血清 Cys-C、hs-CRP 及 NGAL 联合检测在Ⅱ型糖尿病早期肾病诊断中的价值 被引量:5

摘要 目的:探讨胱抑素 C(Cys-C)、超敏 C 反应蛋白(hs-CRP)、中性粒细胞明胶酶相关载脂蛋白(NGAL)联合检测在早期糖尿病肾病(DN)诊断中的应用。方法选取2014年1月~2015年1月在我院就诊的Ⅱ型糖尿病(T2DM)患者84例作为研究对象,按尿微量白蛋白排泄率(UAER)分为单纯糖尿病组(A 组,n=21)、早期 DN 组(B 组,n=29)、DN 组(C 组, n=34),选择同期在我院体检者30例为健康对照组。采用全自动生化分析仪检测受检者肝肾功能指标,并用 ELISA 法检测受检者血清 NGAL 水平。结果 A 组肝肾功能指标与对照组比较无显著性差异(P>0.05);B 组血清肌酐(Scr)、尿素(Urea)测定结果与 A 组和对照组无显著性差异(P>0.05),而 Cys-C、hs-CRP 及 NGAL 测定结果显著高于 A 组和对照组(P<0.05);C 组与 A、B 两组及对照组比较,各指标均有显著性差异(P<0.05);T2DM 患者出现早期肾功能损伤时,Cys-C、hs-CRP、NGAL 的异常检出率显著高于 Scr 及 Urea(P<0.05)。结论联合检测 Cys-C、hs-CRP 及 NGAL 有助于诊断早期肾损伤,对控制糖尿病肾病的发展有重要的临床意义。 Objective To explore the early diagnostic value of plasma Cys -C, hs -CRP and NGAL in diabetic nephropathy. Methods 84 patients with type 2 diabetes mellitus from Jan 2014 to Jan 2015 in our hospital were divided into 3 groups according to urinary albumin excretion rate (UAER):simple diabetic mellitus group (group A, n=21), early diabetic nephropathy group (group B, n=29), clinical diabetic nephropathy group (group C, n=34). 30 healthy subjects in the same pe-riod determined by physical examination were enrolled as control group. Liver and kidney function were detected by automatic analyzer, and the level of plasma NGAL was deceted by ELISA in all the subjects. Results Liver and kidney function index of group A had no significant differece compared with control group(P<0.05). Scr and Urea in group B had no significant differ-ence compared with group A and control group (P>0.05), but Cys-C, hs-CRP, NGAL in group B were significant higher than group A and control group (P<0.05). Each index of group C had significant difference compared with group A, group B and control group. Serum Cys-C, hs-CRP and NGAL had higher abnormal detection rate in early renal damage of type 2 diabetes compared with Scr and Urea (P<0.05). Conclusion The combined detection of Cys-C, hs-CRP and NGAL is helpful to the early diagnosis of renal injury, it has certain clinical value to control the development of DN.
