针刺联合体感音乐疗法治疗脑卒中后抑郁症的临床疗效

Efficacy of acupuncture combined with vibroacoustic therapy in the treatment of post-stroke depression

目的 探讨针刺联合体感音乐疗法对脑卒中后抑郁症患者情绪变化、认知及日常生活能力的效果.方法 将脑卒中后抑郁症患者60例随机分为联合组和针刺组,在常规疗法基础上联合组采用针刺联合体感音乐疗法,针刺组采用针刺治疗.治疗4、8周后,分别用Hamilton抑郁量表(HAMD)评估患者的情绪、简易精神状态检查量表(MMSE)评定患者认知功能及Barthel指数评定量表(BI)评定患者的日常生活活动能力.结果 两组治疗4、8周后,HAMD评分较治疗前均有明显降低(P＜0.01);两组治疗4周后,MMSE较治疗前提高不明显(P＞0.05),治疗8周后,两组MMSE评分明显提高(P＜0.05);两组治疗4、8周后,BI指数较治疗前明显提高(P＜0.05);与针刺组比较,联合组治疗4周后,HAMD评分降低及BI指数提高更明显(P＜0.05),治疗8周后,HAMD评分降低、MMSE评分及BI指数提高更明显(P＜0.05).结论 针刺联合体感音乐疗法治疗脑卒中后抑郁症的效果优于单纯针刺治疗,更利于改善卒中后抑郁情绪,促进脑卒中后神经功能恢复,提高生活质量.

Objective:To explore the efficacy of acupuncture combined with vibroacoustic therapy for the treatment of mood changes,cognition and daily living ability in patients with post-stroke depression.Methods Sixty patients with post-stroke depression were randomly divided into combined group and acupuncture group under the routine therapy.The patients was treated with acupuncture combined with vibroacoustic therapy in combined group,acupuncture alone in acupuncture group under the routine therapy.The patient's mood was assessed using the Hamilton depression scale (HAMD),the Mental state examination scale (MMSE) was used to assess the patient's cognitive function and Barthel index rating scale to assess the daily activities at 4 and g weeks after the treatment.Results HAMD was significantly lowered at 4 and 8 weeks aftter the treatment than that before the treatment (P＜0.01),more obviously in combined group (P＜0.05).There was no significant difference in MMSE at 4 weeks affter the treatment as compared with pre-treatment in the two groups (P＞0.05),with significant difference at 8 weeks after the treatment (P＜0.05),more obviously in combined group (P＜0.05).Barthel index of the two groups was significantly higher at 4 and 8 weeks after the treatment than that before the treatment (P＜0.05),more obviously in combined group (P＜0.05).Conclusion The efficacy of acupuncture combined with vibroacoustic therapy is better than that of acupuncture alone,in improving the post-stroke depression,promoting the recovery of neurological functiou and improving the quality of life aftter stroke.
