联用多步骤虚拟筛选方法发现具有新母核的GABAA受体正性变构调节剂

GABAA受体主要介导哺乳动物中枢神经系统的抑制性信号传递,是镇静催眠药的关键靶点。在寻找具有新母核的镇静催眠药的过程中,计算机辅助药物设计(CADD)方法显示出巨大的优势。在这项研究中,首先,我们通过机器学习模型、分子对接模型和分子力学广义玻恩比表面积(MMGBSA)方法筛选了来自于商业数据库中的41112种化合物。经过筛选,我们得到了16个化合物,然后我们通过全细胞膜片钳电生理学实验验证了4个结构新颖的化合物确实为有效的GABAA受体正性变构调节剂。其中,化合物GPR120在细胞水平和动物水平都得到了实验验证。在重组表达α1β2γ2型受体的皮层神经元中,在10和50µmol∙L^(−1)浓度下,GPR120可将GABA EC3-10电流分别增强71.5%和163.8%。通过全分解贡献分析和点突变实验,我们发现GPR120与GABAA受体结合的关键位点是H102,与阳性药物地西泮相似。为了进一步验证GPR120在动物水平上的功能,我们进行了运动活动测试和翻正反射消失(LORR)实验。GPR120对小鼠的运动活动有抑制作用,6 h后可恢复,说明GPR120是一种中度镇静剂。在戊巴比妥钠(PB)诱导的翻正反射消失实验中,与生理盐水组相比,GPR120(20 mg∙kg^(−1))可显著缩短开始LORR的时间并延长LORR的持续时间。综上所述,通过联用多种虚拟筛选方法,我们发现了GPR120是一种具有新型母核的中度强度镇静剂。 展开更多

目的研究miR-223调控Nod样受体蛋白3(Nod-like receptor protein3,NLRP3)抑制心力衰竭(heart failure,HF)大鼠心肌纤维化的机制。方法选取54只成年Wistar大鼠,采用腹主动脉缩窄法建立大鼠充血性HF动物模型,按照随机数字表法将其分为对... 目的研究miR-223调控Nod样受体蛋白3(Nod-like receptor protein3,NLRP3)抑制心力衰竭(heart failure,HF)大鼠心肌纤维化的机制。方法选取54只成年Wistar大鼠,采用腹主动脉缩窄法建立大鼠充血性HF动物模型,按照随机数字表法将其分为对照组、HF组、HF+miR-223过表达组,每组各18只。HF+miR-223过表达组尾静脉注射miR-223mimics,72h转染成功后,应用动物心脏彩超检测各组大鼠心功能相关指标,即舒张末期室间隔厚度(interventricular septal thickness at end-diastole,IVSd)、舒张末期左心室后壁厚度(left ventricular posterior wall thickness at end-diastolic,LVPWd)、左心室舒张末期内径(left ventricular end-diastolic diameter,LVEDd)、左心室收缩末期内径(left ventricular end systolic diameter,LVED)和射血分数(leftventricularejectfraction,LVEF)。计算测定心/体比、肺/体比;检测各组大鼠心肌组织中miR-223、NLRP3、白细胞介素-1β(interleukin-1β,IL-1β)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、Collagen-3、Collagen-1、Bax和Bcl-2的表达情况。结果HF组心功能较对照组更差,NLRP3、IL-1β、TNF-α、Collagen-1、Collagen-3和Bax表达高于对照组,miR-223、Bcl-2表达低于对照组(P<0.05),心肌间质纤维化程度明显加重。HF+miR-223过表达组心功能较HF组好转,NLRP3、IL-1β、TNF-α、Collagen-1、Collagen-3、Bax表达低于HF组,miR-223、Bcl-2表达高于HF组,心肌间质纤维化程度明显减轻。结论miR-223可通过作用于其靶标NLRP3抑制Collagen-1、Collagen-3的表达,在HE心肌纤维化的发生、发展过程中起到重要作用。 展开更多

Economical water electrolysis requires highly active non-noble electrocatalysts to overcome the sluggish kinetics of the two half-cell reactions,oxygen evolution reaction,and hydrogen evolution reaction.Although intensive efforts have been committed to achieve a hydrogen economy,the expensive noble metal-based catalysts remain under consideration.Therefore,the engineering of self-supported electrocatalysts prepared using a direct growth strategy on three-dimensional(3D)nickel foam(NF)as a conductive substrate has garnered significant interest.This is due to the large active surface area and 3D porous network offered by these electrocatalysts,which can enhance the synergistic eff ect between the catalyst and the substrate,as well as improve electrocatalytic performance.Hydrothermal-assisted growth,microwave heating,electrodeposition,and other physical methods(i.e.,chemical vapor deposition and plasma treatment)have been applied to NF to fabricate competitive electrocatalysts with low overpotential and high stability.In this review,recent advancements in the development of self-supported electrocatalysts on 3D NF are described.Finally,we provide future perspectives of self-supported electrode platforms in electrochemical water splitting. 展开更多

二苯甲酮是有机化学中最重要的结构单元之一,广泛存在于药物、香料、光敏剂以及许多天然产物中.然而,传统的二苯甲酮合成方法通常涉及添加剂或有毒试剂,且产生大量废弃物,不利于环保.苯甲醛与卤代苯的直接偶联反应因原料易得而备受关注... 二苯甲酮是有机化学中最重要的结构单元之一,广泛存在于药物、香料、光敏剂以及许多天然产物中.然而,传统的二苯甲酮合成方法通常涉及添加剂或有毒试剂,且产生大量废弃物,不利于环保.苯甲醛与卤代苯的直接偶联反应因原料易得而备受关注,但苯甲醛C(sp^(2))-H键的高键能(371 kJ mol^(-1))导致其活化困难,反应条件苛刻.此外,常用的Ni或Pd均相催化剂在反应体系中难以分离回收,限制了其应用.因此,构筑多相可回收催化剂成为研究热点.将活性钯金属负载到多相载体上进行酮合成,可以解决催化剂回收问题,但活性金属钯的浸出仍是挑战.本研究旨在探索制备有效的多相可回收催化剂,以推动二苯甲酮的环保合成.本文通过煅烧Pd^(2+)/MIL-125-NH_(2)制备了碳修饰二氧化钛负载的钯纳米颗粒(Pd/C-TiO_(2)),这是一种高效且可回收的多相催化剂.在紫外光照射下,该催化剂能有效催化苯甲醛和碘苯偶联合成二苯甲酮.研究结果表明,苯甲醛活化是该反应过程的控速步骤,而紫外光诱导生成的酰基自由基则是反应进行的关键.在不添加强氧化剂和配体的情况下,二苯甲酮的产率最高可达98%,选择性高达99%.通过电子顺磁共振技术检测到,反应过程中苯甲醛和碘苯均有可能生成碳自由基.利用密度泛函理论计算研究了基于碳自由基生成二苯甲酮的两种可能发生的路径,结果表明,相较于Pd0-PdI-PdIII过程(苯自由基进攻醛基自由基与钯加成的复合物),该反应更有可能通过Pd0-PdII-PdIII催化路径发生,即苯甲醛在紫外光下直接活化生成酰基自由基,然后进攻碘苯与钯氧化加成的复合物,最终通过还原消除生成二苯甲酮.醋酸钯、Pd/SiO_(2)催化剂等一系列对照试验结果表明,钯在反应体系中首先溶解并参与反应,随后在紫外光照射下被二氧化钛光催化剂原位还原,从而实现金属钯的原位沉积,有效抑制了钯金属的流失.催化剂在循环使用10次后,其活性没有明显下降.综上所述,本文利用高能紫外光直接活化苯甲醛与TiO_(2)光催化剂的高氧化还原性能相匹配,实现了紫外光下苯甲醛与碘苯偶联制备二苯甲酮和Pd催化剂的高效回收利用.本研究为在紫外光下活化苯甲醛与其他化合物偶联合成含羰基化合物提供了一种简便的方法,并为设计有机反应中可回收的钯催化剂提供了参考. 展开更多

Edge and lithium concentration effects on intercalation kinetics for graphite anodes

Graphite interfaces are an important part of the anode in lithium-ion batteries(LIBs),significantly influencing Li intercalation kinetics.Graphite anodes adopt different stacking sequences depending on the concentration of the intercalated Li ions.In this work,we performed first-principles calculations to comprehensively address the energetics and dynamics of Li intercalation and Li vacancy diffusion near the no n-basal edges of graphite,namely the armchair and zigzag-edges,at high Li concentration.We find that surface effects persist in stage-Ⅱ that bind Li strongly at the edge sites.However,the pronounced effect previously identified at the zigzag edge of pristine graphite is reduced in LiC_(12),penetrating only to the subsurface site,and eventually disappearing in LiC_(6).Consequently,the distinctive surface state at the zigzag edge significantly impacts and restrains the charging rate at the initial lithiation of graphite anodes,whilst diminishes with an increasing degree of lithiation.Longer diffusion time for Li hopping to the bulk site from either the zigzag edge or the armchair edge in LiC_(6) was observed during high state of charge due to charge repulsion.Effectively controlling Li occupation and diffusion kinetics at this stage is also crucial for enhancing the charge rate. 展开更多

BACKGROUND This work explored the effects of cognitive behavior therapy(CBT)-based comprehensive nursing intervention(CNI)mode in arch expansion to treat patients with orthodontic osteodilated arch(OOA).AIM To explore the application effect of CBT-based CNI model in orthodontic expansion arch treatment.METHODS Using convenient sampling method,81 patients with OOA were selected and rolled into a control group(Ctrl group,40 cases)and an observation group(Obs group,41 cases).During the treatment,patients in the Ctrl group received routine nursing intervention mode,and the those in the Obs group received CBT mode on the basis of this.Before and after intervention,the incidence of oral mucositis,the mastery rate of correct arch expansion method,self-rating anxiety scale score,soft scale index,and plaque index were compared for patients in different groups.In addition,satisfaction and complications were comparatively analyzed.RESULTS Incidence of oral mucositis in the Obs group was lower(14.6%vs 38.5%),and the mastery rate of correct arch expansion method was obviously higher(90.2%vs 55.0%)was obviously higher(all P<0.05).Meanwhile,the soft scale index and plaque index in the Obs group were much lower(P<0.05).The compliance(90.24%)and satisfaction(95.12%)in the Obs group were greatly higher(P<0.05).CONCLUSION The CBT-based CNI mode greatly improved the mastery rate of correct arch expansion method during arch expansion in treating patients with OOA and enhanced the therapeutic effect of arch expansion and the oral health of patients,improving the patient compliance. 展开更多

Effect of dienogest treatment on uterine fibroid volume in patients with endometriosis or adenomyosis complicated by uterine fibroids

BACKGROUND Currently,the use of dienogest in clinical practice has increased significantly,and many studies have focused on its effectiveness and safety in the treatment of endometriosis and adenomyosis;however,the effects of treatment with dienogest on uterine fibroid size in patients with endometriosis or adenomyosis have not been investigated.AIM To explore changes in fibroid size in patients with concomitant uterine fibroids undergoing dienogest treatment for endometriosis or adenomyosis and to evaluate the effectiveness and safety of the drug.METHODS The clinical data of patients with uterine fibroids treated with dienogest for endometriosis or adenomyosis at Peking University First Hospital from January 2021 to January 2023 were retrospectively analyzed.RESULTS The maximum uterine fibroid diameter and volume increased after 3 months,6 months and 1 year of dienogest treatment compared with those before treatment(P<0.01).The maximum diameter and volume of the uterine adenomyoma increased after 3 months of dienogest treatment but decreased after 6 months and 1 year of treatment compared with those before treatment,but the difference was not significant(P>0.05).Endometrial thickness and antigen 125 levels were significantly thinner and decreased,respectively,after dienogest treatment(P<0.01).Pearson's correlation analysis revealed that the increase in uterine fibroid volume after 3 months of dienogest treatment was positively correlated with the basic uterine fibroid volume(r=0.792,P<0.01).Among 64 patients with dysmenorrhea,63 experienced significant relief of dysmenorrhea after 6 months of treatment with dienogest,and all patients experienced significant relief of dysmenorrhea after 12 months.Patients were able to tolerate the drugs,with an average drug tolerance score of 8.73.CONCLUSION The use of dienogest in patients with endometriosis or adenomyosis combined with uterine fibroids can effectively relieve the patient's pain symptoms and significantly reduce the sizes of ovarian endometriotic cysts,but it cannot inhibit uterine fibroid growth. 展开更多

A comparative study of the main factors controlling geohazards induced by 10 strong earthquakes in Western China since the Wenchuan earthquake in 2008 被引量:4

Determining the main controlling factors of earthquake-triggered geohazards is a prerequisite for studying earthquake geohazards and post-disaster emergency response.By studying these factors,the geomorphic and geological factors controlling the nature,condition,and distribution of earthquake-induced geohazards can be analyzed.Such insights facilitate earthquake disaster prediction and emergency response planning.The authors combined field investigations and spatial data analysis to examine geohazards induced by seismic events,examining ten earthquakes including the Wenchuan,Yushu,Lushan events,to elucidate the main control factors of seismic geohazard.The authors observed that seismic geohazard occurrence is usually affected by many factors,among which active nature of the seismogenic fault,seismic peak ground acceleration(PGA),topographic slope and geomorphic height differences,and distance from the fault zone and river system are the most important.Compared with strike-slip earthquakes,thrust earthquakes induce more high-altitude and high-speed remote landslides,which can cause great harm.Slopes of 0°–40°are prone to secondary seismic geohazards,which are mainly concentrated 0–6 km from the river system.Secondary geohazards are not only related to seismogenic fault but also influenced by the associated faults in the earthquake area.The maximum seismic PGA and secondary seismic geohazard number are positively correlated,and the horizontal and vertical ground motions play leading and promoting roles in secondary geohazard formation,respectively.Through the research,the spatial distribution of seismic geohazards is predicted,providing a basis for the formulation of emergency response plans following disasters. 展开更多

Experiments and simulation studies on 283 MeV I ion induced single event effects of silicon carbide(SiC) metal–oxide–semiconductor field-effect transistors(MOSFETs) were carried out. When the cumulative irradiation fluence of the SiC MOSFET reached 5×10^(6)ion·cm^(-2), the drain–gate channel current increased under 200 V drain voltage, the drain–gate channel current and the drain–source channel current increased under 350 V drain voltage. The device occurred single event burnout under 800 V drain voltage, resulting in a complete loss of breakdown voltage. Combined with emission microscope, scanning electron microscope and focused ion beam analysis, the device with increased drain–gate channel current and drain–source channel current was found to have drain–gate channel current leakage point and local source metal melt, and the device with single event burnout was found to have local melting of its gate, source, epitaxial layer and substrate. Combining with Monte Carlo simulation and TCAD electrothermal simulation, it was found that the initial area of single event burnout might occur at the source–gate corner or the substrate–epitaxial interface, electric field and current density both affected the lattice temperature peak. The excessive lattice temperature during the irradiation process appeared at the local source contact, which led to the drain–source channel damage. And the excessive electric field appeared in the gate oxide layer, resulting in drain–gate channel damage. 展开更多

The Luan River is the most important water system in north-eastern Hebei Province,China and is located in the transitional zone of the Eastern Yan Mountains,North China Plain and Songliao Plain.The well-developed river terraces of its tributary,the Yixun River,provide excellent information for studying neotectonics and climate change.There are seven terraces in the lower reaches of the Yixun River,numbered T7-T1.The optically stimulated luminescence dating results of 23 samples show that terraces T7-T2 formed at 111.36±5.83 ka,78.20±4.45 ka,65.29±4.15 ka,56.44±3.07 ka,40.08±2.66 ka,and 13.14±0.76 ka,respectively.A comparison with the oxygen isotope curves of deep-sea sediments reveals that the sediment formation of each terrace corresponded to cold periods of marine isotope stages MIS 4 and MIS 2 and the relatively cold periods of MIS 5e,MIS 3,and MIS 1.Since the Late Pleistocene,the incision rate of the Yixun River has ranged from 0.371-1.740 mm/a.During the formation of T7-T6,T5-T4,T4-T3,and T3-T2,the incision rate was low.However,in the two stages during which T6-T5 and T2-T1 formed(13.14±0.76 ka to 0.58±0.08 ka and 10.79±0.64 ka to 0.16±0.01 ka),these rates reached 1.554 mm/a and 1.592-1.740 mm/a,respectively.At approximately 30 ka,the activity of the Langying Fault increased,leading to footwall uplift.The river gathered in the north of Langying to form the ancient Erdaowan Lake,which resulted in the drying of the river in the lower reaches of the Yixun River during the last glacial maximum without forming river deposits.In the Early Holocene,headward erosion in the lower reaches of the Yixun River was enhanced,which resulted in the disappearance of the lake,and incised meandering formed due to increased neotectonism.Based on the analyses of river incision and the formation of ancient lakes and incised meandering,it was inferred that there have been three periods of strong tectonism in the river basin since the Late Pleistocene. 展开更多

Radiation effects of silicon carbide metal–oxide–semiconductor field-effect transistors(SiC MOSFETs)induced by 20 MeV proton under drain bias(V_(D)=800 V,V_(G)=0 V),gate bias(V_(D)=0 V,V_(G)=10 V),turn-on bias(V_(D)=0.5 V,V_(G)=4 V)and static bias(V_(D)=0 V,V_(G)=0 V)are investigated.The drain current of SiC MOSFET under turn-on bias increases linearly with the increase of proton fluence during the proton irradiation.When the cumulative proton fluence reaches 2×10^(11)p·cm^(-2),the threshold voltage of SiC MOSFETs with four bias conditions shifts to the left,and the degradation of electrical characteristics of SiC MOSFETs with gate bias is the most serious.In the deep level transient spectrum test,it is found that the defect energy level of SiC MOSFET is mainly the ON2(E_(c)-1.1 eV)defect center,and the defect concentration and defect capture cross section of SiC MOSFET with proton radiation under gate bias increase most.By comparing the degradation of SiC MOSFET under proton cumulative irradiation,equivalent 1 MeV neutron irradiation and gamma irradiation,and combining with the defect change of SiC MOSFET under gamma irradiation and the non-ionizing energy loss induced by equivalent 1 MeV neutron in SiC MOSFET,the degradation of SiC MOSFET induced by proton is mainly caused by ionizing radiation damage.The results of TCAD analysis show that the ionizing radiation damage of SiC MOSFET is affected by the intensity and direction of the electric field in the oxide layer and epitaxial layer. 展开更多

在信号处理领域,传统的自适应滤波算法采用的固定步长会导致稳态误差和收敛速度无法同时兼顾。针对这个问题,对最小平均p范数(Least Mean p-norm,LMP)算法进行改进,提出了一种基于改进双曲正切(tanh)函数的变步长最小平均p范数算法。该... 在信号处理领域,传统的自适应滤波算法采用的固定步长会导致稳态误差和收敛速度无法同时兼顾。针对这个问题,对最小平均p范数(Least Mean p-norm,LMP)算法进行改进,提出了一种基于改进双曲正切(tanh)函数的变步长最小平均p范数算法。该算法利用改进的tanh函数来调节步长,采用移动加权平均法构造变步长函数;同时引入了一个调节函数以进一步提升算法的性能。通过在海洋脉冲噪声干扰下进行仿真,实验表明,与已有的固定步长和变步长算法相比,改进的变步长LMP算法较好地兼顾系统的收敛速度和稳态误差;引入调节函数后的新算法在保证原有算法收敛速度的同时进一步降低了算法的稳态误差,从而兼顾了算法的收敛性和稳定性,具有较好的可行性。 展开更多

基于氧化三甲胺的稳定性冠心病低强度他汀类药物敏感性预测模型建立与验证

目的观察稳定性冠心病(CHD)患者血浆氧化三甲胺(TMAO)水平对他汀类药物敏感性的影响,建立基于TMAO的他汀类药物敏感性预测模型并验证。方法前瞻性收集稳定性CHD且接受低强度他汀类药物治疗的患者,对完成1年随访计划的63名患者随访终点... 目的观察稳定性冠心病(CHD)患者血浆氧化三甲胺(TMAO)水平对他汀类药物敏感性的影响,建立基于TMAO的他汀类药物敏感性预测模型并验证。方法前瞻性收集稳定性CHD且接受低强度他汀类药物治疗的患者,对完成1年随访计划的63名患者随访终点事件为血脂不达标(低密度脂蛋白胆固醇≤1.8 mmol/L)。采用超高效液相色谱—三重四极杆质谱(UHPLC-QQQ/MS)法检测患者在接受他汀类药物之前肠道菌群代谢产物TMAO水平,采用PCR荧光探针法检测患者他汀类药物敏感基因基因SLCO1B1、ABCB1、ABCG2、ApoE的突变情况。1年后统计纳入患者终点事件发生情况以及各项指标的检测情况,比较高、低TMAO水平者的血脂达标率。采用单因素、多因素Cox回归分析筛选影响他汀类药物敏感性的独立危险因素,建立预测他汀类药物敏感性的列线图模型及风险评分模型。通过受试者工作特征曲线下面积(AUC)、一致性指数(C指数)、赤池信息准则(AIC)值验证预测模型的准确性,内部验证对预测模型的稳定性进行验证。结果高TMAO水平者血脂不达标率高于低TMAO水平者(P<0.01)。单因素、多因素Cox回归分析显示,TMAO、SLCO1B1基因突变、ABCB1基因突变、ABCG2基因突变、ApoE基因突变为影响他汀类药物敏感性的独立危险因素。联合以上5项指标建立的临床预测模型AUC为0.762,C指数为0.781,AIC为569.428。内部验证显示,建模组、验证组AUC分别为0.770、0.755,两组比较差异无统计学意义。结论高TMAO水平的稳定性CHD患者对他汀类药物不敏感的概率更大,TMAO、SLCO1B1基因突变、ABCB1基因突变、ABCG2基因突变、ApoE基因突变是影响他汀类药物敏感性的危险因素,以上述预测因子构建的风险评分模型预测稳定性CHD患者他汀类药物敏感性的效果较好。 展开更多

目的:探讨心肌梗死后左心室附壁血栓(LVT)患者体循环栓塞发生情况及新型口服抗凝药物(NOAC)及传统维生素K拮抗剂治疗的优劣以及口服抗凝药物最佳时间的选择。方法:选取2012~2017年就诊于新疆维吾尔自治区人民医院所有确诊心肌梗死后LV... 目的:探讨心肌梗死后左心室附壁血栓(LVT)患者体循环栓塞发生情况及新型口服抗凝药物(NOAC)及传统维生素K拮抗剂治疗的优劣以及口服抗凝药物最佳时间的选择。方法:选取2012~2017年就诊于新疆维吾尔自治区人民医院所有确诊心肌梗死后LVT形成的145例患者作为LVT组,随机抽取同时间段明确诊断为急性心肌梗死但未形成LVT的238例患者作为非LVT组,对两组患者进行随访,主要终点事件为新发体循环栓塞事件,次要终点为出血事件的发生。结果:最终LVT组有136例,非LVT组有216例患者参与了研究,中位随访时间为33.8个月。新发体循环栓塞在LVT组共有23例(16.9%),非LVT组共有11例(5.1%),LVT组发生风险更高(HR=5.8,95%CI:1.51~8.98,P<0.001);LVT组口服华法林患者70例,口服利伐沙班56例,口服华法林患者中12例(17.1%)出现新发体循环栓塞,口服利伐沙班患者中有2例(3.6%)出现新发体循环栓塞,两者在栓塞事件方面差异有统计学意义(17.1%vs 3.6%,HR=2.138, 95%CI:1.086~3.974,P=0.029),口服华法林患者中共11例(15.7%)出血,口服利法沙班患者中5例(8.9%)出血,两组差异无统计学意义(HR=1.14, 95%CI:0.832~1.245,P=0.32)。口服抗凝药物<5个月,5~11个月,12~14个月、15~18个月发生栓塞事件分别为7例,4例,2例,1例(P=0.021)。结论:LVT是体循环栓塞的独立危险因素,NOACs与维生素K受体拮抗剂相比可在不增加出血风险的基础上减少栓塞事件的发生,且抗凝时间越长,发生栓塞事件的概率越小。 展开更多

目的探讨血糖波动和低血糖对心房颤动合并2型糖尿病患者预后的影响。方法本研究为前瞻性队列研究。选取2012年1月至2014年12月在新疆维吾尔自治区人民医院诊治的心房颤动合并2型糖尿病患者,记录血糖变化,根据血糖波动情况分为两组,比较... 目的探讨血糖波动和低血糖对心房颤动合并2型糖尿病患者预后的影响。方法本研究为前瞻性队列研究。选取2012年1月至2014年12月在新疆维吾尔自治区人民医院诊治的心房颤动合并2型糖尿病患者,记录血糖变化,根据血糖波动情况分为两组,比较预后情况。结果共306例患者入选,男性221例(72.2%),年龄18~80岁,平均(51.6±15.1)岁,中位随访时间1.5年,中位血糖波动3.6 mmol/L。随访期间,高于中位血糖波动组的不良心血管事件发生率明显高于低于中位血糖波动组(P<0.05)。Kaplan-Meier生存分析发现,有低血糖史的患者主要终点事件[16.8%比7.9%,风险比(HR)=1.83,95%CI:1.128~3.536,P<0.01]和次要终点事件(17.1%比8.4%,HR=1.92,95%CI:1.235~3.848,P<0.01)发生率明显高于无低血糖史的患者。COX风险比例模型发现,基线血糖较高的患者血糖波动的幅度越大,其主要终点事件(11.9%比6.2%,P<0.001)和次要终点事件(22.8%比15.9%,P<0.001)发生率越高。结论血糖波动可影响心房颤动合并2型糖尿病患者的预后,且低血糖增加患者的不良心血管事件风险。 展开更多

目的探讨改善生活方式对心房颤动患者射频消融术后复发的影响。方法选取2012年1月至2014年12月就诊于新疆维吾尔自治区人民医院的271例首次行射频消融术的心房颤动患者并进行随访,记录患者随访1年是否有心房颤动复发情况,并统计术后患... 目的探讨改善生活方式对心房颤动患者射频消融术后复发的影响。方法选取2012年1月至2014年12月就诊于新疆维吾尔自治区人民医院的271例首次行射频消融术的心房颤动患者并进行随访,记录患者随访1年是否有心房颤动复发情况,并统计术后患者的血压、血糖、吸烟、饮酒、体重指数改善情况。运用Kaplan-Meier法和多因素Cox比例风险模型分析改善生活方式对心房颤动射频消融术后患者复发的影响。结果术后随访1年,271例患者中79例(29.2%)心房颤动复发。术后1年改善吸烟患者占29.9%(81/271),改善饮酒患者占25.8%(70/271),改善血压患者占21.0%(57/271),改善体重指数患者占18.5%(50/271),改善血糖患者占9.6%(26/271)。Kaplan-Meier法和多因素Cox比例风险模型分析均显示,除吸烟外,改善收缩压、舒张压、血糖、饮酒、体重指数单个因素均可减少心房颤动术后复发(均P<0.05),且心房颤动术后改善生活方式的项目数越多,其对术后复发的防护作用越强。结论改善血压、血糖、饮酒、体重指数均可以减少心房颤动患者射频消融术后复发。 展开更多
