摘要
目的 :探讨AECOPD患者的NT-ProBNP、CRP水平变化,并分析其临床意义。方法 :回顾性分析2015年3月~2016年10月在我院接受治疗的COPD患者的临床资料,根据其疾病程度分为稳定期COPD组和AECOPD组,并根据AECOPD组患者的预后将其分为好转组和恶化组,观察不同疾病程度COPD和不同预后AECOPD患者血清炎症因子、心肌酶、肺功能和血气指标的差异,分析AECOPD患者NT-ProBNP、CRP水平与肺功能的相关性。结果 :AECOPD组患者IL-8、IL-17、CRP和NT-ProBNP水平高于稳定期COPD组患者;好转的AECOPD患者的IL-8、IL-17、CRP和NT-ProBNP水平低于恶化组AECOPD组患者;AECOPD组患者的FVC、FEV1、MMEF、PEF和Sp O2水平低于稳定期COPD组患者;好转AECOPD患者的FVC、FEV1、MMEF、PEF和Sp O2水平高于恶化组;AECOPD患者的NT-ProBNP、CRP水平与FVC、FEV1、MMEF、PEF和Sp O2水平负相关。结论 :AECOPD患者的NT-ProBNP、CRP水平较高,且与患者的炎症因子和肺功能水平密切相关,可作为临床监测的重要指标。
Objective To investigate the changes of NT-ProBNP and CRP in patients with AECOPD and to analyze its clinical significance. Methods The clinical data of COPD patients who were treated in our hospital from March 2015 to October 2016 were retrospectively analyzed. The patients were divided into COPD group and AECOPD group according to the severity of the disease, According to the prognosis of the patients in the AECOPD group, the patients were divided into the improvement group and the deterioration group, Objective To observe the difference of serum inflammatory factors, myocardial enzymes, pulmonary function and blood gas indexes in patients with different COPD and different prognosis of AECOPD, and to analyze the correlation between NT-ProBNP, CRP and lung function in patients with AECOPD. Results The levels of IL-8, IL-17, CRP and NT-ProBNP in patients with AECOPD were higher than those in group COPD; The levels of IL-8, IL-17, CRP and NT-ProBNP in the patients with improved AECOPD were lower than those in the AECOPD group; The levels of FVC, FEV1, MMEF, PEF and Sp O2 in group AECOPD were lower than those in group COPD; The levels of FVC, FEV1, MMEF, PEF and Sp O2 in the patients with improved AECOPD were higher than those in the deteriorated group; The levels of NT-ProBNP and CRP in patients with AECOPD were negatively correlated with FVC, FEV1, MMEF, PEF and Sp O2. Conclusion The level of NT-ProBNP and CRP in patients with AECOPD is high, which is closely related to the level of inflammatory factors and pulmonary function in patients.
