为了更好地利用拮抗酵母菌控制果蔬采后病害,实现酵母制剂的商业化,作者以拮抗酵母菌汉逊德巴利酵母(Debaryomyces hansenii,D.hansenii)为研究对象,使用冷冻干燥技术制备酵母制剂,并研究该制剂对草莓采后软腐病的影响。首先通过单因素... 为了更好地利用拮抗酵母菌控制果蔬采后病害,实现酵母制剂的商业化,作者以拮抗酵母菌汉逊德巴利酵母(Debaryomyces hansenii,D.hansenii)为研究对象,使用冷冻干燥技术制备酵母制剂,并研究该制剂对草莓采后软腐病的影响。首先通过单因素试验研究冻干过程中不同保护剂在不同质量分数下对D.hansenii存活率的影响,随后利用响应面法优化保护剂复配方案,最后验证在最优复配方案的保护剂下D.hansenii冻干菌制剂在贮藏期间的存活率及其对草莓采后软腐病的控制效果。结果表明D.hansenii冻干菌制剂的最优保护剂方案为质量分数2.22%的海藻糖、质量分数17.83%的脱脂乳粉、质量分数0.06%的L-抗坏血酸和质量分数4.06%的L-谷氨酸钠。D.hansenii在最优复配方案得到的冻干菌制剂长达90 d的贮藏期内能够保持70%~80%的存活率,且该制剂对由匍枝根霉(Rhizopus stolonifer,R.stolonifer)引起的草莓采后软腐病具有良好的控制效果。 展开更多

目的分析南京鼓楼医院新型冠状病毒感染(COVID-19)期间互联网抗凝门诊与传统抗凝门诊治疗效果及成本效益。方法回顾收集2020年6月1日—2021年6月30日在南京鼓楼医院互联网抗凝门诊与传统抗凝门诊接受抗凝管理患者的临床资料。计算纳入... 目的分析南京鼓楼医院新型冠状病毒感染(COVID-19)期间互联网抗凝门诊与传统抗凝门诊治疗效果及成本效益。方法回顾收集2020年6月1日—2021年6月30日在南京鼓楼医院互联网抗凝门诊与传统抗凝门诊接受抗凝管理患者的临床资料。计算纳入患者治疗范围内时间(TTR)和国际标准化比值(INR)变异性,患者同时满足TTR≥60%和INR变异性<0.65时,认为抗凝质量优。从患者角度分析成本效果,以增量成本效果比表示结果。结果互联网抗凝门诊和传统抗凝门诊分别有19例(67.86%)和67例(76.14%)达到抗凝质量优(P>0.05),平均TTR分别为(71.83±19.17)%和(71.74±23.41)%,两组具有相同的华法林抗凝效果,以实际成本计算,增量成本效果比为327.17元,低于2021年人均国内生产总值。结论COVID-19流行期间,南京鼓楼医院互联网抗凝门诊诊疗服务有效配合执行政府防疫政策,在保证相同抗凝效果的同时,明显节约患者经济成本,为后疫情时代优化医药卫生服务提供了新思路和新启示。 展开更多

Tyrosine kinase inhibitors(TKIs)have emerged as the first-line small molecule drugs in many cancer therapies,exerting their effects by impeding aberrant cell growth and proliferation through the modulation of tyrosine kinase-mediated signaling pathways.However,there exists a substantial inter-individual variability in the concentrations of certain TKIs and their metabolites,which may render patients with compromised immune function susceptible to diverse infections despite receiving theoretically efficacious anticancer treatments,alongside other potential side effects or adverse reactions.Therefore,an urgent need exists for an up-to-date review concerning the biological matrices relevant to bioanalysis and the sampling methods,clinical pharmacokinetics,and therapeutic drug monitoring of different TKIs.This paper provides a comprehensive overview of the advancements in pretreatment methods,such as protein precipitation(PPT),liquid-liquid extraction(LLE),solid-phase extraction(SPE),micro-SPE(μ-SPE),magnetic SPE(MSPE),and vortex-assisted dispersive SPE(VA-DSPE)achieved since 2017.It also highlights the latest analysis techniques such as newly developed high performance liquid chromatography(HPLC)and high-resolution mass spectrometry(HRMS)methods,capillary electrophoresis(CE),gas chromatography(GC),supercritical fluid chromatography(SFC)procedures,surface plasmon resonance(SPR)assays as well as novel nanoprobes-based biosensing techniques.In addition,a comparison is made between the advantages and disadvantages of different approaches while presenting critical challenges and prospects in pharmacokinetic studies and therapeutic drug monitoring. 展开更多

目的探讨抗菌肽LL-37和维生素D受体(VDR)在呼吸机相关性肺损伤(VILI)大鼠肺组织表达及作用机制。方法30只洁净级SD大鼠(雌雄各半)随机分为自主呼吸组,正常潮气量组(VT=6 ml/kg)和大潮气量组(VT=40 ml/kg),每组10只,雌雄各半,3组大鼠经... 目的探讨抗菌肽LL-37和维生素D受体(VDR)在呼吸机相关性肺损伤(VILI)大鼠肺组织表达及作用机制。方法30只洁净级SD大鼠(雌雄各半)随机分为自主呼吸组,正常潮气量组(VT=6 ml/kg)和大潮气量组(VT=40 ml/kg),每组10只,雌雄各半,3组大鼠经腹腔注射麻醉后行气管切开插管术。其中自主呼吸组保持大鼠自主呼吸,正常潮气量组按6 ml/kg给予机械通气,大潮气量组按40 ml/kg给予机械通气,4 h后经心脏穿刺放血,处死大鼠。采用精密电子秤称量肺湿干比重(W/D),HE染色观察肺形态变化,透射电镜观察Ⅱ型肺泡超微结构,ELISA检测血清和BALF中的白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、髓过氧化物酶(MPO)和人巨噬细胞炎性蛋白2(MIP2)促炎因子的水平,Western-blot检测肺组织中抗菌肽LL-37和维生素D受体蛋白表达。结果大潮气量组W/D明显高于自主呼吸组和正常潮气量组(P<0.05),而正常潮气量组W/D明显高于自主呼吸组(P<0.05);HE染色结果显示大潮气量组有明显的水肿、肺间质变宽,且肺泡充血,同时伴随有大量的炎性细胞浸润,肺泡组织紊乱,大潮气量组病理评分明显高于自主呼吸组和正常潮气量组(P<0.05),而正常潮气量组病理评分明显高于自主呼吸组(P<0.05);大潮气量组AT-Ⅱ细胞的细胞膜、细胞质以及细胞核与自主呼吸组和正常潮气量组相比出现明显改变,细胞膜微绒毛随着潮气量增大而减少,薄膜增厚,细胞质中的板层小体数量减少,分布不均,细胞核形态不一,出现固缩;大潮气量组血清和BALF中IL-6、TNF-α、MPO和MIP2明显高于自主呼吸组和正常潮气量组(P<0.05),而正常潮气量组血清和BALF中IL-6、TNF-α、MPO和MIP2明显高于自主呼吸组(P<0.05);大潮气量组LL-37和VDR明显低于自主呼吸组和正常潮气量组(P<0.05),而正常潮气量组LL-37和VDR明显低于自主呼吸组(P<0.05)。结论机械通气引起的肺损伤会导致抗菌肽LL-37和维生素D受体表达下调,免疫功能紊乱,从而介导或者加重VILI。 展开更多

在水产养殖中,鱼类的摄食强度可以反映其食欲,准确客观地评估鱼类的摄食强度对指导投喂和生产实践具有重要意义。针对当前鱼类摄食强度评估过程中存在的人工观测效率低、客观性不强的问题,本研究以实现鱼类食欲的自动客观分析为目的,提... 在水产养殖中,鱼类的摄食强度可以反映其食欲,准确客观地评估鱼类的摄食强度对指导投喂和生产实践具有重要意义。针对当前鱼类摄食强度评估过程中存在的人工观测效率低、客观性不强的问题,本研究以实现鱼类食欲的自动客观分析为目的,提出了一种基于近红外机器视觉的游泳型鱼类摄食强度的评估方法。首先,利用近红外工业相机搭建了近红外图像采集系统,采集了鱼类摄食过程中的图像。经过一系列图像处理步骤后,利用灰度共生矩阵提取摄食图像的纹理特征变量信息,包括对比度、能量、相关性、逆差距和熵等。之后,将这5个特征变量作为输入向量构建了模型的数据集,并训练了支持向量机分类器。为了提高模型分类的准确率,利用网格搜索法选取支持向量机分类器的最优惩罚系数c和核函数参数g。最后利用训练好的模型将鱼类的摄食强度分为弱、一般、中和强4类,最终实现了鱼类摄食强度的评估。试验结果表明,图像纹理可以较好地描述鱼类摄食过程中的行为变化,正确识别4类摄食强度的准确率达到87.78%,且不需要考虑水花等对成像质量的影响,具有较强的适应性。本方法可用于鱼类食欲的自动客观评估,为后续投喂决策提供理论依据和方法支持。 展开更多

设计能高效溶解木质素的溶剂对木质素的高值化利用具有重要意义。本文中设计了基于氯化胆碱、甜菜碱和左旋肉碱作为氢键受体(HBA)和四种氢键供体(HBD)的生物质衍生的酸性低共熔溶剂(DESs),可以溶解包括碱木质素(AL)、脱碱木质素(DAL)、... 设计能高效溶解木质素的溶剂对木质素的高值化利用具有重要意义。本文中设计了基于氯化胆碱、甜菜碱和左旋肉碱作为氢键受体(HBA)和四种氢键供体(HBD)的生物质衍生的酸性低共熔溶剂(DESs),可以溶解包括碱木质素(AL)、脱碱木质素(DAL)、酶解木质素(EHL)和硫酸盐木质素(KL)在内的不同类型的木质素。在大多数所设计的DESs中,EHL比AL、KL和DAL更容易溶解,而不同木质素中羟基的含量对木质素的溶解有显著影响,但是并非在所有DESs中木质素的溶解情况都符合上述规则。氯化胆碱是构建DESs的首选HBA,具有良好的性能并适应于不同类型木质素的溶解,而合适的酸度使苯甲酸和没食子酸乙酯成为对木质素溶解有利的HBDs。研究表明能有效溶解木质素的DESs应具有强的氢键酸度(α值>0.95)以及与溶解的木质素匹配的合适极性。此外,HBD的pKa值和DESs的酸度也是评价酸性DESs溶解木质素性能的有效指标。通常具有适中pKa值的HBDs能够用于构建具有高效的木质素溶解性能的DESs。DESs的粘度对木质素溶解也有一定影响,较低的粘度有助于木质素溶解。 展开更多

目的:探讨超微血管成像(super micro-vascular imaging,SMI)和彩色多普勒超声血流显像(color Doppler flow imaging,CDFI)联合妇科影像报告和数据系统(gynecologic imaging reporting and data system,GI-RADS)对卵巢肿瘤良恶性的评估... 目的:探讨超微血管成像(super micro-vascular imaging,SMI)和彩色多普勒超声血流显像(color Doppler flow imaging,CDFI)联合妇科影像报告和数据系统(gynecologic imaging reporting and data system,GI-RADS)对卵巢肿瘤良恶性的评估价值。方法:选取2019年9月至2021年3月于河北省廊坊市人民医院诊治的168例卵巢肿瘤患者的临床资料,分为恶性组56例、良性组112例,采用SMI和CDFI分别观察同一肿瘤中血流情况,并联合GI-RADS分别预测肿瘤良恶性,结果以术后病理组织诊断为金标准。结果:恶性组中SMI检测血流的丰富程度显著高于CDFI检测(P<0.05),良性组中SMI和CDFI检测血流的丰富程度进行比较差异无统计学意义(P>0.05)。采用CDFI联合GI-RADS和SMI联合GI-RADS诊断恶性的分别为53例和55例,敏感度分别为94.6%和98.2%、特异度分别为93.8%和95.5%、准确率分别为94.0%和96.4%,均与病理结果的一致性较好(κ=0.868和κ=0.921)。结论:SMI联合GI-RADS对卵巢肿瘤的良恶性具有较好的鉴别诊断效能,较CDFI联合GI-RADS诊断时敏感度、特异度更高。 展开更多

AIM: To evaluate the therapeutic effects of bone marrow-derived mesenchymal stem cells(BMSCs) with human urokinase-type plasminogen activator(u PA) on liver fibrosis, and to investigate the mechanism of gene therapy.METHODS: BMSCs transfected with adenovirusmediated human urokinase plasminogen activator(Adu PA) were transplanted into rats with CCl4-induced liver fibrosis. All rats were sacrificed after 8 wk, and their serum and liver tissue were collected for biochemical, histopathologic, and molecular analyzes. The degree of liver fibrosis was assessed by hematoxylin and eosin or Masson's staining. Western blot and quantitative reverse transcription-polymerase chain reaction were used to determine protein and m RNA expression levels.RESULTS: Serum levels of alanine aminotransferase, aminotransferase, total bilirubin, hyaluronic acid, laminin, and procollagen type Ⅲ were markedly decreased, whereas the levels of serum albumin were increased by u PA gene modified BMSCs treatment. Histopathology revealed that chronic CCl4-treatment resulted in significant fibrosis while u PA gene modified BMSCs treatment significantly reversed fibrosis. By quantitatively analysing the fibrosis area of liver tissue using Masson staining in different groups of animals, we found that model animals with CCl4-induced liver fibrosis had the largest fibrotic area(16.69% ± 1.30%), while fibrotic area was significantly decreased by BMSCs treatment(12.38% ± 2.27%) and was further reduced by u PA-BMSCs treatment(8.31% ± 1.21%). Both protein and m RNA expression of β-catenin, Wnt4 and Wnt5 a was down-regulated in liver tissues following u PA gene modified BMSCs treatment when compared with the model animals.CONCLUSION: Transplantation of u PA gene modified BMSCs suppressed liver fibrosis and ameliorated liver function and may be a new approach to treating liver fibrosis. Furthermore, treatment with u PA gene modified BMSCs also resulted in a decrease in expression of molecules of the Wnt signaling pathway. 展开更多

BACKGROUND Enteral nutrition(EN)is essential for critically ill patients.However,some patients will have enteral feeding intolerance(EFI)in the process of EN.AIM To develop a clinical prediction model to predict the risk of EFI in patients receiving EN in the intensive care unit.METHODS A prospective cohort study was performed.The enrolled patients’basic information,medical status,nutritional support,and gastrointestinal(GI)symptoms were recorded.The baseline data and influencing factors were compared.Logistic regression analysis was used to establish the model,and the bootstrap resampling method was used to conduct internal validation.RESULTS The sample cohort included 203 patients,and 37.93%of the patients were diagnosed with EFI.After the final regression analysis,age,GI disease,early feeding,mechanical ventilation before EN started,and abnormal serum sodium were identified.In the internal validation,500 bootstrap resample samples were performed,and the area under the curve was 0.70(95%CI:0.63-0.77).CONCLUSION This clinical prediction model can be applied to predict the risk of EFI. 展开更多

BACKGROUND:Swallowing disorder is a common clinical symptom that can lead to a series of complications,including aspiration,aspiration pneumonia,and malnutrition.This study aimed to investigate risk factors of post-extubation dysphagia(PED)in intensive care unit(ICU)patients with endotracheal intubation,and to develop a risk-predictive model for PED,which could serve as an assessment tool for the prevention and control of PED.METHODS:Patients retrospectively selected from June to December 2021 in a tertiary hospital served as the derivation cohort.Patients recruited from the same hospital from March to June 2022served as the external validation cohort for the predictive model.We used a combination of variable screening and least absolute shrinkage and selection operator(LASSO)regression to select the most useful candidate predictors and checked the multicollinearity of independent variables using the variance inflation factor method.Multivariate logistic regression analysis was performed to calculate the odds ratio(OR;95%confidence interval[95%CI])and P-value for each variable to predict diagnosis.The screened risk factors were introduced into R software to build a nomogram model.The performance of the model,including discrimination ability,calibration,and clinical benefit,was evaluated by plotting the receiver operating characteristic(ROC),calibration,and decision curves.RESULTS:A total of 305 patients were included in this study.Among them,235 patients(53PED vs.182 non-PED)were enrolled in the derivation cohort,while 70 patients(17 PED vs.53 nonPED)were enrolled in the validation cohort.The independent predictors included age,pause of sedatives,level of consciousness,activities of daily living(ADL)score,nasogastric tube,sore throat,and voice disorder.These predictors were used to establish the predictive nomogram model.The model demonstrated good discriminative ability,and the area under the ROC curve(AUC)was 0.945(95%CI 0.904-0.970).Applying the predictive model to the validation cohort demonstrated good discrimination with an AUC of 0.907(95%CI 0.831-0.983)and good calibration.The decision-curve analysis of this nomogram showed a net benefit of the model.CONCLUSION:A predictive model that incorporates age,pause of sedatives,level of consciousness,ADL score,nasogastric tube,sore throat,and voice disorder may have the potential to predict PED in ICU patients. 展开更多

BACKGROUND Joint,skin,oral cavity,and eye lesions are the most common extraintestinal manifestations of ulcerative colitis that can occur before or after its onset.The cases of ulcerative colitis with dermatomyositis(DM)are rare.In this study,we report a rare case of ulcerative colitis with DM that was effectively treated with infliximab.CASE SUMMARY The patient was a 57-year-old female with a 2-year history of DM.The patient was admitted to hospital with abdominal pain,diarrhea,and blood in stool lasting for more than 2 mo.Colonoscopy revealed multiple erosions and ulcers in the entire colon and rectum.Pathological sections showed chronic inflammatory cell infiltration,especially neutrophil infiltration,in the colonic mucosa;therefore,the patient was diagnosed with ulcerative colitis.Preparations of 5-aminosalicylic acid was added to her treatment based on the original treatment for DM,but its effect was unsatisfactory.The patient’s discomfort was relieved after infliximab treatment.CONCLUSION Infliximab can improve DM in the treatment of ulcerative colitis.Specialists need to raise awareness about patients with inflammatory bowel disease who have rare extraintestinal manifestations. 展开更多

The ears are air-filled structures that are directly impacted during blast exposure.In addition to hearing loss and tinnitus,blast victims often complain of vertigo,dizziness and unsteady posture,suggesting that blast exposure induces damage to the vestibular end organs in the inner ear.However,the underlying mechanisms remain to be elucidated.In this report,single vestibular afferent activity and the vestibuloocular reflex(VOR)were investigated before and after exposure to blast shock waves(~20 PSI)delivered into the left external ear canals of anesthetized rats.Single vestibular afferent activity was recorded from the superior branch of the left vestibular nerves of the blast-treated and control rats one day after blast exposure.Blast exposure reduced the spontaneous discharge rates of the otolith and canal afferents.Blast exposure also reduced the sensitivity of irregular canal afferents to sinusoidal head rotation at 0.5e2Hz.Blast exposure,however,resulted in few changes in the VOR responses to sinusoidal head rotation and translation.To the best of our knowledge,this is the first study that reports blast exposure-induced damage to vestibular afferents in an animal model.These results provide insights that may be helpful in developing biomarkers for early diagnosis of blast-induced vestibular deficits in military and civilian populations. 展开更多

AIM: To study the polymorphisms of toll-like receptor 4 (TLR4) gene Asp299Gly, Thr399Ile and TLR2 gene Arg753Gln, Arg677Trp and susceptibility to inflammatory bowel disease (IBD) in the Zhuang population from Guangxi, China. METHODS: A case-control study was performed from February 2007 to October 2011 which included 146 Zhuang patients with IBD in the experimental group and 164 healthy Zhuang subjects who acted as the control group. All patients and healthy subjects were from the Guangxi Zhuang Autonomous Region of China. Genomic DNA was extracted from intestinal tissue by the phenol chloroform method. TLR4 gene Asp299Gly, Thr399Ile and TLR2 gene Arg753Gln, Arg677Trp were amplified by polymerase chain reaction (PCR), and then detected by PCR-restriction fragment length polymorphism (RFLP). RESULTS: The TLR4 gene Asp299Gly was digested using Nco Ⅰ restriction enzyme, and a single band of 249 bp was observed which showed that it was a wild type (AA). The TLR4 gene Thr399Ile was digested using Hinf Ⅰrestriction enzyme and only the wild type (CC) was detected. In addition, the TLR2 gene Arg-677Trp was digested using Aci Ⅰ restriction enzyme and only the wild type (CC) was detected. The TLR2 gene Arg753Gln was digested using Pst Ⅰ restriction enzyme. Only the wild type (GG) as a single band of 254 bp was observed during RFLP. Overall, no heterozygous or homozygous single nucleotide polymorphism mutations were found in patients with Crohn's disease and ulcerative colitis both in the TLR4 gene Asp299Gly, Thr399Ile and the TLR2 gene Arg677Trp, Arg753Gln in the Zhuang population from the Guangxi Zhuang Autonomous Region of China. CONCLUSION: The TLR4 gene Asp299Gly, Thr399Ile and TLR2 gene Arg753Gln, Arg677Trp polymorphisms may not be associated with IBD in the Zhuang population from the Guangxi Zhuang Autonomous Region of China. 展开更多

Blast overpressure has become an increasing cause of brain injuries in both military and civilian populations. Though blast's direct effects on the cochlea and vestibular organs are active areas of study, little attention has been given to the ear's contribution to the overall spectrum of blast injury. Acute auto- nomic responses to blast exposure, including bradycardia and hypotension, can cause hypoxia and contribute to blast-induced neurotrauma. Existing literature suggests that these autonomic responses are elicited through blast impacting the thorax and lungs. We hypothesize that the unprotected ear also provides a vulnerable locus for blast to cause autonomic responses. We designed a blast generator that delivers controlled overpressure waves into the ear canal without impacting surrounding tissues in order to study the ear's specific contribution to blast injury. Anesthetized adult rats' left ears were exposed to a single blast wave ranging from 0 to 110 PSI (0-758 kPa). Blast exposed rats exhibited decreased heart rates and blood pressures with increased blast intensity, similar to results gathered using shock tubes and whole-body exposure in the literature. While rats exposed to blasts below 50 PSI (345 kPa) exhibited increased respiratory rate with increased blast intensity, some rats exposed to blasts higher than 50 PSI (345 kPa) stopped breathing immediately and ultimately died. These autonomic responses were significantly reduced in vagally denervated rats, again similar to whole-body exposure literature. These results support the hypothesis that the unprotected ear contributes to the autonomic responses to blast. 展开更多
