集束化干预在预防恶性肿瘤住院化疗患者下肢深静脉血栓形成的效果评价

Effect evaluation of the cluster of intervention for the prevention of lower extremity deep venous thrombosis in hospitalized patients with cancer during chemotherapy

目的：探讨集束化干预预防恶性肿瘤住院化疗患者下肢深静脉血栓形成的效果。方法收集2015年1－12月住院的恶性肿瘤化疗患者527例次作为试验组，2014年1－12月的502例次作为对照组；试验组给予集束化干预，对照组实施常规干预，比较2组患者下肢深静脉血栓形成发生率和满意度评分。结果试验组下肢深静脉血栓形成、血栓形成后综合征发生率分别为0.95%（5/527）和0.38%（2/527），对照组分别为3.39%（17/502）和1.99%（10/502），2组比较差异有统计学意义（χ2=6.148、4.472，均P＜0.05）；试验组患者满意度评分为（96.00±2.00）分，对照组为（92.92±2.39）分，2组比较差异有统计学意义（t=3.427，P＜0.05）。结论集束化干预应用于恶性肿瘤住院化疗患者，可以降低患者下肢深静脉血栓形成、血栓形成后综合征发生率，提高患者满意度，值得临床推广应用。

Objective To discuss the effect of the cluster of intervention for the prevention of limb deep venous thrombosis (LDVT) in hospitalized patients with cancer during chemotherapy. Methods 527 patients with cancer accepted chemotherapy admitted in the department of chemotherapy from January to December 2015 were collected as the experimental group, and 502 patients admitted from January to December 2014 as the control group. The cluster intervention was applied in the experimental group. The conventional intervention was applied in the control group. The incidence of limb deep venous thrombosis and satisfaction scores were compared between two groups of patients. Results The incidence of limb deep venous thrombosis and post-thrombosis syndrome (PTS) of patients were 0.95%(5/527) and 0.38% (2/527) in the experimental group, 3.39% (17/502) and 1.99% (10/502) in the control group, the difference between the two groups was statistically significant (χ2=6.148, 4.472, P<0.05). The satisfaction scores of patients was (96.00±2.00) points in the experimental group,and (92.92±2.39) points in the control group,the difference between the two groups was statistically significant (t=3.427, P<0.05). Conclusions The application of cluster intervention in hospitalized patients with cancer during chemotherapy could reduce the incidence of limb deep venous thrombosis and post-thrombosis syndrome, and improve the satisfaction of patients. The cluster intervention deserves further clinical promotion and application.
