levels problem got much higher score than before treatment, at the same time, patients with Ⅳlevel problem promoted much less than before, and patients with Ⅴ level problem changed the least. Conclusion Occupational therapy can promte the fine motors skills of cerebral palsy(over 4 years old) with different levels. Enhancing hand function training can improve the children patients self-care ability, which can help them integrate into the social life quickly.
