摘要
目的探讨MOTOmed功能训练对创伤性膝关节挛缩患者关节功能恢复的意义。方法选取2015年1月至2016年1月在安徽医科大学第二附属医院康复医学科就诊的以创伤性膝关节挛缩为第一诊断的患者共37例,其中股骨干骨折11例,股骨远端骨折10例,髌骨骨折6例,胫骨平台骨折10例,将符合入选标准的37例膝关节挛缩患者分为治疗组和对照组,治疗组19例,对照组18例,均按照膝关节挛缩的治疗方案正规治疗,对照组行常规康复治疗,治疗组在此基础上同时行MOTOmed功能训练治疗,出院时比较两组患者膝关节活动度和步速。利用配对样本t检验比较两组患者治疗前和治疗后有无差异,其中膝关节活动度以膝关节屈曲角度(°)表示,步速以cm/s表示。结果两组患者治疗前关节活动度[治疗组:(45.826 3±6.785 18)°;对照组:(45.922 2±6.479 61)°]和步速[治疗组:(97.742 1±6.541 52)cm/s;对照组:(97.377 8±6.912 18)cm/s]差异无统计学意义(P>0.05)。两组治疗后膝关节活动度[治疗组:(69.352 6±6.744 90)°;对照组:(57.488 9±6.877 40)°]和步速[治疗组:(111.968 4±6.365 36)cm/s;对照组:(103.738 9±7.373 65)cm/s]与治疗前膝关节活动度和步速差异有统计学意义(P<0.05)。治疗组和对照组经治疗后关节活动度和步速差异有统计学意义(P<0.05)。结论 MOTOmed功能训练可有效改善创伤性膝关节挛缩中的膝关节活动度,提高步行能力,且与传统的常规康复治疗相比有利于节省人力和时间,但远期效果仍需进一步研究。
Objective To observe the effect of MOTOmed functional training on the recovery of joint function in patients with traumatic knee joint contracture. Methods A total of 37 patients with traumatic knee contracture diagnosed as the first diagnosis were selected from the Department of Rehabilitation Medicine of the Second Affiliated Hospital of Anhui Medical University from January, 2015 to January, 2016, including 11 cases of femoral shaft fracture, 10 cases of femur fracture, 6 cases of patellar fracture and 10 cases of tibial plateau fracture, were divided into the treatment group and the control group, with 19 cases in the treatment group and 18 cases in the control group. All of them were treated in accordance with the treatment of joint contracture. The control group received routine rehabilitation therapy, and the treatment group was treated with MOTOmed function training at the same time. At the time of discharge, the range of motion and the walking speed were compared between the two groups. The paired samples t test was used to compare the differences between the two groups before and after treatment. The knee joint range of motion was expressed in the knee flexion angle (°), and the walking speed was expressed in cm/s. Conclusions MOTOmed function training can effectively improve the range of motion and the ability to walk, what's more, it is beneficial to save labor and time compared with the traditional rehabilitation treatment. Further investigation is needed for evaluating the long-term effect of MOTOmed function training.
