肱骨干骨折合并桡神经损伤不同治疗方法的临床对比研究

Comparative study on the clinical effect of humeral shaft fracture with radial nerve injury patients treated with different methods

目的：对比分析保守治疗、早期桡神经探查两种不同方法治疗肱骨干骨折合并桡神经损伤的临床效果。方法对2013年1月~2016年3月我院收治的65例肱骨干骨折合并桡神经损伤患者的资料进行分析，将其按照不同的治疗方法分组，A组（n=33）给予早期桡神经探查，B组（n=32）给予保守治疗。对两组患者均进行为期5~18个月的随访，比较两组患者的神经功能恢复优良率、骨折愈合时间、神经功能损伤开始恢复时间及完全恢复时间。结果 A组神经功能恢复优良率、神经功能损伤开始恢复时间及完全恢复时间与B组比较差异无统计学意义（P＞0.05）。A组骨折愈合时间与B组比较更短，两组差异有统计学意义（P＜0.05）。结论保守治疗及早期桡神经探查两种不同方法治疗肱骨干骨折合并桡神经损伤均可收到良好的临床效果，神经功能恢复良好，但早期桡神经探查骨折愈合时间更短。

Objective To compare the clinical effect of humeral shaft fracture with radial nerve injury patients treated with different methods of conservative treatment,early radial nerve exploration. Methods 65 cases of humeral shaft fracture with radial nerve injury patients were selected from Jan 2013 to Mar 2016 in our hospital ,all the patients were divided into two groups according to different methods of treatment,group A (n=33) administered early radial nerve exploration,group B (n=32) was given conservative treatment. Two groups of patients were followed up for a period of 5~18 months ,the neurological function recovery excellent rate,fracture healing time,neurological damage begin to recover time and fully recover time of two groups were compared. Results The neurological function recovery excellent rate,neurologic damage begin to recover time,and full recov-ery time of group A compared with group B,there were no significant differences between two groups(P>0.05).The fracture heal-ing time of group A was shorter than group B,the difference was statistically significant(P<0.05). Conclusion Early conser-vative treatment of radial nerve exploration of two different methods for treating humeral shaft fracture with radial nerve injury can receive good clinical results,good recovery of neurological function,but early exploration of the radial nerve fracture healing time is shorter.
