解脲脲原体感染与宫颈上皮内瘤变的相关性

Correlation of ureaplasma urealyticum infection and cervical intraepithelial neoplasm

目的：探究解脲脲原体感染与宫颈上皮内瘤变（CIN）的相关性。方法选取2012年9月至2015年4月本院经病理组织学确诊的CIN患者共110例，其中CIN Ⅰ期患者48例，CIN Ⅱ期患者40例，CINⅢ期患者22例。随访观察110例CIN患者3年内病变情况，其中85例（77.27%）未发生癌变，25例（22.73%）转变为宫颈癌；选取同期110例宫颈上皮正常者作为对照组，分析解脲脲原体（Uu）感染与宫颈上皮内瘤变的相关性。结果不同级别的CIN患者Uu感染率相比，CIN Ⅰ期、CIN Ⅱ期和CINⅢ期组患者中Uu感染者显著多于无Uu感染者（χ2=6.000、16.200、5.818，P =0.01、0.00、0.02）。不同级别CIN患者分别与对照组比较，CINⅠ期、CINⅡ期、CINⅢ期组患者Uu感染率均显著高于对照组，差异均具有统计学意义（χ2=9.251、15.421、7.636，P =0.01、0.00、0.01）。进展性癌变患者Uu感染率显著高于未癌变患者（χ2=4.112、P =0.043）。结论解脲脲原体与CIN相关性较高，临床上减少Uu感染可更好地控制CIN的发生，并减少其发展为宫颈癌的几率。

Objective To investigate the correlation of ureaplasma urealyticum (Uu) infection and cervical intraepithelial neoplasm (CIN).Methods Total of 110 CIN patients diagnosed by pathology in our hospital from September 2012 to April 2015 were selected, including 48 cases of CINⅠ, 40 cases of CINⅡ and 22 cases of CINⅢ. All the CIN patients were followed-up for 3 years, 85 cases (77.27%) were not cancerous, but the other 25 cases (22.73%) developed to cervical cancer, while 110 healthy volunteers were selected as control group. The correlation of Uu infection and cervical intraepithelial neoplasm was analyzed. Results The rates of Uu infection in different levels of CIN patients were compared, cases with CINⅠ, CINⅡ and CINⅢ were signiifcantly more than cases without Uu infection (χ2 = 6.000, 16.200, 5.818;P = 0.01, 0.00, 0.02). The Uu infection rate of patients with CINⅠ, CINⅡ and CINⅢ were all signiifcantly higher than that of the control group (χ2 = 9.251, 15.421, 7.636;P = 0.01, 0.00, 0.01). The rate of Uu infection in patients with cervical cancer was higher than that of patients without cancer, with signiifcant difference (χ2 = 4.112,P = 0.043).Conclusions There was signiifcant correlation between Uu infection and CIN, and reduction of Uu infection was beneiftial to control the occurrence and development to cancer of CIN.
