Based on the engineering background of the contact channel between Shangyang and Gushan of Fuzhou Metro Line 2 undercrossing the existing tunnel line, the freezing temperature field of the contact channel, the displacement field of the existing tunnel line and the contact channel with different net distances and horizontal angles are analyzed by ANSYS finite element software and field measurement method. The obtained results indicate that during the freezing period, the temperature drops at different measuring holes are almost the same. The temperature near the bottom freezing tube drops faster than that far from the tube.It is found that the bilateral freezing technique improves the formation of the freezing wall in the intersection area.In this case, the intersection time of the cross-section is 7 days faster than that of the adjacent ordinary section. The change curve of the displacement of the surface uplift in different freezing periods with the distance from the center of the channel is“M”shaped. The maximum uplift displacement at 12 m from channel center is 25 mm.The vertical displacement of the measuring point located above the central axis of the connecting channel is large. The farther the point from the central axis, the smaller the corresponding vertical displacement. When the horizontal angle between the existing tunnel and the connecting channel is less than 60,the existing vertical displacement of the tunnel changes rapidly with the horizontal angle, reaching 0.17 mm/.Meanwhile,when the net distance is less than 6.1 m,the change rate of the vertical displacement of the tunnel is up to 2.4 mm/m. 展开更多

BACKGROUND It has been shown that children with Pierre Robin sequence(PRS)have a higher risk of difficult intubation before surgery. When mask ventilation or tracheobronchial intubation is expected to be challenging, flexible bronchoscopy(FB)is advantageous in airway safety when it is used to guide tracheobronchial intubation(TI).AIM To evaluate the complications of TI using FB in children with PRS and explore the effect of nursing services on postoperative complications.METHODS One hundred and five children with PRS underwent TI using FB before early mandibular distraction osteogenesis. One hundred and eight children with common pneumonia who did not have a difficult airway were set as the control group.Demographic data, success rates of TI,time required for TI,number of TI attempts, and the incidence of postoperative complications were assessed.Besides, the strategies used to attenuate complications were investigated.RESULTS The success rate of TI was 100%in children with PRS,while the success rate at the first attempt in the PRS group was significantly lower than that in the control group(88.6%vs 98.2%,P=0.005).The time required for TI in the PRS group was markedly longer than that in the control group(P<0.001).Children in the PRS group required repetitive operations to enter the glottis successfully(P=0.017).The incidence of complications was noticeably higher in the PRS group(50/105,47.6%)than in the control group(36/108,33.3%)(P=0.034).Seven of 105 PRS children experienced laryngeal edema (LE) (6.7%), compared with one (0.9%) in the control group(P = 0.034). Out of the seven patients who had LE, all were reintubated and managed withsteroids: six recovered with inhaled steroids alone before extubated, and one was given systemiccorticosteroids before recovery.CONCLUSIONFB contributes to a high success rate of TI in children with PRS. To prevent LE, operators shouldpay more attention to catheter material, catheter lubrication and intubation time. 展开更多

Respiratory syncytial virus(RSV)is a significant cause of acute lower respiratory tract infection(ALRTI)in children underfive years of age.Between 2017 and 2021,396 complete sequences of the RSV F gene were obtained from 500 RSV-positive throat swabs collected from ten hospitals across nine provinces in China. In addition,151 sequences from China were sourced from GenBank and GISAID,making a total of 549 RSV F gene sequences subjected to analysis.Phylogenetic and genetic diversity analyses revealed that the RSV F genes circulating in China from 2017 to 2021 have remained relatively conserved, although some amino acids(AAs)have undergone changes.AA mutations with frequencies10%were identified at six sites and the p27 region:V384I(site I),N276S(site II),R213S(siteØ),and K124N(p27)for RSV A;F45L(site I),M152I/L172Q/S173 L/I185V/K191R(site V),and R202Q/I206M/Q209R(siteØ)for RSV B.Comparing mutational frequencies in RSV-F before and after 2020 revealed minor changes for RSV A,while the K191R,I206M,and Q209R frequencies increased by over 10%in RSV B.Notably, the nirsevimab-resistant mutation,S211N in RSV B,increased in frequency from 0%to 1.15%.Both representative strains aligned with the predicted RSV-F structures of their respective prototypes exhibited similar conformations, with low root-mean-square deviation values. These results could provide foundational data from China for the development of RSV mAbs and vaccines. 展开更多

Background Children as a population have high antimicrobial prescribing rates which may lead to high resistance of bacteria according to data from some single-center surveys of antibiotic prescribing rates in China. The acquirement of baseline data of antibiotic prescribing is the basis of developing intervention strategies on inappropriate antimicrobial prescriptions. Few studies show clearly the pattern and detailed information on classes of antibiotics and distribution of indications of antibiotic prescriptions in children in China. This study aims to assess the antibiotic prescribing patterns among children and neonates hospitalized in 18 hospitals in China. Methods A 24-hour point prevalence survey on antimicrobial prescribing was conducted in hospitalized neonates and chil-dren in China from December 1st, 2016 to February 28th, 2017. Information on the antibiotic use of patients under 18 years of age who were administered one or more on-going antibiotics in the selected wards over a 24-hour period was collected. These data were submitted to the GARPEC (Global Antimicrobial Resistance, Prescribing and Efficacy in Children and Neonates) web-based application ( https ://pidrg -datab ase.sgul.ac.uk/redca p/ ). For statistical analysis, Microsoft Excel 2007 and SPSS 22.0 were used. Results The antibiotic data were collected in 35 wards in 18 hospitals from 9 provinces. In total, 67.76% (975/1439) of the patients (n = 1439) were given at least one antibiotic, including 58.1% (173/298) of neonates (n = 298) and 70.3% (802/1141) of children (n = 1141). In neonates, the three most frequently prescribed antibiotics were third-generation cephalosporins (41.7%), penicillins plus enzyme inhibitor (23.8%), and carbapenems (11.2%). In children, the three most frequently pre-scribed antibiotics were third-generation cephalosporins (35.5%), macrolides (23.2%), and penicillins plus enzyme inhibi-tors (15.9%). The most common indication for antibiotics was proven or probable bacterial lower respiratory tract infection (30.9% in neonates and 66.6% in children). Conclusions Antibiotics are commonly prescribed in the Chinese children population. It is likely that the third-generation cephalosporins and macrolides are currently overused in Chinese children. Efforts must be made to ensure safe and appropri-ate antibiotic prescribing to reduce and prevent the future development of antibiotic resistance. 展开更多

Human respiratory syncytial virus(RSV) is a major pathogen of acute lower respiratory tract infection among young children. To investigate the prevalence and genetic characteristics of RSV in China, we performed a molecular epidemiological study during 2015–2019. A total of 964 RSV-positive specimens were identified from 5529 enrolled patients during a multi-center study. RSV subgroup A(RSV-A) was the predominant subgroup during this research period except in2016. Totally, 535 sequences of the second hypervariable region(HVR-2) of the G gene were obtained. Combined with182 Chinese sequences from GenBank, phylogenetic trees showed that 521 RSV-A sequences fell in genotypes ON1(512),NA1(6) and GA5(3), respectively;while 196 RSV-B sequences fell in BA9(193) and SAB4(3). ON1 and BA9 were the only genotypes after December 2015. Genotypes ON1 and BA9 can be separated into 10 and 7 lineages, respectively. The HVR-2 of genotype ON1 had six amino acid changes with a frequency more than 10%, while two substitutions H258 Q and H266 L were co-occurrences. The HVR-2 of genotype BA9 had nine amino acid substitutions with a frequency more than10%, while the sequences with T290 I and T312 I were all from 2018 to 2019. One N-glycosylation site at 237 was identified among ON1 sequences, while two N-glycosylation sites(296 and 310) were identified in the 60-nucleotide duplication region of BA9. To conclusion, ON1 and BA9 were the predominant genotypes in China during 2015–2019. For the genotypes ON1 and BA9, the G gene exhibited relatively high diversity and evolved continuously. 展开更多

Since the outbreak of coronavirus disease 2019(COVID-19),it has become a global pandemic. The spike(S)protein of etiologic severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)specifically recognizes human angiotensin-converting enzyme 2(hACE2)as its receptor, which is recently identified as an interferon(IFN)-stimulated gene. Here,we find that hACE2 exists on the surface of exosomes released by different cell types, and the expression of exosomal hACE2 is increased by IFNα/β treatment. In particular, exosomal hACE2 can specifically block the cell entry of SARS-CoV-2,subsequently inhibit the replication of SARS-CoV-2 in vitro and ex vivo. Our findings have indicated that IFN is able to upregulate a viral receptor on the exosomes which competitively block the virus entry, exhibiting a potential antiviral strategy. 展开更多

Adoptive cell therapy(ACT)is an emerging powerful cancer immunotherapy, which includes a complex process of genetic modification, stimulation and expansion.During these in vitro or ex vivo manipulation, sensitive cells are inescapability subjected to harmful external stimuli.Although a variety of cytoprotection strategies have been developed, their application on ACT remains challenging.Herein,a DNA network is constructed on cell surface by rolling circle amplification(RCA),and T cell-targeted trivalent tetrahedral DNA nanostructure is used as a rigid scaffold to achieve high-efficient and selective coating for T cells. The cytoprotective DNA network on T-cell surface makes them aggregate over time to form cell clusters, which exhibit more resistance to external stimuli and enhanced activities in human peripheral blood mononuclear cells and liver cancer organoid killing model.Overall, this work provides a novel strategy for in vitro T cell-selective protection, which has a great potential for application in ACT. 展开更多

Monkeypox is a zoonotic disease.Since the first human monkeypox case was detected in 1970,it has been prevalent in some countries in central and western Africa.Since May 2022,monkeypox cases have been reported in more than 96 non-endemic countries and regions worldwide. As of September 14,2022,there have been more than 58,200 human monkeypox cases, and there is community transmission. The cessation of smallpox vaccination in 1980,which had some cross-protection with monkeypox, resulted in a general lack of immunity to monkeypox, which caused global concern and vigilance. As of Sep-tember 14,2022,there are four monkeypox cases in China, including three in Taiwan province and one in Hong Kong city.Previous foreign studies have shown that children are vulnerable to monkeypox and are also at high risk for severe disease or complications. In order to improve pediatricians'understanding of monkeypox and achieve early detection, early diagno-sis,early treatment, and early disposal,we have organized national authoritative experts in pediatric infection, respiratory, dermatology, critical care medicine, infectious diseases, and public health and others to formulate this expert consensus,on the basis of the latest"Clinical management and infection prevention and control for monkeypox"released by The World Health Organization, the"guidelines for diagnosis and treatment of monkeypox(version 2022)"issued by National Health Commission of the People's Republic of China and other relevant documents.During the development of this consensus, multidisciplinary experts have repeatedly demonstrated the etiology, epidemiology, transmission, clinical manifestations, laboratory examinations, diagnosis, differential diagnosis, treatment, discharge criteria, prevention, disposal process, and key points of prevention and control of suspected and confirmed cases. 展开更多
