三种股动脉穿刺点封堵法的临床效果与安全性研究

Clinical Efficacy and Safety Outcome of Three Methods of Femoral Artery Occlusion

目的探讨股动脉穿刺点的三种血管封堵法的疗效与安全性。方法对90例行股动脉穿刺的患者分别采取传统手工压迫、血管封堵器、血管缝合器的方法进行治疗,每种方法 30例,对止血时间、制动时间、住院时间,术中出血量,术后穿刺点有无血肿、假性动脉瘤,下肢有无缺血、肿胀进行分析研究。结果血管缝合组和血管封堵组止血时间、制动时间及住院时间均较手工压迫组缩短(P均<0.05),血管缝合组和血管封堵组间差异无统计学意义(P>0.05);三治疗组术中出血量比较差异无统计学意义(P>0.05);血管缝合组和血管封堵组术后穿刺点血肿、假性动脉瘤,下肢缺血、肿胀发生率较手工压迫组明显减少(P<0.05)。结论血管缝合器和封堵器在处理周围血管疾病患者股动脉穿刺点方面有微创、风险小、恢复快、术后并发症少等优点。

Objective To investigate the efficacy and security of three methods with blocking puncture of femoral artery.Methods 90 patients with femoral artery puncture were enrolled and 30 patients in each group which respectively included the traditional manual pressure,vascular closure,blood vessel suture method. The end points included bleeding time,braking time,the hospital staying time,intraoperative blood loss,puncture point with postoperative hematoma,or with pseudoaneurysms,lower limbs with ischemia,lower limbs with swelling.Results The bleeding time,braking time and the length of hospital stay in vascular suture group and vascular blocking group were significantly shorter than those in manual compression group(P<0.05). There was no significant difference in bleeding amount among three treatment group(P>0.05). The incidence of puncture point hematoma,pseudoaneurysms,lower limb ischemia or ischemia aggravated,and swelling in the lower limbs in the vascular suture and vascular block group were decreased significantly compared with manual compression group(P<0.05).Conclusion Compared with manual compression,vascular suture and vascular closure take advantages in the treatment of peripheral vascular disease patients with femoral artery puncture of minimal injury,little risk,rapid recovery and less postoperative complications. Therefore in the treatment of peripheral vascular disease patients with femoral artery puncture point,the vascular suture and vascular closure are recommended.
