合并深静脉血栓患者行下肢闭合骨折复位内固定手术麻醉的回顾性分析

Retrospective Analysis of Anesthesia in patients with Deep vein Thrombosis Undergoing Reduction and Internal Fixation of Closed Fracture of Lower Limbs

目的探究合并深静脉血栓患者行下肢闭合骨折复位内固定手术麻醉的情况,总结临床经验。方法将该院2016年3月—2017年3月间收治的200例患者作为研究对象,患者均为合并深静脉血栓且行下肢闭合骨折复位内固定手术;按照麻醉方法不同将其分为对照组(气管插管全麻)和观察组(腰硬联合阻滞),回顾分析两组麻醉效果。结果麻醉实施前两组心率、血压均无明显差异,麻醉后两组差异明显,观察组恢复室停留时间较短、医师满意度较高;两组比较均为观察组显著较好,差异有统计学意义(P<0.05)。结论回顾性分析合并深静脉血栓患者行下肢闭合骨折复位内固定手术麻醉情况,可见腰硬联合麻醉的效果相对较好,且对于患者影响较小,故值得临床推广应用。

Objective To investigate the anaesthesia of closed fracture of lower extremity by reduction and internal fixation in patients with deep venous thrombosis and summarize the clinical experience. Methods 200 patients admitted to our hospital from March 2016 to March 2017 were all treated with deep venous thrombosis and underwent closed fracture reduction and internal fixation of lower extremity. According to the anesthetic methods, they were divided into two groups:control group(general anesthesia with tracheal intubation) and observation group(combined spinal-epidural block). Results There was no significant difference in heart rate and blood pressure between the two groups before anesthesia, but there was significant difference between the two groups after anesthesia. The difference between the two groups was significant(P<0.05). Conclusion Retrospective analysis of patients with deep venous thrombosis undergoing closed fracture of lower extremity reduction and internal fixation anesthesia, we can see that the effect of combined spinal-epidural anesthesia is relatively good, and the impact on patients is relatively small, so it is worthy of clinical application.
