血府逐瘀汤联合恩替卡韦治疗瘀血阻络型乙型肝炎肝硬化代偿期患者的临床效果

目的 探讨血府逐瘀汤联合恩替卡韦治疗瘀血阻络型乙型肝炎肝硬化代偿期患者的临床效果。方法 选取80例瘀血阻络型乙型肝炎肝硬化代偿期患者,随机分为观察组和对照组,每组40例。给予观察组血府逐瘀汤联合恩替卡韦治疗,给予对照组恩替卡... 目的 探讨血府逐瘀汤联合恩替卡韦治疗瘀血阻络型乙型肝炎肝硬化代偿期患者的临床效果。方法 选取80例瘀血阻络型乙型肝炎肝硬化代偿期患者,随机分为观察组和对照组,每组40例。给予观察组血府逐瘀汤联合恩替卡韦治疗,给予对照组恩替卡韦治疗,疗程均为24周。比较两组患者治疗前后失代偿期肝硬化发生率,HBV-DNA载量、肝功能[ALT、AST、总胆红素(TBIL)]、血清炎症因子[肿瘤坏死因子α(TNF-α)、白细胞介素(IL)-6、IL-22]、肝脏超声相关指标(门静脉主干内径、脾静脉内径、脾脏厚度、脾脏长度)、中医证候积分和中医证候疗效的变化情况,以及治疗期间不良反应发生情况。结果 治疗后12周、治疗后24周,两组患者失代偿期肝硬化发生率及门静脉主干内径、脾静脉内径、脾脏厚度、脾脏长度差异无统计学意义(P>0.05)。两组患者的血清ALT、AST、TBIL、TNF-α、IL-6、IL-22水平,以及HBV-DNA载量、中医证候积分随治疗时间延长而降低,且治疗后12周、治疗后24周,观察组的上述指标均低于对照组(P<0.05)。治疗后,观察组的中医证候疗效的总有效率高于对照组(P<0.05)。治疗期间两组患者的不良反应发生率差异无统计学意义(P>0.05)。结论 血府逐瘀汤联合恩替卡韦治疗瘀血阻络型乙型肝炎肝硬化代偿期患者的临床疗效优于单纯恩替卡韦治疗,可有效降低炎症细胞因子的表达水平,改善肝功能,且安全性较好。 展开更多

Developing high-performance aqueous Zn-ion batteries from sustainable biomass becomes increasingly vital for large-scale energy storage in the foreseeable future.Therefore,γ-MnO_(2) uniformly loaded on N-doped carbon derived from grapefruit peel is successfully fabricated in this work,and particularly the composite cathode with carbon carrier quality percentage of 20 wt%delivers the specific capacity of 391.2 mAh g^(−1)at 0.1 A g^(−1),outstanding cyclic stability of 92.17%after 3000 cycles at 5 A g^(−1),and remarkable energy density of 553.12 Wh kg^(−1) together with superior coulombic efficiency of~100%.Additionally,the cathodic biosafety is further explored specifically through in vitro cell toxicity experiments,which verifies its tremendous potential in the application of clinical medicine.Besides,Zinc ion energy storage mechanism of the cathode is mainly discussed from the aspects of Jahn–Teller effect and Mn domains distribution combined with theoretical analysis and experimental data.Thus,a novel perspective of the conversion from biomass waste to biocompatible Mn-based cathode is successfully developed. 展开更多

Repetitive traumatic brain injury impacts adult neurogenesis in the hippocampal dentate gyrus,leading to long-term cognitive impairment.However,the mechanism underlying this neurogenesis impairment remains unknown.In this study,we established a male mouse model of repetitive traumatic brain injury and performed long-term evaluation of neurogenesis of the hippocampal dentate gyrus after repetitive traumatic brain injury.Our results showed that repetitive traumatic brain injury inhibited neural stem cell proliferation and development,delayed neuronal maturation,and reduced the complexity of neuronal dendrites and spines.Mice with repetitive traumatic brain injuryalso showed deficits in spatial memory retrieval.Moreover,following repetitive traumatic brain injury,neuroinflammation was enhanced in the neurogenesis microenvironment where C1q levels were increased,C1q binding protein levels were decreased,and canonical Wnt/β-catenin signaling was downregulated.An inhibitor of C1 reversed the long-term impairment of neurogenesis induced by repetitive traumatic brain injury and improved neurological function.These findings suggest that repetitive traumatic brain injury–induced C1-related inflammation impairs long-term neurogenesis in the dentate gyrus and contributes to spatial memory retrieval dysfunction. 展开更多

【研究目的】人体日常摄入的硒主要通过食物链源于土壤,而中国是富硒土地资源相对匮乏的国家,面对有限且珍贵的富硒土地资源,亟需构建科学合理的富硒土地开发利用评价体系,为特色土地资源的高效开发提供依据。【研究方法】本文以陕西关... 【研究目的】人体日常摄入的硒主要通过食物链源于土壤,而中国是富硒土地资源相对匮乏的国家,面对有限且珍贵的富硒土地资源,亟需构建科学合理的富硒土地开发利用评价体系,为特色土地资源的高效开发提供依据。【研究方法】本文以陕西关中地区为研究区,在土地质量地球化学调查成果基础上,融合反映农用地自然条件、基础设施水平的评价指标,构建了涵盖立地条件、土壤硒含量、理化性质、土壤质量和土地管理等5大类12项指标的关中地区富硒土地开发利用评价体系。同时以西安市临潼区为例,开展了临潼区富硒土地开发利用适宜性评价。【研究结果】将临潼区土地划分为高度适宜区、适宜区、较不适宜区、不适宜区和极不适宜区共5个等级,其中高度适宜开发区及适宜区开发面积共248.35 km^(2),主要分布在临潼区北部的新市乡、栎阳镇、徐杨乡、相桥镇及中东部的何寨镇。基于不同适宜评价区内小麦硒含量及富硒比例,发现其富硒土地适宜性与小麦富硒程度高度耦合。【结论】说明构建的富硒土地开发利用评价体系对富硒土地资源的开发利用具有较好的科学指导价值及重要的示范意义。 展开更多

Background:Most bone-related injuries to grassroots troops are caused by training or accidental injuries.To establish preventive measures to reduce all kinds of trauma and improve the combat effectiveness of grassroots troops,it is imperative to develop new strategies and scafolds to promote bone regeneration.Methods:In this study,a porous piezoelectric hydrogel bone scafold was fabricated by incorporating polydopamine(PDA)-modified ceramic hydroxyapatite(PDA-hydroxyapatite,PHA)and PDA-modified barium titanate(PDABaTiO_(3),PBT)nanoparticles into a chitosan/gelatin(Cs/Gel)matrix.The physical and chemical properties of the Cs/Gel/PHA scafold with 0–10 wt%PBT were analyzed.Cell and animal experiments were performed to characterize the immunomodulatory,angiogenic,and osteogenic capabilities of the piezoelectric hydrogel scafold in vitro and in vivo.Results:The incorporation of BaTiO_(3) into the scafold improved its mechanical properties and increased self-generated electricity.Due to their endogenous piezoelectric stimulation and bioactive constituents,the prepared Cs/Gel/PHA/PBT hydrogels exhibited cytocompatibility as well as immunomodulatory,angiogenic,and osteogenic capabilities;they not only effectively induced macrophage polarization to M2 phenotype but also promoted the migration,tube formation,and angiogenic differentiation of human umbilical vein endothelial cells(HUVECs)and facilitated the migration,osteodifferentiation,and extracellular matrix(ECM)mineralization of MC3T3-E1 cells.The in vivo evaluations showed that these piezoelectric hydrogels with versatile capabilities significantly facilitated new bone formation in a rat large-sized cranial injury model.The underlying molecular mechanism can be partly attributed to the immunomodulation of the Cs/Gel/PHA/PBT hydrogels as shown via transcriptome sequencing analysis,and the PI3K/Akt signaling axis plays an important role in regulating macrophage M2 polarization.Conclusion:The piezoelectric Cs/Gel/PHA/PBT hydrogels developed here with favorable immunomodulation,angiogenesis,and osteogenesis functions may be used as a substitute in periosteum injuries,thereby offering the novel strategy of applying piezoelectric stimulation in bone tissue engineering for the enhancement of combat efectiveness in grassroots troops. 展开更多

The risk of gas leakage due to geological flaws in offshore carbon capture, utilization, and storage, as well as leakage from underwater oil or gas pipelines, highlights the need for underwater gas leakage monitoring technology. Remotely operated vehicles(ROVs) and autonomous underwater vehicles(AUVs) are equipped with high-resolution imaging sonar systems that have broad application potential in underwater gas and target detection tasks. However, some bubble clusters are relatively weak scatterers, so detecting and distinguishing them against the seabed reverberation in forward-looking sonar images are challenging. This study uses the dual-tree complex wavelet transform to extract the image features of multibeam forward-looking sonar. Underwater gas leakages with different flows are classified by combining deep learning theory. A pool experiment is designed to simulate gas leakage, where sonar images are obtained for further processing. Results demonstrate that this method can detect and classify underwater gas leakage streams with high classification accuracy. This performance indicates that the method can detect gas leakage from multibeam forward-looking sonar images and has the potential to predict gas leakage flow. 展开更多

BACKGROUND This case report presents the rare occurrence of hematochezia due to an internal iliac artery aneurysm leading to an arterioenteric fistula,expanding the differential diagnosis for gastrointestinal bleeding.It emphasizes the importance of considering vascular origins in cases of atypical hematochezia,particularly in the absence of common gastrointestinal causes,and highlights the role of imaging and multidisciplinary management in diagnosing and treating such unusual presentations.CASE SUMMARY A 75-year-old man with a history of hypertension presented with 12 d of hematochezia,experiencing bloody stools 7-8 times per day.Initial computed tomography(CT)scans revealed an aneurysmal rupture near the right internal iliac artery with suspected hematoma development.Hemoglobin levels progressively decreased to 7 g/dL.Emergency arterial angiography and iliac arterycovered stent placement were performed,followed by balloon angioplasty.Despite initial stabilization,minor rectal bleeding and abdominal pain persisted,leading to further diagnostic colonoscopy.This identified a neoplasm and potential perforation at the proximal rectum.An exploratory laparotomy confirmed the presence of a hematoma and an aneurysm invading the rectal wall,necessitating partial rectal resection,intestinal anastomosis,and ileostomy.Postoperative recovery was successful,with no further bleeding incidents and normal follow-up CT and colonoscopy results after six months.CONCLUSION In cases of unusual gastrointestinal bleeding,it is necessary to consider vascular causes for effective diagnosis and intervention. 展开更多

BACKGROUND Adaptor protein,phosphotyrosine interacting with PH domain and leucine zipper 1(APPL1)plays a crucial role in regulating insulin signaling and glucose metabolism.Mutations in the APPL1 gene have been associated with the development of maturity-onset diabetes of the young type 14(MODY14).Currently,only two mutations[c.1655T>A(p.Leu552*)and c.281G>A p.(Asp94Asn)]have been identified in association with this disease.Given the limited understanding of MODY14,it is imperative to identify additional cases and carry out comprehensive research on MODY14 and APPL1 mutations.AIM To assess the pathogenicity of APPL1 gene mutations in diabetic patients and to characterize the functional role of the APPL1 domain.METHODS Patients exhibiting clinical signs and a medical history suggestive of MODY were screened for the study.Whole exome sequencing was performed on the patients as well as their family members.The pathogenicity of the identified APPL1 variants was predicted on the basis of bioinformatics analysis.In addition,the pathogenicity of the novel APPL1 variant was preliminarily evaluated through in vitro functional experiments.Finally,the impact of these variants on APPL1 protein expression and the insulin pathway were assessed,and the potential mechanism underlying the interaction between the APPL1 protein and the insulin receptor was further explored.RESULTS A total of five novel mutations were identified,including four missense mutations(Asp632Tyr,Arg633His,Arg532Gln,and Ile642Met)and one intronic mutation(1153-16A>T).Pathogenicity prediction analysis revealed that the Arg532Gln was pathogenic across all predictions.The Asp632Tyr and Arg633His variants also had pathogenicity based on MutationTaster.In addition,multiple alignment of amino acid sequences showed that the Arg532Gln,Asp632Tyr,and Arg633His variants were conserved across different species.Moreover,in in vitro functional experiments,both the c.1894G>T(at Asp632Tyr)and c.1595G>A(at Arg532Gln)mutations were found to downregulate the expression of APPL1 on both protein and mRNA levels,indicating their pathogenic nature.Therefore,based on the patient’s clinical and family history,combined with the results from bioinformatics analysis and functional experiment,the c.1894G>T(at Asp632Tyr)and c.1595G>A(at Arg532Gln)mutations were classified as pathogenic mutations.Importantly,all these mutations were located within the phosphotyrosinebinding domain of APPL1,which plays a critical role in the insulin sensitization effect.CONCLUSION This study provided new insights into the pathogenicity of APPL1 gene mutations in diabetes and revealed a potential target for the diagnosis and treatment of the disease. 展开更多

目的分析反馈式健康教育结合强化营养干预在肝硬化腹水患者中的应用效果。方法将我院接受治疗的100例肝硬化腹水患者根据随机数字表法分为对照组(常规护理干预)和观察组(反馈式健康教育结合强化营养干预),各50例。比较两组的护理效果。... 目的分析反馈式健康教育结合强化营养干预在肝硬化腹水患者中的应用效果。方法将我院接受治疗的100例肝硬化腹水患者根据随机数字表法分为对照组(常规护理干预)和观察组(反馈式健康教育结合强化营养干预),各50例。比较两组的护理效果。结果随访后,观察组的健康认知水平、健康促进生活方式量表-Ⅱ(HPLP-Ⅱ)评分高于对照组(P<0.05)。护理后,观察组的总胆红素(TBIL)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)水平低于对照组(P<0.05)。随访后,观察组的体质指数(BMI)、上臂肌围(MAMC)及清蛋白、前清蛋白水平高于对照组(P<0.05)。观察组的腹水改善效果优于对照组(P<0.05)。结论反馈式健康教育结合强化营养干预能有效提高肝硬化腹水患者的健康行为和营养状况。 展开更多

目的 探究慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者炎症细胞表型与临床病理特征的相关性。方法 选取郑州大学附属郑州中心医院收治的200例AECOPD患者为研究对象,根据外周... 目的 探究慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者炎症细胞表型与临床病理特征的相关性。方法 选取郑州大学附属郑州中心医院收治的200例AECOPD患者为研究对象,根据外周血细胞计数结果将患者分为嗜中性粒细胞炎症表型组(NE组,120例)和嗜酸性细胞炎症表型组(EOS组,80例)。分别检测NE组和EOS组患者气道炎症反应标志物呼出气一氧化氮(FeNO)、小气道/肺泡一氧化氮(CaNO)、肺功能指标(FVC、FEV1、FEV1/FVC)、呼吸功能(WOB、Raw、MVV),比较两组临床诊疗情况及临床预后。结果 NE组FeNO、CaNO指标均显著高于EOS组(P <0.05),FVC、FEV1、FEV1/FVC指标值均低于EOS组(P <0.05)。NE组WOB、Raw均高于EOS组(P <0.05),而MVV低于EOS组(P=0.009)。NE组患者慢阻肺频繁加重者比例、咳脓痰者比例、抗生素联合使用者比例、无创呼吸机使用者比例均高于EOS组(P <0.05),且急性生理与慢性健康评估(acute physiology and chronic health evaluation,APACHE)评分高于EOS组(57.34±3.23 vs.45.36±3.28,P=0.004)。NE组患者临床预后不良的发生率显著高于EOS组(15.00%vs.5.00%,P=0.010)。结论 NE型AECOPD患者气道炎症反应程度、肺功能及呼吸功能障碍程度、基础临床症状及病情均较EOS型患者更重,且预后不良的发生率较EOS型患者高。 展开更多

目的分析参芪扶正注射液联合化疗治疗晚期乳腺癌的临床效果及对血清肿瘤标志物水平和免疫功能的影响。方法选择2020年1月至2021年1月于我院接受治疗的100例晚期乳腺癌患者作为研究对象,根据治疗方案将其分为对照组和观察组,各50例。对... 目的分析参芪扶正注射液联合化疗治疗晚期乳腺癌的临床效果及对血清肿瘤标志物水平和免疫功能的影响。方法选择2020年1月至2021年1月于我院接受治疗的100例晚期乳腺癌患者作为研究对象,根据治疗方案将其分为对照组和观察组,各50例。对照组给予常规化疗治疗,观察组在化疗基础上给予参芪扶正注射液治疗。比较两组的治疗效果。结果随访结束后,观察组的Kupperman绝经指数(KMI)评分低于对照组,卡氏功能状态量表(KPS)评分高于对照组(P<0.05)。随访结束后,观察组的癌胚抗原(CEA)、癌抗原125(CA125)、癌抗原153(CA153)、癌抗原199(CA199)水平低于对照组(P<0.05)。随访结束后,观察组的白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原4阳性/白细胞分化抗原8阳性(CD4^(+)/CD8^(+))高于对照组,白细胞分化抗原8阳性(CD8^(+))低于对照组(P<0.05)。观察组化疗后的轻度骨髓抑制发生率高于对照组,重度骨髓抑制发生率低于对照组(P<0.05)。观察组的客观缓解率(ORR)和疾病控制率(DCR)均高于对照组(P<0.05)。结论参芪扶正注射液联合化疗治疗晚期乳腺癌能有效控制疾病进展,缓解骨髓抑制程度,降低血清肿瘤标志物水平,改善临床症状,提高免疫功能和健康状态。 展开更多

目的运用文献计量法对急性肺损伤进行可视化图谱分析。方法检索中国知网、PubMed数据库中2002年1月至2021年12月有关急性肺损伤的研究文献,使用CiteSpace和VOSviewer对纳入文献的作者、研究单位、国家、关键词进行可视化图谱分析。结果... 目的运用文献计量法对急性肺损伤进行可视化图谱分析。方法检索中国知网、PubMed数据库中2002年1月至2021年12月有关急性肺损伤的研究文献,使用CiteSpace和VOSviewer对纳入文献的作者、研究单位、国家、关键词进行可视化图谱分析。结果中国知网、PubMed数据库分别纳入10376、20335篇文献,发文比较差异明显;发文作者分析发现目前以钱桂生、邱海波、段蕴铀、Michael Matthay、Paolo Pelosi、Danile Traber团队发文较多;对于该疾病研究较广的国家主要有美国、中国、德国等。关键词对比分析发现中国知网数据库研究主要集中在炎症、氧化应激、信号通路、疾病感染源、动物模型、肺表面活性物质、机械通气等方面;PubMed数据库中对发病人群、动物疾病模型、疾病感染源、血流动力学、急性肺损伤的诊疗统计、实验机制中的炎症因子、过氧化物研究、免疫调节等有详细研究。结论急性肺损伤在国内外一直都被热点关注,中国知网、PubMed数据库研究比较差异明显,目前对于其研究多在血液指标、炎症因子、免疫平衡、机械通气相关的实验基础和临床应用方面,且发达国家对其研究较多。 展开更多

探究桂枝茯苓丸治疗肝硬化潜在的作用机制。利用TCMSP、ETCM和HERB数据库获得桂枝茯苓丸的化学成分及作用靶点,Genecards、OMIM和TTD数据库收集肝硬化相关靶点,并运用Venn图获取药物和疾病交集靶点。通过STRING在线数据库对交集靶点蛋... 探究桂枝茯苓丸治疗肝硬化潜在的作用机制。利用TCMSP、ETCM和HERB数据库获得桂枝茯苓丸的化学成分及作用靶点,Genecards、OMIM和TTD数据库收集肝硬化相关靶点,并运用Venn图获取药物和疾病交集靶点。通过STRING在线数据库对交集靶点蛋白构建蛋白相互作用网络(PPI),采用Cytoscape 3.9.1软件对结果进行可视化分析。使用Metascape数据平台进行GO功能分析和KEGG通路富集分析,AutoDock Tools 1.5.7和Discovery Studio 2021软件进行分子对接验证。共获得桂枝茯苓丸的有效成分56个,PPI包含靶点蛋白121个,关键蛋白包括TNF、AKT1和TP53等。GO功能分析,得到桂枝茯苓丸治疗肝硬化主要通过对活性氧的反应、对脂多糖的反应和对细菌来源的分子反应等发挥作用。KEGG通路富集分析结果显示,桂枝茯苓丸治疗肝硬化的机制可能与AGE-RAGE信号通路、TNF信号通路和IL-17信号通路等有关。分子对接发现药物核心活性成分与靶点基因之间的结合活性良好。本文初步揭示了桂枝茯苓丸治疗肝硬化的多成分、多靶点和多通路的作用机制,为桂枝茯苓丸的进一步研究提供了理论基础。 展开更多

目的探讨胆道镜钬激光碎石术联合肝叶切除术对复杂性肝内胆管结石患者结石清除率、生化指标及生活质量的影响。方法选取2019年5月至2021年5月我院接收的150例复杂性肝内胆管结石患者为研究对象,根据治疗方案不同将其分为对照组和观察组,... 目的探讨胆道镜钬激光碎石术联合肝叶切除术对复杂性肝内胆管结石患者结石清除率、生化指标及生活质量的影响。方法选取2019年5月至2021年5月我院接收的150例复杂性肝内胆管结石患者为研究对象,根据治疗方案不同将其分为对照组和观察组,各75例。对照组给予胆总管切开取石术+肝叶切除术治疗,观察组给予胆道镜钬激光碎石术+肝叶切除术治疗。比较两组的治疗效果。结果观察组的手术时间、胃肠功能恢复时间及住院时间短于对照组,术中出血量少于对照组,结石清除率高于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。术后5 d,两组的总胆红素、谷丙转氨酶、天门冬氨酸氨基转氨酶、碱性磷酸酶及谷氨酰基转移酶水平均降低,且观察组低于对照组(P<0.05)。术后3个月,观察组的健康调查简表(SF-36)各维度评分均高于对照组(P<0.05)。结论胆道镜钬激光碎石术联合肝叶切除术治疗复杂性肝内胆管结石效果较好,可提高结石清除率,缩短住院时间,降低并发症发生率,改善生化指标水平,提高生活质量。 展开更多

Converting CO_(2)to valuable materials is attractive in environmental protection and resource utilization.In this study,a vapor-liquid interface reaction system for mass production of high-quality graphene is reported.The graphene obtained has high crystallinity and few defects during the reaction of CO_(2)and Mg melt.The growth mechanism of graphene is demonstrated in vapor-liquid interface area by combining the CO_(2)bubbles as a soft template to guide growth with the confinement effect of dense MgO nanoparticles.The quality of the graphene is verified by epoxy composites with high electromagnetic shielding effectiveness.Additionally,the V-L reaction method ingeniously solves the dispersion of graphene in metal,providing a preparation strategy of Mg matrix composites with structure and function integration. 展开更多
