Diabetic retinopathy(DR),as one of the most common and significant microvascular complications of diabetes mellitus(DM),continues to elude effective targeted treatment for vision loss despite ongoing enrichment of the understanding of its pathogenic mechanisms from perspectives such as inflammation and oxidative stress.Recent studies have indicated that characteristic neuroglial degeneration induced by DM occurs before the onset of apparent microvascular lesions.In order to comprehensively grasp the early-stage pathological changes of DR,the retinal neurovascular unit(NVU)will become a crucial focal point for future research into the occurrence and progression of DR.Based on existing evidence,ferroptosis,a form of cell death regulated by processes like ferritinophagy and chaperone-mediated autophagy,mediates apoptosis in retinal NVU components,including pericytes and ganglion cells.Autophagy-dependent ferroptosis-related factors,including BECN1 and FABP4,may serve as both biomarkers for DR occurrence and development and potentially crucial targets for future effective DR treatments.The aforementioned findings present novel perspectives for comprehending the mechanisms underlying the early-stage pathological alterations in DR and open up innovative avenues for investigating supplementary therapeutic strategies. 展开更多

目的 研究丙酮酸盐腹膜透析液腹腔复苏对失血性休克复苏后大鼠肾脏缺血再灌注损伤的保护作用及其可能机制。方法 40只健康雄性SD大鼠随机分为假手术组(Sham组)、静脉复苏组(VR组)、生理盐水腹腔复苏组(NR组)、丙酮酸盐腹膜透析液腹腔复... 目的 研究丙酮酸盐腹膜透析液腹腔复苏对失血性休克复苏后大鼠肾脏缺血再灌注损伤的保护作用及其可能机制。方法 40只健康雄性SD大鼠随机分为假手术组(Sham组)、静脉复苏组(VR组)、生理盐水腹腔复苏组(NR组)、丙酮酸盐腹膜透析液腹腔复苏组(PR组)4组,每组10只。VR组、NR组与PR组制备失血性休克大鼠模型,Sham组仅行右颈总动脉、左股动脉、右股静脉穿刺及全身肝素化。复苏后24 h采集标本,使用血液分析仪检测肌酐和尿素氮浓度,HE染色观察肾组织病理学结果并进行Paller评分,ELISA法检测肾组织IL-1β、TNF-α含量,Western blot法测定p-STAT3、p-JAK2的表达水平。结果 与Sham组相比,VR组、NR组与PR组肾小管Paller评分、血清肌酐及尿素氮水平均升高,IL-1β、TNF-α含量增加,p-STAT3、p-JAK2表达水平上调(P <0.05)。与VR组、NR组相比,PR组肾小管Paller评分、血清肌酐及尿素氮水平降低,IL-1β、TNF-α含量减少,p-STAT3、p-JAK2表达水平下调(P <0.05)。结论 丙酮酸盐腹膜透析液腹腔复苏能够减轻失血性休克复苏后肾缺血再灌注损伤,改善肾功能,其机制可能与降低炎症反应、抑制JAK2/STAT3信号通路的激活有关。 展开更多

AIM:To determine if serum inter-cellular adhesion molecule 1(ICAM-1)is an early marker of the diagnosis and prediction of severe acute pancreatitis(SAP) within 24 h of onset of pain,and to compare the sensitivity,specificity and prognostic value of this test with those of acute physiology and chronic health evaluation(APACHE)Ⅱscore and interleukin-6(IL-6). METHODS:Patients with acute pancreatitis(AP)were divided into two groups according to the Ranson's criteria:mild acute pancreatitis(MAP)group and SAP group.Serum ICAM-1,APACHEⅡand IL-6 levels were detected in all the patients.The sensitivity,specificity and prognostic value of the ICAM-1,APACHEⅡscore and IL-6 were evaluated. RESULTS:The ICAM-1 level in 36 patients with SAP within 24 h of onset of pain was increased and was significantly higher than that in the 50 patients with MAP and the 15 healthy volunteers(P<0.01).The ICAM-1 level(25 ng/mL)was chosen as the optimum cutoff to distinguish SAP from MAP,and the sensitivity,specificity,positive predictive value,negative predictive value(NPV),positive likelihood ratio and negative likelihood ratio were 61.11%,71.42%,0.6111,0.7142, 2.1382 and 0.5445,respectively.The area under the curve demonstrated that the prognostic accuracy of ICAM-1(0.712)was similar to the APACHE-Ⅱscoring system(0.770)and superior to IL-6(0.508)in distinguishing SAP from MAP. CONCLUSION:ICAM-1 test is a simple,rapid and reliable method in clinical practice.It is an early marker of diagnosis and prediction of SAP within the first 24 h after onset of pain or on admission.As it has a relatively low NPV and does not allow it to be a stand-alone test for the diagnosis of AP,other conventional diagnostic tests are required. 展开更多

Research showed that Catastrophe Theory was applicable in the RDWSA. Adding RDWSA based on Catastrophe Theory to the supportive system for decision-making in Ecological Hydrographic Management Decision Support System of Ya’an helped obtain the rank and results of RDWSE by the automatic calculation of programs, which could assist the risk assessment and risk management associated with rural drinking water in Ya’an. 展开更多
