综合心理干预对卒中后抑郁患者运动功能康复的影响

The effect of comprehensive psychological intervention on motor function rehabilitation in patients with post stroke depression

目的 探讨综合心理干预对卒中后抑郁患者运动功能康复的影响.方法 将124例卒中后抑郁患者按随机数字表法分为研究组与对照组,每组62例,均予以脑卒中常规治疗,研究组在常规治疗基础上进行综合心理干预,观察4周.于治疗前后采用汉密顿抑郁量表、美国国立卫生院脑卒中量表、简式Fugl-Meyer运动功能评分量表评定两组抑郁状况及运动功能康复效果.结果 治疗前两组各量表评分比较差异均无统计学意义(P＞0.05);治疗后两组汉密顿抑郁量表、美国国立卫生院脑卒中量表评分较治疗前显著下降(P＜0.01),且研究组显著低于对照组(P＜0.01);简式Fugl-Meyer运动功能评分量表评分较治疗显著升高(P＜0.01),且研究组显著高于对照组(P＜0.01).结论 早期对卒中后抑郁患者进行综合心理干预,可显著改善患者的抑郁情绪,有效促进患者神经功能及运动功能的康复.

Objective To explore the effect of comprehensive psychological intervention on motor function rehabilitation in patients with post stroke depression. Methods 124 cases of PSD patients were randomly divided into the intervention group and the control group, 62 cases in each group. The two groups of patients were all treated with conventional treatment, the intervention group was given comprehensive psychological intervention on the basis of conventional treatment. Patients were observed for 4 weeks. The HAMD, NIHSS and FM scales were used to evaluate the depression status and motor function rehabilitation efficacy before and after treatment. Results Before treatment, there was no significant difference in HAMD, NIHSS and FM scores between the two groups of patients (P ＞0.05); after treatment, the HAMD, NIHSS scores of the two groups were all significantly declined compared with those before treatment (P＜0.01), the intervention group was significantly lower than the control group (P＜0.01); the FM scores of the two groups were all significantly increased compared with those before treatment (P＜0.01),the intervention group was significantly higher than the control group (P＜0.01). Conclusion Early comprehensive intervention for patients with post-stroke depression can significantly improve the depression state of patients, and effectively promote the rehabilitation of patients' neurological function and motor function.
