In this paper, we analyze the association between daily hospital admissions for asthma and ambient air pollution concentrations in four New York City counties. Negative binomial regression is used to model the association between daily asthma hospital admissions and ambient air pollution concentrations. Potential confounding factors such as heat index, day of week, holidays, yearly population changes, and seasonal and long-term trends are controlled for in the models. Nitrogen dioxide (NO2), sulfur dioxide (SO2) and carbon monoxide (CO) show the most consistent statistically significant associations with daily hospitalizations for asthma during the entire period (1996-2000). The associations are stronger for children (0 - 17 years) than for adults (18 - 64 years). Relative risks (RR) for the inter-quartile range (IQR) of same day 24-hour average pollutant concentration and asthma hospitalizations for children for the four county hospitalization totals were: NO2 (IQR = 0.011 ppm, RR = 1.017, 95% CI = 1.001, 1.034), SO2 (IQR = 0.008 ppm, RR = 1.023, 95% CI = 1.004, 1.042), CO (IQR = 0.232 ppm, RR = 1.014, 95% CI = 1.003, 1.025). In the case of ozone (O3) and particulate matter (PM2.5) statistically significant associations were found for daily one-hour maxima values and children’s asthma hospitalization in models that used lagged values for air pollution concentrations. Five-day weighted average lag models resulted in these estimates: O3 (one-hour maxima) (IQR = 0.025 ppm, RR = 1.049, 95% CI = 1.002, 1.098), PM2.5 (one-hour maxima) (IQR = 16.679 μg/m3, RR = 1.055, 95% CI = 1.008, 1.103). In addition, seasonal variations were also explored for PM2.5 and statistically significant associations with daily hospital admissions for asthma were found during the colder months (November-March) of the year. Important differences in pollution effects were found across pollutants, counties, and age groups. The results for PM2.5 suggest that the composition of PM is important to this health outcome, since the major sources of NYC PM differ between winter and summer months.展开更多

The aim of this article is to highlight any gaps in knowledge regarding represented demographics in these treatment studies, and to discuss the current and upcoming treatment options. Background: This literature review explores under-researched psychiatric conditions: Cotard’s syndrome, Alice in Wonderland syndrome, and Catatonic Schizophrenia. Understanding psychiatric disorders requires basic knowledge of brain anatomy. These conditions are often result of or associated with neurological issues, such as migraines or tumors. The brain has eight lobes, two of four kinds: frontal, parietal, occipital, and temporal lobes, which all govern different functions and abilities. Frontal lobes control judgment, decision-making, personality traits, and fine motor movements. Parietal lobes interpret pain and temperature, occipital lobes handle visual stimuli, and temporal lobes enable hearing. The pre-frontal cortex is associated with high intelligence, psychotic traits, and psychosis. The Broca’s Area in the frontal lobes controls expressive language. These areas and divisions of the brain contribute to the complexity of the psychiatric disorders discussed in this review. Introduction: Cotard’s syndrome is a psychiatric disorder characterized by delusions of being dead or not having certain limbs or organs. It is believed that there is a disconnect between their fusiform face area and the amygdala, causing a lack of familiarity between one’s mind and body. Alice in Wonderland Syndrome (AIWS) is another psychiatric disorder which is characterized by visual hallucinations, such as distorted perceptions of color, size, distance, and speed. The most common symptoms include micropsia and macropsia. Catatonia/Catatonic Schizophrenia is an uncommon type of schizophrenia. This type of schizophrenia is characterized by motor rigidity, verbal rigidity, the flat effect, psychomotor retardation, waxy flexibility, and overall negative symptoms. Thus, these people may come off as emotionally detached, and able to stay frozen in odd positions for periods on end. Treatments and Results: Cotard’s syndrome seemed to be most effectively treated by ECT (electroconvulsive therapy). Alice in Wonderland Syndrome (AIWS) had the highest positive responses to treatment by Valproate (an anti-epileptic drug), as well as intervention to treat the associated neurological conditions they had. Catatonia/Catatonic Schizophrenia seemed to be most effectively treated with a combination of benzodiazepines and ECT. Discussion and Demographics: In all 3 disorders, the Latino and African communities were underrepresented. There also seemed to be an underrepresentation of men in Cotard’s syndrome, and of women in Alice in Wonderland Syndrome. Japan and India seemed to have the highest density of treatment studies in all 3 disorders.展开更多

Ryan asserts that Philo’s defense of this claim, as set out in the third and fourth paragraphs in Part 2, has not received the attention it deserves from commentators. This, according to Ryan, is surprising, since, if Philo is successful here, he will have effectively shut down the project of natural theology, before it can even begin. Ryan goes further, citing my work, Scepticism and Belief in Hume’s Dialogues Concerning Natural Religion (The Hague: Martinus Nijhoff, 1986) to say that Philo’s challenge receives no direct answer from Cleanthes. Instead, Cleanthes proceeds directly to formulate his version of the Argument from Design. Ryan concludes his introductory remarks by asserting that, in these two paragraphs, Philo offers not one, but two, distinct arguments for the conclusion that we can achieve no strictly philosophical knowledge of the nature of God. He then proceeds to flesh out these two arguments, and attempts to show that neither of Philo’s objections is ultimately telling against Cleanthes’ experimental natural theology.展开更多

文摘Named Entity Recognition(NER)stands as a fundamental task within the field of biomedical text mining, aiming to extract specific types of entities such as genes, proteins, and diseases from complex biomedical texts and categorize them into predefined entity types. This process can provide basic support for the automatic construction of knowledge bases. In contrast to general texts, biomedical texts frequently contain numerous nested entities and local dependencies among these entities, presenting significant challenges to prevailing NER models. To address these issues,we propose a novel Chinese nested biomedical NER model based on RoBERTa and Global Pointer(RoBGP).Our model initially utilizes the RoBERTa-wwm-ext-large pretrained language model to dynamically generate word-level initial vectors. It then incorporates a Bidirectional Long Short-Term Memory network for capturing bidirectional semantic information, effectively addressing the issue of long-distance dependencies.Furthermore, the Global Pointer model is employed to comprehensively recognize all nested entities in the text.We conduct extensive experiments on the Chinese medical dataset CMeEE and the results demonstrate the superior performance of RoBGP over several baseline models. This research confirms the effectiveness of RoBGP in Chinese biomedical NER,providing reliable technical support for biomedical information extraction and knowledge base construction.
文摘Purpose: Cotard’s syndrome, Alice in Wonderland Syndrome, and Catatonia are all rare psychiatric disorders that have relatively little research regarding their treatments. Japan and India seemed to have the highest density of treatment studies in all 3 disorders.

文摘Photoinduced intermolecular charge transfer(PICT)determines the voltage loss in bulk heterojunction(BHJ)organic photovoltaics(OPVs),and this voltage loss can be minimized by inducing efficient PICT,which requires energy-state matching between the donor and acceptor at the BHJ interfaces. Thus, both geometrically and energetically accessible delocalized state matching at the hot energy level is crucial for achieving efficient PICT.In this study,an effective method for quantifying the hot state matching of OPVs was developed. The degree of energy-state matching between the electron donor and acceptor at BHJ interfaces was quantified using a mismatching factor(MF)calculated from the modified optical density of the BHJ.Furthermore, the correlation between the open-circuit voltage(Voc)of the OPV device and energy-state matching at the BHJ interface was investigated using the calculated MF.The OPVs with small absolute MF values exhibited high Voc values. This result clearly indicates that the energy-state matching between the donor and acceptor is crucial for achieving a high Voc in OPVs.Because the MF indicates the degree of energy-state matching, which is a critical factor for suppressing energy loss,it can be used to estimate the Voc loss in OPVs.
文摘BACKGROUND Tobacco use is a well-documented modifiable risk factor for perioperative complications.AIM To determine the tobacco abstinence rates of patients who made cessation efforts prior to a total joint arthroplasty(TJA)procedure.METHODS A retrospective evaluation was performed on 88 self-reported tobacco users who underwent TJA between 2014-2022 and had tobacco cessation dates within 3 mo of surgery.Eligible patients were contacted via phone survey to understand their tobacco use pattern, and patient reported outcomes.A total of 37 TJA patients participated.RESULTS Our cohort was on average 61-years-old,60%(n=22)women,with an average body mass index of 30 kg/m^(2).The average follow-up time was 2.9±1.9 years.A total of 73.0%(n=27)of patients endorsed complete abstinence from tobacco use prior to surgery.Various cessation methods were used perioperatively including prescription therapy(13.5%),over the counter nicotine replacement(18.9%),cessation programs(5.4%).At final follow up,43.2%(n=16)of prior tobacco smokers reported complete abstinence.Patients who were able to maintain cessation postoperatively had improved Patient-Reported Outcomes Measurement Information System(PROMIS)-10 mental health scores(49 vs 58;P=0.01),and hip dysfunction and osteoarthritis outcome score for joint replacement(HOOS.JR)scores(63 vs 82;P=0.02).No patients in this cohort had a prosthetic joint infection or required revision surgery.CONCLUSION We report a tobacco cessation rate of 43.2%in patients undergoing elective TJA nearly 3 years postoperatively.Patients undergoing TJA who were able to remain abstinent had improved PROMIS-10 mental health scores and HOOS.JR scores. The perioperative period provides clinicians a unique opportunity to assist active tobacco smokers with cessation efforts and improve postoperative outcomes.

文摘Metabolic dysfunction-associated fatty liver disease(MAFLD)is the most prevalent chronic liver condition worldwide.Current liver enzyme-based screening methods have limitations that may missed diagnoses and treatment delays.Regarding Chen et al,the risk of developing MAFLD remains elevated even when alanine aminotransferase levels fall within the normal range. Therefore, there is an urgent need for advanced diagnostic techniques and updated algorithms to enhance the accuracy of MAFLD diagnosis and enable early intervention. This paper proposes two potential screening methods for identifying individuals who may be at risk of developing MAFLD:Lowering these thresholds and promoting the use of noninvasive liver fibrosis scores.
文摘I begin my comments by acknowledging that Professor Ryan is correct when he states early in his paper that Philo makes common cause with Demea by arguing that philosophical reflection can yield no knowledge of the nature or essence of the divine being. He then proceeds to flesh out these two arguments, and attempts to show that neither of Philo’s objections is ultimately telling against Cleanthes’ experimental natural theology.

文摘Magnesium(Mg)and its alloys have recently gained increasing attention in the biomedical field as promising biodegradable materials with harmless degradation products.Magnesium-based alloys have a wide range of biomedical applications because of their outstanding biocompatibility and unique mechanical properties.Widespread use of Mg-based biomedical devices eliminates the need for post-healing biomaterial removal surgery and minimizes the negative consequences of the implantation of permanent biomaterials, including stress shielding and undesired metal ion release in the body. This paper provides a literature review on the properties and manufacturing methods of Mgbased alloys for biomedical applications, including orthopedic implants, cardiovascular applications, surgical wires and staplers, and antitumor activities. Each application of Mg-based biomaterials is investigated from a biological perspective, including matching functional properties, biocompatibility, host tissue responses, and anti-microbial strategies, along with potential additive manufacturing technologies for these applications. Finally,an outlook is presented to provide recommendations for Mg-based biomaterials in the future.
文摘Appropriate oral hygiene significantly reduces the possibility of oral infections. However, dental caries and periodontal diseases are major oral health issues causing chronic diseases due to poor oral health.Recently, herbal compounds have gained interest in maintaining oral health.Extracts of burdock root(Arctium),noni fruit(Morinda citrifolia),and neem leaf(Azadirachta indica)are now used as intracanal medicaments in endodontics and periodontics.Plectranthus amboinicus species and other plants produces essential oil likeβ-caryophyllene,p-cymene,andγ-terpinene can exhibit antibacterial activity;highlighting phytoconstituents plays a vital role in oral health. The COVID-19 pandemic highlighted the importance of hygiene and sanitization,to curb SARS-CoV-2.Oral cavity is among the gateways for virus entry into saliva.Saliva is a potential reservoir of SARS-CoV-2,and there is an increased risk of infection if there is any fissure in the mouth. This enables entry of virus into the vascular system through gingival or periodontal pocket, possibly reaching lung periphery then to lung vessels by interacting with endothelial surface receptors triggering pulmonary vasoconstriction and lung damage due to endothelial dysfunction. This review aims to draw attention to the possible route of SARS-CoV-2 infection via the oral cavity and the importance of oral hygiene against COVID-19.

文摘BACKGROUND Hepatitis C is the leading cause of chronic liver disease worldwide and it significantly contributes to the burden of hepatocellular carcinoma(HCC).However, there are marked variations in the incidence and mortality rates of HCC across different geographical regions. With the advent of new widely available treatment modalities, such as direct-acting antivirals,it is becoming increasingly imperative to understand the temporal and geographical trends in HCC mortality associated with Hepatitis C.Furthermore, gender disparities in HCC mortality related to Hepatitis C are a crucial, yet underexplored aspect that adds to the disease's global impact. While some studies shed light on gender-specific trends, there is a lack of comprehensive data on global and regional mortality rates, particularly those highlighting gender disparities. This gap in knowledge hinders the development of targeted interventions and resource allocation strategies.DISCUSSION The results of our study show an overall decline in the mortality rates of patients with hepatitis C-related HCC over the last two decades.Notably, females exhibited a remarkable decrease in mortality compared to males.Regionally,East Asia and the Pacific displayed a significant decline in mortality, while Europe and Central Asia witnessed an upward trend.Latin America and the Caribbean also experienced an increase in mortality rates. However,no significant difference was observed in the Middle East and North Africa.North America exhibited a notable upward trend.South Asia and Sub-Saharan Africa significantly declined throughout the study period. This raises the hope of identifying areas for implementing more targeted resources.Despite some progress, multiple challenges remain in meeting the WHO 2030 goal of eliminating viral hepatitis[24].
