抗高血压治疗与老年人认知功能得分的相关性研究

Correlation between antihypertensive therapy and cognitive function of the elderly

目的：研究抗高血压药物治疗对老年患者认知功能的影响，并分析两者相关性。方法选择2012年1月~2013年1月在我院接受药物治疗的老年高血压患者（年龄≥60岁）75例为观察组。再选择同期在我院诊断为高血压但未进行药物治疗的老年患者75例为对照组。对比两组老年高血压患者认知功能得分以及生活质量评分，并对引起老年高血压患者认知功能障碍的相关因素进行Logistic回归分析。结果观察组定向力等5项认知功能评分以及MMSE总评分均显著高于对照组，并且观察组所有生活质量评分高于对照组，差异均有统计学意义（P＜0.05）。根据Logistic回归分析，得到4个老年高血压患者认知功能的影响因素，即教育程度、高血压病程、是否使用抗高血压药物治疗以及同型半胱氨酸（HCY）水平。结论老年人使用抗高血压药物治疗后能明显提高认知功能和生活质量，适当药物治疗能降低老年人认知功能障碍的发生率。同时老年人受教育程度、高血压病程以及HCY水平也会在一定程度上对其认知功能产生影响，临床上应引起足够的重视。

Objective To explore the effect of antihypertensive drug therapy to the cognitive function of the elderly hy-pertension patients and analyze the correlation between them. Methods 75 patients with elderly hypertension aged 60 or above who received drug treatment in our hospital from Jan 2012 to Jan 2013 were selected as observation group.75 cases diag-nosed as elderly hypertension treated without drugs in our hospital in the same period were selected as the control group.Com-pared the scores of cognitive function and quality of lives between the two groups ,and Logistic regression analysis was conducted to analyze the related factors of cognitive dysfunction in elderly patients with hypertension. Results In the observation group, the scores of cognitive function and the total score of MMSE were significantly higher than those in the control group ,and all the scores of quality of lives in the observation group were higher than those in the control group ,the difference was statistically sig-nificant(P<0.05).Logistic regressive analysis showed that there were four influence factors of cognitive function of the elderly pa-tients with hypertension,including education degree,the course of hypertension,the use of antihypertensive drugs and HCY level. Conclusion Antihypertensive therapy can significantly improve the cognitive function and quality of lives of elderly hyperten-sion patients,and appropriate drug treatment can reduce the incidence of cognitive dysfunction.At the same time,the education level,the course of hypertension and the HCY level have certain effect on the cognitive function of the elderly patients ,which should be paid enough attention in clinic.
