北京市EV-D68型肠道病毒引起的急性呼吸道感染 被引量:7

Acute respiratory tract infections associated with Enterovirus D68 in Beijing

摘要 目的 了解北京地区呼吸道感染病例中EV-D68型肠道病毒感染情况,发现北京地区EV-D68型肠道病毒的流行规律,为呼吸道传染病预防控制策略的制定提供依据.方法 从监测哨点医院采集呼吸道感染病例的咽拭子、痰液、肺泡灌洗液等标本,利用实时荧光PCR方法,筛查肠道病毒,利用EV-D68型肠道病毒特异性引物对肠道病毒核酸检测阳性标本进行RT-PCR扩增和测序.测序结果在NCBI中进行BLAST,分析肠道病毒血清型,并进行进化分析.结果 从2015-2016年6月份的10 494例呼吸道感染病例病原学标本中,检测到330例肠道病毒核酸阳性标本,其中两例为EV-D68型特异性引物扩增阳性,经测序证实其为EV-D68血清型.对VP1区的序列进化分析结果表明,两株EV-D68肠道病毒均属于3个已知的进化分支中的亚系1.结论 北京地区存在EV-D68型肠道病毒的流行,并可引起呼吸道感染,流行的EV-D68均属于亚系1,与国际上主要流行的亚系一致.本研究结果为阐明EV-D68病毒的流行特点提供了数据参考. Objective To investigate the respiratory infection associated with enterovirus D68 in Beijing,learn the epidemic regularity of enterovirus D68,and provide scientific basis for the work of respiratory illness prevention and control.Methods The oropharyngeal swabs,sputum and alveolar lavage fluid from the respiratory illness cases in were collected monitoring sentinel hospitals.The Enterovirus was detected by real-time RT-PCR.The gene fragments of enterovirus specimens were amplified by a pair of specific primers for enterovirus D68.The sequences were analyzed in NCBI by BLAST method to determine the serotype and genetic analysis.Results In total of 10 494 specimens from respiratory illness cases,330 cases were positive for enterovirus.Of the 330 cases in whom enterovirus was confirmed,two cases were associated with enterovirus D68 by RT-PCR test.The sequences of PCR products further confirmed that the two viruses were enterovirus D68.Alignment of phylogenetic analysis of VP1 gene sequences showed that these viruses clustered and belonged to clade lineage 1.Conclusions The study confirmed that there were respiratory illness cases infected by enterovirus D68 in Beijing.The EV-D68 strains since 2014 all belonged to lineage 1,which was the dominant sublineage epidemic worldwide.
