摘要
目的观察双水平气道正压通气(BiPAP)无创呼吸机正压通气对老年重叠综合征并呼吸衰竭患者肺动脉压力及血浆血管紧张素Ⅱ(ANGH)的影响。方法将入选的老年重叠综合征并呼吸衰竭患者随机分为两组(A组、B组),A组给予BiPAP无创呼吸机治疗Ⅱ型呼吸衰竭,B组不给予无创呼吸机治疗,两组患者其他治疗方法相同。两组患者分别测定治疗前、治疗后7 d及治疗后14 d的肺动脉压力、血浆ANGII的浓度,并比较两组患者数据的差异。结果 A组治疗前与治疗后7 d、14 d肺动脉压力、血浆ANGII的浓度比较,差异均有统计意义(P<0.05),治疗后7 d与治疗后14 d肺动脉压力、血浆ANGII的比较,差异有统计意义(P<0.05);B组治疗前与治疗后7 d、14 d肺动脉压力、血浆ANGII的浓度比较,差异无统计意义(P>0.05),治疗后7d与治疗后14d肺动脉压力、血浆ANGII的比较,差异无统计意义(P>0.05)。结论 BiPAP无创呼吸正压通气治疗老年重叠综合征并呼吸衰竭患者,可以降低肺动脉压力及血浆ANGH浓度,具有较好的疗效。
Objective To investigate the effect of noninvasive mechanical ventilation on pulmonary artery pressure and plasma ANGII in the elderly patients with overlap syndrome and respiratory failure.Methods Elderly patients with overlap syndrome and respiratory failure were randomly divided into two groups(group A and group B).BiPAP noninvasive ventilator was given for treating type Ⅱ respiratory failure in group A.The other treatment were the same between the two group.The pulmonary artery pressure and plasma ANGII were determined at prior treatment,7 and 14 days after treatment.Results Compared to posttreatment measurements,the pulmonary artery pressure and plasma ANGII in prior treatment were significantly different(P<0.05).Pulmonary artery pressure and plasma ANGII in 7 days after treatment compared with 14 days after treatment was significantly different(P<0.05) in group A.The pulmonary artery pressure and plasma ANGII in prior treatment compared with 7 days after treatment and 14 days after treatment were significantly different(P<0.05).The pulmonary artery pressure and plasma ANGII in 7 days after treatment compared with 14 days after treatment was significantly different(P< 0.05) in group B.Conclusion The pulmonary artery pressure and plasma ANGII in elderly patients with overlap syndrome and respiratory failure treated by BiPAP noninvasive ventilator declined obviously.
