Background:Our clinical practice of laparoscopic liver resection(LLR)had achieved better short-term and long-term benefits for patients with hepatocellular carcinoma(HCC)over open liver resection(OLR),but the underlyi...Background:Our clinical practice of laparoscopic liver resection(LLR)had achieved better short-term and long-term benefits for patients with hepatocellular carcinoma(HCC)over open liver resection(OLR),but the underlying mechanisms are not clear.This study was to find out whether systemic inflammation plays an important role.Methods:A total of 103 patients with early-stage HCC under liver resection were enrolled(LLR group,n=53;OLR group,n=50).The expression of 9 inflammatory cytokines in patients at preoperation,postoperative day 1(POD1)and POD7 was quantified by Luminex Multiplex assay.The relationships of the cytokines and the postoperative outcomes were compared between LLR and OLR.Results:Seven of the circulating cytokines were found to be significantly upregulated on POD1 after LLR or OLR compared to their preoperative levels.Compared to OLR,the POD1 levels of granulocytemacrophage colony-stimulating factor(GM-CSF),interleukin-6(IL-6),IL-8,and monocyte chemoattractant protein-1(MCP-1)in the LLR group were significantly lower.Higher POD1 levels of these cytokines were significantly correlated with longer operative time and higher volume of blood loss during operation.The levels of these cytokines were positively associated with postoperative liver injury,and the length of hospital stay.Importantly,a high level of IL-6 at POD1 was a risk factor for HCC recurrence and poor disease-free survival after liver resection.Conclusions:Significantly lower level of GM-CSF,IL-6,IL-8,and MCP-1 after liver resection represented a milder systemic inflammation which might be an important mechanism to offer better short-term and long-term outcomes in LLR over OLR.展开更多

BACKGROUND:The aim of the present study is to describe the clinical correlates of hypotension and its associated outcomes in patients with acute organophosphorus poisoning(AOPP).METHODS:In this retrospective cohort st...BACKGROUND:The aim of the present study is to describe the clinical correlates of hypotension and its associated outcomes in patients with acute organophosphorus poisoning(AOPP).METHODS:In this retrospective cohort study,we analyzed data pertaining to 871 patients with AOPP who were treated at two hospitals.Data from hypotensive and non-hypotensive patients were compared to identify clinical correlates of hypotension.We also evaluated the association between clinical parameters(including hypotension)and in-hospital mortality.RESULTS:The incidence of hypotension in AOPP patients was 16.4%.Hypotensive patients showed signifi cantly higher in-hospital mortality(1.1%vs.39.9%,P<0.001).Advanced age(odds ratio[OR]1.25,95%confi dence interval[CI]1.08–1.44),history of diabetes(OR 2.65,95%CI 1.14–5.96),and increased white blood cell count(OR 1.06,95%CI 1.03–1.09),plasma cholinesterase(OR 0.91,95%CI 0.84–0.94),plasma albumin(OR 0.88,95%CI 0.85–0.92),serum amylase(OR 1.01,95%CI 1.01–1.02),and blood pH(OR 0.64,95%CI 0.54–0.75)were signifi cantly associated with hypotension.After adjusting for potential confounders,hypotension was associated with increased in-hospital mortality(hazard ratio 8.77–37.06,depending on the controlled variables).CONCLUSIONS:Hypotension is a common complication of AOPP and is associated with increased in-hospital mortality.Advanced age,history of diabetes,and changes in laboratory parameters were associated with hypotension in AOPP patients.展开更多

However, th...Nuclear magnetic resonance-visible mobile lipid, at 1.28 parts per million (ppm), is thought to be due to mobile lipid droplets formed in cells and has been considered unique for neural progenitor cells. However, this idea remains controversial. The present study examined the 1.28 ppm biomarker in other stem cells and non-stem cells, and explored the relationship between 1.28 ppm biomarker and mobile lipid droplets. 1H nuclear magnetic resonance spectroscopy of EC109 cells, mesenchymal stem cells (MSCs) and adipogenic cells differentiated from MSCs was performed. Results show that 1.28 ppm biomarker was observed in human MSCs, but was absent from EC109 cells. Following adipogenic differentiation induced for 2 weeks, the 1.28 ppm biomarker climbed remarkably, with mobile lipid droplet generation, suggesting that the 1.28 ppm biomarker is not specific for neural progenitor cells because it is also observed in MSCs and adipogenic-induced differentiated cells. Moreover, it is possible to monitor MSCs differentiation following cell transplantation, using 1.28 ppm biomarker changes.展开更多

BACKGROUND Choriocarcinoma is a highly malignant trophoblastic tumor that presents with early symptoms similar to those of an ectopic pregnancy.Here we present a patient with suspected ectopic pregnancy diagnosed by l...BACKGROUND Choriocarcinoma is a highly malignant trophoblastic tumor that presents with early symptoms similar to those of an ectopic pregnancy.Here we present a patient with suspected ectopic pregnancy diagnosed by laparoscopic surgery in our hospital.The patient was found to have choriocarcinoma that had metastasized to the lumbar muscle and presented with symptoms similar to those of an ectopic pregnancy.CASE SUMMARY The patient was a 34-year-old female who complained of amenorrhea lasting 53 d,7 d of right lower back pain,and 3 d of right lower abdominal pain.Transvaginal ultrasonography revealed the absence of a gestational sac in the uterus and a mass in the left adnexa.After 6 d of re-examination,ultrasound and computed tomography(CT)examination were performed on the mass located in the left adnexa area.We also noted that the patient's serumβ-human chorionic gonadotropin(h CG)level was increased.Considering an ectopic pregnancy,we performed a laparoscopy and hysteroscopy.During the operation,a left ovarian mixed echogenic mass approximately 2.5 cm×2.0 cm with no villous tissue was found.Postoperative levels of serum h CG continued to increase.Lung CT examination showed lung nodules.Both CT and magnetic resonance imaging showed a mixed echogenic mass in the lumbar muscle.Considering lumbar metastasis of choriocarcinoma,six courses of cisplatin,dactinomycin,and etoposide chemotherapy were given after surgery.The patient's serumβ-h CG level decreased to normal and the mixed echogenic mass in the lumbar muscle decreased in size after the fifth course of chemotherapy.All symptoms subsequently disappeared after treatment.CONCLUSION In summary,lumbar metastasis from choriocarcinoma is extremely rare.Appropriate chemotherapy can successfully treat these metastasized tumors.展开更多

The results indicated that MAH modifiedβ-CD (MAH-β-CD) could polymerize to NIPAM by UV light irradiation in the presence of TiO2-MWCNTs composite nanoparticles. The characteristic results confirmed that the TiO2-MWCNTs composite nanoparticles were embedded evenly within the thermally responsive copolymer of NIPAM and MAH-β-CD. The effects of irradiation time and TiO2-MWCNTs concentration on the yield of the composites were investigated by keeping NIPAM to MAH-β- CD mass ratio constant. The optimal polymerization reaction conditions were a TiO2-MWCNTs concentration of 10 wt.% under UV light for the illumination of 3 h.展开更多
