时辰化疗治疗晚期非小细胞肺癌的疗效及不良反应观察摘要

Hour the curative effect of treatment of advanced non-small cell lung cancer chemotherapy and to observe the adverse reaction

目的：本研究通过比较用设定固定时间并按正弦曲线式时间调节静滴化疗药物的治疗方法和常规化疗的方法，来观察对晚期非小细胞肺癌的近期疗效及其不良反应。方法74例晚期非小细胞肺癌患者，全部病例均是病理诊断为非小细胞肺癌患者。采用第七版AJCC分期法，纳入局部晚期的患者，分期为Ⅳ期，有客观观察指标，年龄<70岁，采用Zubrod-ECOG-WHO（ZPS，5分法）评分：0-2分，血常规、肝功、肾功检测值在正常值范围内，签订知情同意书。结果常规治疗组（A组），治疗有效率为97%(37/38)，时辰化疗（B组）治疗有效率为97%（35/36），两组经卡方统计分析后P＞0.05，两组有效率没有统计学差异。然而在毒副反应方面，A组血小板减少的发生率高达92%；，B组血小板减少发生率为59%，，经卡方检验P值＜0.05.有显著的统计学差异。其他的不良反应两组之间无明显差异。结论时辰化学疗法对晚期肺癌使用紫杉醇联合卡铂治疗，在减轻血小板减少有明显的优势，该方法临床操作简单易行，值得临床推广应用以及进一步发掘时辰化学疗法在临床肺癌治疗的价值，尤其值得进一步观察其远期疗效。

Objective This study by comparing with set a fixed time and time adjustment according to the sine curve type static drop chemotherapy drug treatment and the method of standard chemotherapy, to observe for advanced non-small cell lung cancer in the near future curative effect and adverse reactions. Methods 74 patients with advanced non-small cell lung cancer, all cases is the pathological diagnosis of non-small cell lung cancer. The seventh edition AJCC staging method is adopted, in patients with locally advanced stage forⅣ, have objective observation index, age<70 years old, the Zubrod-ECOG-WHO (ZPS, five points method) score:0 to 2 points, routine blood, liver, kidney function values within the normal range, signed the informed consent. Results The routine treatment group (group A), the treatment effective rate was 97%(37/38), hour chemotherapy (group B) effective rate was 97%(35/36), two groups after chi-square statistic analysis (P>0.05, two groups of efficient no statistical differences. In the aspect of adverse reaction, however, A group, the incidence of thrombocytopenia is as high as 92%; The incidence of thrombocytopenia, group B was 59%, and the chi-square test P values<0.05. The differences were statistically significant. Other adverse reactions have no obvious difference between two groups. Conclusion Hour chemotherapy for advanced lung cancer treated with paclitaxel plus carboplatin, has obvious advantages in ease thrombocytopenia, clinical operation is simple, the method is worthy of clinical application and further explore hour chemotherapy in clinical value for lung cancer treatment, especially deserves further observe the long-term curative effect.
