Experience in Surgical Treatment of Legg-Perthes' Disease

Legg-Pertles'病的手术治疗,近年来国内已屡有文章发表,介绍应用各种手术方法以图解除关节内压力、促进股骨头骨骺的静脉血液回流和坛加头骺的血供来治疗本病(1～6).据各学者的近、远期报道(1～4)在临床上和X线片表现均可取得满意的疗效,我科自1981～1985年对本病应用滑膜切除、血管束植入术治疗共37例,现将我们的病例、经验教训和体会介绍如下.

This paper reports 37 cases of Legg-Per-thes' disease treated by synovectomy and vas-cular transplantation, of which 28 cases were followed up for 1 year and 2 months. Clinically, 23 cases showed excellent or good results. The femoral epiphyseal index measured 28. 6-9.8 preoperatively and 30.4-9.9 postoperatively (p>0.1), showing no statistical difference.We hold that primary Legg-Perthes' dise-ase is one type of avascular necrosis syndrome of the femoral head and is different from ava-scular necrosis of the femoral head secondary to definite etiology. Our operations achieved satisfactory clinical results and epiphyseal re-pair. However, improvement of the epiphyseal index was not remarkable. The femoral epiph-ysis remained flat in most cases, which calls for further researches. The cut of adductor longus tendon is conducive to the restoration of hip function.
