消化内镜诊断食管胃黏膜异位检出率的临床意义

Clinical significance of endoscopic detection of ectopic gastric mucosa in digestive tract

目的 :探究不同消化内镜模式对于诊断胃粘膜异位症的临床价值。方法 :回顾性分析2014年1月~2017年1月期间于我院门诊行胃镜检查的45618名患者,根据患者自身情况或意愿分为无痛白光内镜组,无痛染色内镜组和无痛NBI组,比较各组食管胃粘膜异位症的检出情况。结果 :HGMUE的总检出率为0.55%,HGMUE的内镜下表现:食管黏膜病变距门齿距离为(18.1+2.5)cm,病灶平均直径为(16.3+7.6)mm,多位于食管右侧壁(占比49.3%),平坦型多见(占比89.2%),7.9%患者合并Barrett食管;三组检出的HGMUE患者在年龄、性别、既往史、用药史、临床表现、内镜下表现以及伴发Barrett食管的差异无统计学意义;无痛NBI组检出率明显高于无痛白光组和染色内镜组,存在统计学意义。结论 :无痛NBI对于食管黏膜异位症的检出率高,在临床诊断HGMUE具有较好的临床价值值得进行推广。

Objective To explore the different digestive endoscopy mode for the clinical diagnostic value of ectopic gastric mucosa. Methods A retrospective analysis from January 2014 to January 2017 during the period of 45618 patients in our hospital outpatient gastroscopy, according to the patients' condition or divided into Painless Endoscopy Group, painless painless endoscopy group and NBI group were detected. Results The total HGMUE positive rate was 0.55%, HGMUE: endoscopic esophageal mucosal lesions from incisor distance(18.1+2.5) cm,(average diameter of lesions 16.3+7.6) mm, located in the right side wall of esophagus(accounting for 49.3%), flat type rare(accounting for 89.2%), 7.9% patients with Barrett esophagus; three were detected in HGMUE patients in age, gender, medical history, medication history, clinical manifestations, endoscopic features and differences with Barrett's esophagus was not statistically significant; painless group NBI detection rate was significantly higher than that of white painless group and staining endoscopy group, there were statistical significance. Conclusion Painless NBI for esophageal mucosal endometriosis of the high detection rate in clinical diagnosis Conclusion HGMUE has good clinical value and is worthy of promotion.
