输尿管囊肿治疗方式的探讨

Ureterocele in Infants and Children-A clinical evaluation of surgical management

输尿管囊肿系输尿管远端向膀胱内之囊性膨出.小儿单纯型少见,绝大多数并有肾及输尿管重复畸型.我们曾治疗过16例.现以此为基础着重对治疗进行探讨.

This paper presents the authors'experience in the management of 14 children,age 6 mons to 12 yrs,with ectopic ureterocele.No death was reported in our series.Excision of the ureterocele or unroofing technique through a lower approach was performed on 5 of the patients.Heminephrectomy and subtotal ureterectomy plus transvesical excision of the ureterocele or unroofing technique was employed in 7 cases.The ureterocele was left intact so as to observe the possibility of spontaneous collapse.Otherwise,a second stage transvesical excision of ureterocele or unroofing procedure will be necessary.Our conclusion confirms that most ureteroceles can be managed by the simplified upper tract approach.However,traditional procedure of transvesical excision or unroofing of the ureterocele may prove useful sometimes in septic cases as a temporarizing procedure.
