ACE基因I/D多态性与OSAHS合并高血压的关联性研究 被引量:2

Angiotensin Converting Enzyme Gene Polymorphism in Patients with Obstructive Apnea-hypopnea Syndrome Accompanied Hypertension

摘要 目的 探讨血管紧张素转换酶(ACE)基因I/D多态性与阻塞性睡眠呼吸暂停低通气综合征(OSAHS)合并高血压的关联性.方法 OSAHS患者157例,根据是否确诊为高血压分为OSAHS组(72例)和OSAHS合并高血压组(85例).将同期排除基础疾病的92例作为对照组.应用聚合酶链反应(PCR)检测ACE基因I/D多态性.结果 对照组、单纯OSAHS组、OSAHS合并高血压组年龄、体重指数(BMI)、呼吸暂停低通气指数(AHI)、平均血氧饱和度(MSaO2)、最低血氧饱和度(LSaO2)、收缩压、舒张压比较差异均有统计学意义(P均<0.01);单纯OSAHS组的BMI、AHI、MSa02、LSaO2均高于对照组(P均<0.01);OSAHS合并高血压组的年龄、AHI 、MSa02、LSaO2、收缩压、舒张压均高于对照组及单纯OSAHS组(P均<0.01),BMI高于对照组而与单纯OSAHS组差异无统计学意义(P<0.01).单纯OSAHS组、OSAHS合并高血压组Ⅱ基因型频率及Ⅰ等位基因频率均高于对照组(x2值分别为6.32、13.1,6.7、15.3,P均<0.05);单纯OSAHS组、OSAHS合并高血压组DD基因型频率及D等位基因频率均低于对照组(x2值分别为6.7、7.1,6.7、15.3,P均<0.05);单纯OSAHS组与OSAHS合并高血压组之间基因型及等位基因频率差异均无统计学意义(x2值分别为1.3、1.4,P>0.05).Ⅱ基因型OSAHS患者AHI高于ID及DD型(F=17.1,P<0.01).结论 ACE基因Ⅱ基因型及Ⅰ等位基因与西北汉族人群OSAHS发病及OSAHS合并高血压存在关联性. Objective To explore the association of angiotensin converting enzyme(ACE)gene polymorphism with obstructive sleep apnea-hypopnea syndrome(OSAHS)accompanied hypertension.Methods 157 OSAHS patients divided into two group:85 hypertensive and 72 normotensive;92 healthy subjects enrolled as control.DNA was extracted form blood samples and amplified by polymerase chain reaction.The distribution of ACE I/D allele and gene type were compared in 3 groups.Results The age,body mass index(BMI),apnea hypopnea index(AHI),mean oxygen saturation(MSaO),lowest2 oxygen saturation(LSaO),systolic 2 pressure,diastolic pressure in normotensive OSAH S were significantly higher than those in control group.Age,AHI,MSaO_2,LSaO_2,systolic pressure,diastolic pressure in hypertensive group were significantly higher than those in normotensive and control gourp.The frequency of II genotype and I allele in the hypertensive OSAHS were significantly higher and normotensive OSAHS than those in the normal controls(χ~2=6.32,13.1,6.7,15.3,P<0.05).The frequency of DD genotype and D allele in the hypertensive OSAHS were significantly lower and normotensive OSAHS than those in the normal controls(χ~2=6.7,7.1,6.7,15.3,P<0.05).The frequency of II genotype and I allele had no significantly different between the normotensive OSAHS patients and hypertensive OSAHS(P>0.05).Conclusion These results indicated that the I allele and II genotype were correlated with OSAHS and OSAHS accompanied hypertension.
