目的探讨“通督治郁”针刺法治疗卒中后抑郁(PSD)患者的临床疗效以及对患者17项版汉密尔顿抑郁量表(HAMD-17)中各因子和项目评分的影响。方法回顾性收集2022年1月—2023年1月安徽中医药大学第二附属医院脑病六科住院或门诊诊断为PSD患... 目的探讨“通督治郁”针刺法治疗卒中后抑郁(PSD)患者的临床疗效以及对患者17项版汉密尔顿抑郁量表(HAMD-17)中各因子和项目评分的影响。方法回顾性收集2022年1月—2023年1月安徽中医药大学第二附属医院脑病六科住院或门诊诊断为PSD患者的资料,应用“通督治郁”针刺法进行治疗,比较治疗前后患者HAMD-17总分及项目评分、因子评分变化情况。结果本研究共纳入122例患者,治疗后疗效确切,无不良反应发生。治疗后,患者兴趣与工作、胃肠道症状、性症状和体质量减轻与治疗前比较,差异无统计学意义(P>0.05),其他HAMD-17项目评分与治疗前比较,差异有统计学意义(P<0.05)。治疗后,除因子2(体质量因子)外,患者HAMD-17各因子评分均较治疗前降低,差异有统计学意义(P<0.05)。结论“通督治郁”针刺法可明显改善PSD患者焦虑/躯体化症候群、睡眠障碍症候群、认知障碍症候群和阻滞症候群症状,是一种安全、有效的治疗方法。 展开更多

BACKGROUND:Sepsis is one of the main causes of mortality in intensive care units(ICUs).Early prediction is critical for reducing injury.As approximately 36%of sepsis occur within 24 h after emergency department(ED)admission in Medical Information Mart for Intensive Care(MIMIC-IV),a prediction system for the ED triage stage would be helpful.Previous methods such as the quick Sequential Organ Failure Assessment(qSOFA)are more suitable for screening than for prediction in the ED,and we aimed to fi nd a light-weight,convenient prediction method through machine learning.METHODS:We accessed the MIMIC-IV for sepsis patient data in the EDs.Our dataset comprised demographic information,vital signs,and synthetic features.Extreme Gradient Boosting(XGBoost)was used to predict the risk of developing sepsis within 24 h after ED admission.Additionally,SHapley Additive exPlanations(SHAP)was employed to provide a comprehensive interpretation of the model's results.Ten percent of the patients were randomly selected as the testing set,while the remaining patients were used for training with 10-fold cross-validation.RESULTS:For 10-fold cross-validation on 14,957 samples,we reached an accuracy of 84.1%±0.3%and an area under the receiver operating characteristic(ROC)curve of 0.92±0.02.The model achieved similar performance on the testing set of 1,662 patients.SHAP values showed that the fi ve most important features were acuity,arrival transportation,age,shock index,and respiratory rate.CONCLUSION:Machine learning models such as XGBoost may be used for sepsis prediction using only a small amount of data conveniently collected in the ED triage stage.This may help reduce workload in the ED and warn medical workers against the risk of sepsis in advance. 展开更多

Kojic acid is usually used as an antimicrobial. This study investigated the sensory scores, total bacteria number, and total volatile base nitrogen (TVB-N) value of chilled duck meat under kojic acid treatment during storage. The results presented that kojic acid suppressed the growth of spoilage bacteria and prolonged the shelf-life of chilled duck meat by at least two days. In further investigation, the bacterial community composition was explored through high-throughput sequencing. Pseudomonas spp. were the predominant spoilage bacteria after 6-day storage and were significantly suppressed by kojic acid. Besides, Brochothrix showed drug resistance and became the dominant bacteria in the kojic acid treated sample after 6-day storage. In conclusion, kojic acid maintained the sensorial, chemical, and microbial quality of duck meat stored at low temperatures. Meanwhile, kojic acid enriched the bacteria composition and showed strong antimicrobial activity against Pseudomonas spp. 展开更多

Knowing the long-term degradation trajectory of Lithium-ion(Li-ion) battery in its early usage stage is critical for the maintenance of the battery energy storage system(BESS) in reality. Previous battery health diagnosis methods focus on capacity and state of health(SOH) estimation which can receive only the short-term health status of the cell. This paper proposes a novel degradation trajectory prediction method with synthetic dataset and deep learning, which enables to grasp the characterization of the cell's health at a very early stage of Li-ion battery usage. A transferred convolutional neural network(CNN) is chosen to finalize the early prediction target, and the polynomial function based synthetic dataset generation strategy is designed to reduce the costly data collection procedure in real application. In this thread, the proposed method needs one full lifespan data to predict the overall degradation trajectories of other cells. With only the full lifespan cycling data from 4 cells and 100 cycling data from each cell in experimental validation, the proposed method shows a good prediction accuracy on a dataset with more than 100 commercial Li-ion batteries. 展开更多

BACKGROUND Hypoparathyroidism,which can be sporadic or a component of an inherited syndrome,is the most common cause of hypocalcemia.If hypocalcemia is accompanied by other electrolyte disturbances,such as hypokalemia and hypomagnesemia,then the cause,such as renal tubular disease,should be carefully identified.CASE SUMMARY An 18-year-old female visited our clinic because of short stature and facial deformities,including typical phenotypes,such as low ear position,depression of the nasal bridge,small hands and feet,and loss of dentition.The lab results suggested normal parathyroid hormone but hypocalcemia.In addition,multiple electrolyte disturbances were found,including hypokalemia,hypocalcemia and hypomagnesemia.The physical signs showed a short fourth metatarsal bone of both feet.The X-ray images showed cortical thickening of long bones and narrowing of the medulla of the lumen.Cranial computed tomography indicated calcification in the bilateral basal ganglia.Finally,the genetic investigation showed a de novo heterogenous mutation of“FAM111A”(c.G1706A:p.R569H).Through a review of previously reported cases,the mutation was found to be the most common mutation site in Kenny-Caffey syndrome type 2(KCS2)cases reported thus far(16/23,69.6%).The mutation was slightly more prevalent in females than in males(11/16,68.8%).Except for hypocalcemia,other clinical manifestations are heterogeneous.CONCLUSION As a rare autosomal dominant genetic disease of hypoparathyroidism,the clinical manifestations of KCS2 are atypical and diverse.This girl presented with short stature,facial deformities and skeletal deformities.The laboratory results revealed hypocalcemia as the main electrolyte disturbance.Even though her family members showed normal phenotypes,gene detection was performed to find the mutation of the FAM111A gene and confirmed the diagnosis of KCS2. 展开更多

目的:研究使用宫腔镜下金属弯钩分离粘连术与常规电切术效果及其对宫腔粘连患者子宫内膜功能的影响。方法:选择在医院就诊的112例轻度和中度宫腔粘连患者,按随机数表法将其分为金属弯钩组和电切组,每组56例。两组患者均采用宫腔镜检查术... 目的:研究使用宫腔镜下金属弯钩分离粘连术与常规电切术效果及其对宫腔粘连患者子宫内膜功能的影响。方法:选择在医院就诊的112例轻度和中度宫腔粘连患者,按随机数表法将其分为金属弯钩组和电切组,每组56例。两组患者均采用宫腔镜检查术,金属弯钩组使用宫腔镜下金属弯钩分离粘连术,电切组使用常规电切术,对两种手术方式均进行3个月术后随访。观察两组术前、术后3个月月经量变化及子宫内膜厚度变化,术后3个月子宫内膜形态改善,术前、术后3个月转化生长因子-β(TGF-β)、Smad3蛋白以及干扰素-γ(IFN-γ)水平。结果:术后3个月,金属弯钩组患者月经量高于电切组,差异有统计学意义(t=2.04,P<0.05);术后3个月,两组患者子宫内膜厚度比较,差异无统计学意义(t=2.20,P>0.05);术后3个月,金属弯钩组患者子宫内膜形态改善率高于电切组,差异有统计学意义(x2=6.17,P<0.05);术后3个月,两组TGF-β、Smad 3蛋白及IFN-γ水平差异无统计学意义(t=0.16,t=0.80,t=1.84;P>0.05)。结论:两种手术方式均能治疗宫腔粘连,且对子宫内膜影响差异不明显;但宫腔镜下金属弯钩分离粘连术在短期内对患者月经量及子宫内膜形态改善率较好。 展开更多

目的探讨阴式修补术治疗Ⅱ型子宫瘢痕妊娠的临床疗效。方法选择2016年7月至2017年7月我院妇产科确诊收治的157例Ⅱ型子宫瘢痕妊娠(CSP)患者作为研究对象,按手术方法的不同分为清宫术组87例,阴式修补术组70例,比较2组患者手术出血量、手... 目的探讨阴式修补术治疗Ⅱ型子宫瘢痕妊娠的临床疗效。方法选择2016年7月至2017年7月我院妇产科确诊收治的157例Ⅱ型子宫瘢痕妊娠(CSP)患者作为研究对象,按手术方法的不同分为清宫术组87例,阴式修补术组70例,比较2组患者手术出血量、手术时间、住院时间、手术前后血人绒毛膜促性腺激素(β-HCG)水平、血β-HCG恢复至正常水平时间、手术前后血红蛋白(HB)水平、月经恢复时间、应激反应、炎性反应以及手术并发症。结果清宫术组患者术中出血量、手术时间以及住院时间少于阴式修补术组,2组比较差异有统计学意义(P<0.05);清宫术组患者术后血β-HCG恢复至正常时间长于阴式修补术组,差异有统计学意义(P<0.05); 2组患者术后Hb水平差异无统计学意义(P>0.05);阴式修补术组患者月经恢复时间短于清宫术组,差异有统计学意义(P<0.05);阴式修补术组患者术后肾上腺素(E)、皮质醇(Cor)水平低于清宫术组,促甲状腺激素(TSH)水平高于清宫术组,差异有统计学意义(P<0.05);阴式修补术组患者术后IL-2、IL-6与IL-8水平低于清宫术组,C反应蛋白(CRP)水平高于清宫术组,差异有统计学意义(P<0.05);清宫术组术后并发症发生率高于阴式修补术组,差异有统计学意义(P<0.05)。结论阴式修补术有利于缩短患者血β-HCG恢复正常时间以及月经回溯时间,加快患者机体内炎性因子以及激素恢复至正常水平,降低了术后并发症的发生率。 展开更多

目的:研究宫颈癌组织及细胞株中长链非编码RNA肠癌相关转录因子1(CCAT1)的表达水平及意义。方法:采用实时荧光定量PCR(qRT-PCR)检测2015年1月至2017年6月接受宫颈癌根治术的48例宫颈癌患者的癌组织、对应癌旁正常组织(距癌<3 cm)以... 目的:研究宫颈癌组织及细胞株中长链非编码RNA肠癌相关转录因子1(CCAT1)的表达水平及意义。方法:采用实时荧光定量PCR(qRT-PCR)检测2015年1月至2017年6月接受宫颈癌根治术的48例宫颈癌患者的癌组织、对应癌旁正常组织(距癌<3 cm)以及人正常宫颈鳞状细胞系ECT1/E6E7、宫颈癌细胞系Hela、C33A中CCAT1的表达,并分析其临床意义;应用小分子RNA(siRNA)分别转染人宫颈癌细胞系Hela、C33A,构建CCAT1低表达细胞系,采用细胞计数试剂盒(CCK-8)法检测CCAT1对人宫颈癌细胞增殖的影响,流式细胞仪检测各组细胞周期,蛋白印迹(Western blot)法检测CCAT1对人宫颈癌细胞中RAS相关区域家族1A蛋白(RASSF1A)、细胞周期蛋白D1(Cyclin D1)表达的影响。结果:与癌旁正常组织相比,人宫颈癌组织中CCAT1的表达显著升高(P<0.05);与正常宫颈细胞ECT1/E6E7相比,宫颈癌细胞系Hela、C33A中CCAT1的表达显著升高(P<0.05);TMN分期较晚、肿瘤大、有淋巴结转移的宫颈癌患者宫颈癌组织中CCAT1相对表达量更高(P<0.05);不同年龄、分化程度患者中CCAT1的表达无显著性差异(P>0.05);与siRNA-NC组相比,转染siRNA-CCAT1的Hela、C33A细胞中CCAT1、细胞增殖活性、cyclin D1蛋白的表达显著降低,RASSF1A蛋白的表达显著升高。结论:CCAT1的表达上调与宫颈癌发生发展密切相关,沉默CCAT1的表达会抑制宫颈癌细胞的增殖,可能与上调RASSF1A的表达、下调cyclin D1的表达有关。 展开更多

目的分析8-氨基异紫堇碱(8-NH2-ICD)对人高转移卵巢癌细胞(HO-8910PM)增殖和迁移的抑制作用。方法取对数生长期人上皮性卵巢癌(OC)HO-8910PM细胞15株,分为阴性对照组、药物组,各组培养24、48、72 h后,采用CCK-8检测药物干预后细胞增殖... 目的分析8-氨基异紫堇碱(8-NH2-ICD)对人高转移卵巢癌细胞(HO-8910PM)增殖和迁移的抑制作用。方法取对数生长期人上皮性卵巢癌(OC)HO-8910PM细胞15株,分为阴性对照组、药物组,各组培养24、48、72 h后,采用CCK-8检测药物干预后细胞增殖抑制率,Hoechst染色观察HO-8910PM细胞的凋亡形态变化,流式细胞技术、Transwell小室分别测定药物作用后细胞周期及迁移能力。结果随时间、药物浓度增加,HO-8910PM细胞增殖抑制率增加(P<0.05),在5~20μg/ml范围内有明显量效、时效关系,至20μg/ml时对细胞增殖抑制作用基本已达最大值;Hoechst 33258染色表明,阴性对照组细胞呈弥散均匀的淡蓝色荧光,添加8-NH2-ICD的细胞株出现细胞核固缩、荧光致密发亮、大小不同颗粒状强蓝色荧光等凋亡形态变化;8-NH2-ICD作用72 h后,与阴性对照组比较,浓度在5~20μg/ml时对细胞周期作用不明显,浓度为40μg/ml时S期细胞减少,G0/G1期细胞比例增加,8-NH2-ICD对HO-8910PM细胞周期阻滞在G0/G1期;处理24、48、72 h时药物组侵袭至滤镜下表面的细胞数明显减少,且低于阴性对照组,尤其是48、72 h时差异有统计学意义(P<0.05)。结论 8-NH2-ICD可明显抑制人OC细胞株HO-8910PM的增殖与迁移,促进细胞凋亡,且其对HO-8910PM细胞周期阻滞在G0/G1期。 展开更多

AIM: To elucidate the impact of hemodynamic parameters on ultrasonography and serum fibrosis markers for the assessment of liver fibrosis in the children with infant hepatitis syndrome (IHS). METHODS: Forty-one children with IHS and 46 healthy infants were examined by ultrasonography, and several hemodynamic indices such as peak systolic velocity (PSV) and resistant index (RI) of proper hepatic artery (PHA) were measured. Serum fibrosis markers including hyaluronic acid (HA), pre-collagen type-Ⅲ (PC-Ⅲ), collagen type Ⅳ (C-Ⅳ), and laminin (LN) were assayed by radioimmunoassays. In children with IHS, liver tissues were obtained either by ultrasound-guided liver biopsy (n = 35) or in the course of operation (n = 6). The stages of hepatic fibrosis were scored as mild (S1 and S2), moderate (S3), or severe (S4) according to liver histological diagnosis. Multiple groups comparative and Spearman correlative analyses were carried out. RESULTS: Histopathologically, 39 children (95.1%) were found to have hepatic fibrosis, 12 of them stage Sl or S2, 12 stage S3, and 15 stage S4. PSV, RI of the PHA, and serum HA showed a consecutive increase from mild to severe hepatic fibrosis and a close positive correlation with hepatic fibrosis in IHS group (r = 0.717, 0.745 and 0.712, respectively, P = 0.001). The Doppler waveform of HV was also positively correlated with the degree of hepatic fibrosis in IHS group (r = 0.783, P 〈 0.001).CONCLUSION: Combination of ultrasonic studies on the hepatic hemodynamics with the evaluation of serum HA may provide an indicator for hepatic fibrosis in patients with IHS, This may be a useful noninvasive method for the diagnosis and evaluation of the prognosis of IHS. 展开更多

Current guidelines encourage large studies in a diverse population to establish normal reference ranges for three-dimensional(3D)echocardiography for different ethnic groups.This study was designed to establish the normal values of 3D-left ventricular(LV)and left atrial(LA)volume and function in a nationwide,population-based cohort of healthy Han Chinese adults.A total of 1117 healthy volunteers aged 18–89 years were enrolled from 28 collaborating laboratories in China.Two sets of 3D echocardiographic instruments were used,and full-volume echocardiographic images were recorded and transmitted to a core laboratory for image analysis with a vendor-independent off-line workstation.Finally,866 volunteers(mean age of 48.4 years,402 men)were qualified for final analysis.Most parameters exhibited substantial differences between different sex and age groups,even after indexation by body surface area.The normal ranges of 3D-LV and 3D-LA volume and function differed from those recommended by the American Society of Echocardiography and the European Association of Cardiovascular Imaging guidelines,presented by the World Alliance Societies of Echocardiography(WASE)study,and from the 2D values in the EMINCA study.The normal reference values of 3D echocardiography-derived LV and LA volume and function were established for the first time in healthy Han Chinese adults.Normal ranges of 3D-LV and 3D-LA echocardiographic measurements stratified with sex,age,and race should be recommended for clinical applications. 展开更多

Global illumination(GI)plays a crucial role in rendering realistic results for virtual exhibitions,such as virtual car exhibitions.These scenarios usually include all-frequency bidirectional reflectance distribution functions(BRDFs),although their geometries and light configurations may be static.Rendering allfrequency BRDFs in real time remains challenging due to the complex light transport.Existing approaches,including precomputed radiance transfer,light probes,and the most recent path-tracing-based approaches(ReSTIR PT),cannot satisfy both quality and performance requirements simultaneously.Herein,we propose a practical hybrid global illumination approach that combines ray tracing and cached GI by caching the incoming radiance with wavelets.Our approach can produce results close to those of ofline renderers at the cost of only approximately 17 ms at runtime and is robust over all-frequency BRDFs.Our approach is designed for applications involving static lighting and geometries,such as virtual exhibitions. 展开更多
