BACKGROUND Portal hypertension(PHT),primarily induced by cirrhosis,manifests severe symptoms impacting patient survival.Although transjugular intrahepatic portosystemic shunt(TIPS)is a critical intervention for managi...BACKGROUND Portal hypertension(PHT),primarily induced by cirrhosis,manifests severe symptoms impacting patient survival.Although transjugular intrahepatic portosystemic shunt(TIPS)is a critical intervention for managing PHT,it carries risks like hepatic encephalopathy,thus affecting patient survival prognosis.To our knowledge,existing prognostic models for post-TIPS survival in patients with PHT fail to account for the interplay among and collective impact of various prognostic factors on outcomes.Consequently,the development of an innovative modeling approach is essential to address this limitation.AIM To develop and validate a Bayesian network(BN)-based survival prediction model for patients with cirrhosis-induced PHT having undergone TIPS.METHODS The clinical data of 393 patients with cirrhosis-induced PHT who underwent TIPS surgery at the Second Affiliated Hospital of Chongqing Medical University between January 2015 and May 2022 were retrospectively analyzed.Variables were selected using Cox and least absolute shrinkage and selection operator regression methods,and a BN-based model was established and evaluated to predict survival in patients having undergone TIPS surgery for PHT.RESULTS Variable selection revealed the following as key factors impacting survival:age,ascites,hypertension,indications for TIPS,postoperative portal vein pressure(post-PVP),aspartate aminotransferase,alkaline phosphatase,total bilirubin,prealbumin,the Child-Pugh grade,and the model for end-stage liver disease(MELD)score.Based on the above-mentioned variables,a BN-based 2-year survival prognostic prediction model was constructed,which identified the following factors to be directly linked to the survival time:age,ascites,indications for TIPS,concurrent hypertension,post-PVP,the Child-Pugh grade,and the MELD score.The Bayesian information criterion was 3589.04,and 10-fold cross-validation indicated an average log-likelihood loss of 5.55 with a standard deviation of 0.16.The model’s accuracy,precision,recall,and F1 score were 0.90,0.92,0.97,and 0.95 respectively,with the area under the receiver operating characteristic curve being 0.72.CONCLUSION This study successfully developed a BN-based survival prediction model with good predictive capabilities.It offers valuable insights for treatment strategies and prognostic evaluations in patients having undergone TIPS surgery for PHT.展开更多

Among the numerous transition metal catalysts,manganese-based compounds are considered as promising peroxymonosulfate(PMS)catalysts due to their low cost and environmental friendliness,such as cryptomelane manganese o...Among the numerous transition metal catalysts,manganese-based compounds are considered as promising peroxymonosulfate(PMS)catalysts due to their low cost and environmental friendliness,such as cryptomelane manganese oxide(K_(2-x)Mn_(8)O_(16):abbreviation KMnO).However,the limited catalytic performance of KMnO limits its practical application.In this work,iron-doped KMnO(Fe-KMnO)was prepared by one-step hydrothermal method to optimize its catalytic performance.Compared with KMnO/PMS system,Fe-KMnO/PMS system possessed more excellent removal efficiency of tetracycline(TC).Meanwhile,the Fe-KMnO/PMS system also exhibited good practical application potential and excellent stability.The mechanism of Fe-KMnO activation of PMS was further analyzed in detail.It was found that Fe participated in the redox of high-valent Mn,which promoted the activation of PMS.Moreover,The Fe site as an adsorption site enhanced the TC enrichment ability of the catalyst,reducing the mass transfer resistance and further enhancing the TC removal ability of Fe-KMnO/PMS system.This work not only provides an excellent PMS catalyst,but also offers new insights into the mechanism of PMS activation by bimetallic manganese-based catalysts.展开更多
BACKGROUND During the coronavirus disease 2019(COVID-19)epidemic,the fever clinic is an important link for screening and diagnosing whether a patient is infected with the novel coronavirus.Blood collection from child...BACKGROUND During the coronavirus disease 2019(COVID-19)epidemic,the fever clinic is an important link for screening and diagnosing whether a patient is infected with the novel coronavirus.Blood collection from children’s fingertips is a commonly used detection method;however,in children,the blood collection process may cause discomfort and resistance.To address this problem,the use of heating gloves combined with hand swinging can be considered for fingertip blood collection in children.AIM To explore the application of fever gloves with the handshaking method for fingertip blood collection from children in fever clinics during the COVID-19 epidemic.METHODS A total of 100 children were selected for fingertip blood collection at the fever clinic of our hospital from June 2022 to June 2023 and were divided into two groups using a randomized numerical table method,with 50 cases in each group,including the control and observation groups.The patients in the control group followed the doctor's instructions to cooperate with the routine fingertip blood collection method,and the patients in the observation group followed the doctor's instructions to cooperate with the static fever gloves with the shaking hands method of children's fingertip blood collection.The level of the six blood routine and collection indexes,and the satisfaction of the examination of the patients in the peripheral blood group and the fever gloves with the shaking hands method of the children's fingertip blood collection group were compared.RESULTS The red and white blood cell count,hemoglobin,and red blood cell pressure volume in the observation group were higher than those in the control group(P<0.05);the platelet count in the control group was lower than that in the observation group(P<0.05);the number of times of squeezing the fingertip,the average time of blood collection,and the score of puncture pain in the observation group were significantly better than those in the control group(P<0.05);and satisfaction with the routine blood examination in the observation group was greater than that in the control group.CONCLUSION The application value of the fever gloves with shaking hands method for children's fingertip blood collection was better,the accuracy of examination indexes was higher,and patient satisfaction with the examination was greater.展开更多
Objective:This paper aims to analyze the analgesic effect of ultrasound-guided transversus abdominis plane block(TAPB)combined with non-steroidal anti-inflammatory analgesic drug lornoxicam on abdominal surgery in pat...Objective:This paper aims to analyze the analgesic effect of ultrasound-guided transversus abdominis plane block(TAPB)combined with non-steroidal anti-inflammatory analgesic drug lornoxicam on abdominal surgery in patients with a history of drug addiction.Methods:32 patients aged 18-60 who underwent lower abdominal surgery in the First People's Hospital of Liangshan Yi Autonomous Prefecture and Butuo County People's Hospital of Liangshan Yi Autonomous Prefecture from January 2022 to March 2023 were selected,the patients must have drug abuse history for more than 1 year,with a history of drug withdrawal and relapse.The patients were divided into observation group and control group by the envelope method,with 16 cases in the observation group and 16 cases in the control group.Two groups of patients underwent ultrasound-guided bilateral transversus abdominis plane block after the operation.The observation group was treated with dexmedetomidine hydrochloride 1μg/kg+0.25%ropivacaine hydrochloride 40ml,and the control group was treated with 40ml 0.9%sodium chloride injection,the two groups of patients returned to the ward after operation and given intravenous infusion of lornoxicam for relieving the pain.The visual analogue scale(VAS)score of postoperative pain,the times of rescue analgesia,the time of postoperative anal exhaust,the time of ambulation,nausea and vomiting,withdrawal symptoms,related adverse reactions,and hospitalization days were compared between the two groups.Results:The VAS score of postoperative pain in the observation group was significantly lower than that in the control group,P<0.05.Patients in the observation group used less postoperative rescue analgesics than those in the control group,P<0.05.For postoperative anal exhaust time,the difference between the two groups of patients was relatively small,and the time in the observation group was shorter,P>0.05.The time to get out of bed and the length of hospital stay were not significantly different between the control group and the observation group,P>0.05.The withdrawal symptoms of the patients in the observation group were better,P<0.05,nausea and vomiting,and other adverse reactions were lower in the control group,P<0.05.Conclusion:Ultrasound-guided transversus abdominis plane block combined with lornoxicam can be used to relieve pain in abdominal surgery for patients with drug addiction,which can effectively improve the therapeutic effect of patients and reduce the number of postoperative rescue analgesia.Thus,it has high clinical application value.展开更多

The number of survival retinal ganglion cells (RGCs) was calculated by fluorescent gold retrograde labeling. Retina cells apoptosis was detected by TUNEL staining. The function of optic nerve and retina was evaluated by the visual evoked potential (VEP) and oscillatory potentials (Ops). RESULTS: The optic nerve axons were swollen and sparsely aligned, and the lamellar separation and myelin disintegration occurred after 12wk in TS rats. The density of optic nerve axons was 32.23±3.92 ( ys 37.43±4.13, P= 0.0145), the RGCs density was 1645 ±46 cells/mm^2 (vs 1867±54 cells/mm^2 F=0.0000), the incidence rate of retinal cells apoptosis was 5.38%±0.53% ( vs 4.75%±0.54%, P= 0.0238), the amplitude of VEP-P100 was 15.43±2.14 μV (vs 17.67±2.17 μV, P=0.0424), the latency of VEP-P100 was 69.05v5.34ms (vs 62.43±4.87ms P=0.0143) and the sum amplitude of Ops was 81.05±8.34 μV (Ys 91.67± 10.21 μV, P=0.0280) in TS group and the control group, respectively. CONCLUSION: Long-term weightlessness can induce the ultrastructural changes and functional depress of the optic nerve, as well as retinal cell damages in TS rats.展开更多

METHODS Approximately 25 specimens at each developmental stage of gastric carcinogenesis [non-atrophic gastritis, chronic atrophic gastritis, intestinal metaplasia, dysplasia and gastric carcinoma (GC)] with H. pylori infection [H. pylori (+)] and 25 normal gastric mucosa (NGM) without H. pylori infection [H. pylori (-)] were collected. After transcriptional factor array analysis, the Cx32 and PBX1 expression levels of H. pylori-infected tissues from the developmental stages of GC and NGM with no H. pylori infection were measured by real-time polymerase chain reaction (RT-PCR) and Western blot analysis. Regarding H. pylori-infected animal models, the Cx32 and PBX1 mRNA expression levels and correlation between the gastric mucosa from 10 Mongolian gerbils with long-term H. pylori colonization and 10 controls were analyzed. PBX1 and Cx32 mRNA and protein levels were further studied under the H. pylori-infected condition as well as PBX1 overexpression and knockdown conditions in vitro. RESULTS Incremental PBX1 was first detected by TF microarray in H. pylori-related gastric carcinogenesis. The identical trend of PBX1 and Cx32 expression was confirmed in the developmental stages of H. pylori-related clinical specimens. The negative correlation of PBX1 and Cx32 was confirmed in H. pylori-infected Mongolian gerbils. Furthermore, decreased PBX1 expression was detected in the normal gastric epithelial cell line GES-1 with H. pylori infection. Enforced overexpression or RNAi-mediated knockdown of PBX1 contributed to the diminished or restored Cx32 expression in GES-1 and the gastric carcinoma cell line BGC823, respectively. Finally, dual-luciferase reporter assay in HEK293T cells showed that Cx32 promoter activity decreased by 30% after PBX1 vector co-transfection, indicating PBX1 as a transcriptional downregulator of Cx32 by directly binding to its promoters. ONCLUSION PBX1 is one of the determinants in the Cx32 promoter targeting site, preventing further damage of gap junction protein in H. pylori-associated gastric carcinogenesis.展开更多
AIM:To develop a useful diabetic retinopathy(DR)screening tool for patients with type 2 diabetes mellitus(T2DM).METHODS:A DR prediction model based on the Logistic regression algorithm was established on the developme...AIM:To develop a useful diabetic retinopathy(DR)screening tool for patients with type 2 diabetes mellitus(T2DM).METHODS:A DR prediction model based on the Logistic regression algorithm was established on the development dataset containing 778 samples(randomly assigned to the training dataset and the internal validation dataset at a ratio of 7:3).The generalization capability of the model was assessed using an external validation dataset containing 128 samples.The DR risk calculator was developed through We Chat Developer Tools using Java Script,which was embedded in the We Chat Mini Program.RESULTS:The model revealed risk factors(duration of diabetes,diabetic nephropathy,and creatinine level)and protective factors(annual DR screening and hyperlipidemia)for DR.In the internal and external validation,the recall ratios of the model were 0.92 and 0.89,respectively,and the area under the curve values were 0.82 and 0.70,respectively.CONCLUSION:The DR screening tool integrates education,risk prediction,and medical advice function,which could help clinicians in conducting DR risk assessments and providing recommendations for ophthalmic referral to increase the DR screening rate among patients with T2DM.展开更多

It was also used as a substitute for porcine follicular fluid(PFF) in previous studies. However, FBS components are unclear, and the presence of FBS in culture media may introduce a variation from batch to batch. This study aimed to establish an effective method to screen FBS in place of PFF in the culture media for porcine oocytes in vitro. We screened FBS from different sources by using porcine fetal fibroblast cells. The effects of six FBS samples on porcine fetal fibroblast cell growth were tested via frozen cell survival assay, cell clone formation assay, cell growth curve, and cell passage activity assay. The best serum that we called GFBS(heat-inactivated FBS, cat. no. 10500-64;Gibco) showed a similar effect on the maturation and development of porcine oocytes to that of PFF and can be used as a good substitute for PFF. These results suggested that the porcine fetal fibroblast cell culture test can be used as a valuable method to screen FBS for porcine oocyte maturation and embryonic development in vitro.展开更多

Objective:To investigate the effect of Dingjifumai Decoction(DJFM)on Electrocardiogram(ECG)and sodium potassium pump in rats with ventricular arrhythmia.Methods:Forty healthy male SD rats(200±20g)were randomly di...Objective:To investigate the effect of Dingjifumai Decoction(DJFM)on Electrocardiogram(ECG)and sodium potassium pump in rats with ventricular arrhythmia.Methods:Forty healthy male SD rats(200±20g)were randomly divided into blank group,model group,Metoprolol group and DJFM group.Ten rats in each group were fed with normal diet and free drinking water.Each group was given gavage,and the amount of gavage in each group was calculated according to body weight.In the model group,0.001%Aconitine was injected into the tail vein at 30ug/kg.In the Metoprolol group,Metoprolol suspension was given according to the standard of 5.2mg/kg per day.In the DJFM group,DJFM was given at 17.6g/kg per day.After 2 weeks of administration,the biologic experiment system BL-420F was used to monitor the II lead ECG curve,and the ECG changes were observed and recorded.Then,the left ventricle of the rat was taken,and part of the heart tissue sodium potassium pump was detected.Results:(1)The effect of DJFM on ECG of rats with ventricular arrhythmia:After intravenous injection of aconitine,the incidence of Ventricular Premature beat(VP),Ventricular Tachycardia(VT),Ventricular Fibrillation(VF)in the model group was 100%,suggesting that the model building of rats with ventricular arrhythmia was successful.(2)VP,VT,and VF time:Compared with model group,DJFM group and Metoprolol group can significantly delay the VP,VT and VF,the difference was statistically significant(P<0.05).The effect of DJFM group and Metoprolol group on delaying the appearance of VP,VT and VF was the same,there was no significant difference(P>0.05).(3)The effect of DJFM on sodium potassium pump in rat ventricular arrhythmia heart tissues:Compared with the blank group,the sodium potassium pump value in the model group was significantly decreased,and the difference was statistically significant(P<0.05).Compared with the model group,the sodium potassium pump value of the tissues in the Metoprolol group and the DJFM group increased,and the difference was statistically significant(P<0.05).There was no significant difference in sodium potassium pump between the Metoprolol group and the DJFM group(P>0.05).Conclusion:1.The rat model of ventricular arrhythmia can be successfully prepared by intravenous injection of Aconitine.2.DJFM can prolong the occurrence time of cardiac arrhythmias caused by aconitine in rats,such as VP,VT,VF,et al.The mechanism may be related to fast Na^+channel,and it may prevent and control arrhythmias by inhibiting Na^+influx and reducing the fast response cellular self-discipline.3.DJFM can protect the myocardial tissue sodium potassium pump,which can protect the myocardial cells and improve the myocardial metabolism.展开更多

Disease progression prediction and therapeutic drug target discovery for Coronavirus disease 2019(COVID-19)are particularly important,as there is still no effective strategy for severe COVID-19 patient treatment.Herei...Disease progression prediction and therapeutic drug target discovery for Coronavirus disease 2019(COVID-19)are particularly important,as there is still no effective strategy for severe COVID-19 patient treatment.Herein,we performed multi-platform omics analysis of serial plasma and urine samples collected from patients during the course of COVID-19.Integrative analyses of these omics data revealed several potential therapeutic targets,such as ANXA1 and CLEC3B.Molecular changes in plasma indicated dysregulation of macrophage and suppression of T cell functions in severe patients compared to those in non-severe patients.Further,we chose 25 important molecular signatures as potential biomarkers for the prediction of disease severity.The prediction power was validated using corresponding urine samples and plasma samples from new COVID-19 patient cohort,with AUC reached to 0.904 and 0.988,respectively.In conclusion,our omics data proposed not only potential therapeutic targets,but also biomarkers for understanding the pathogenesis of severe COVID-19.展开更多

It's urgently necessary to develop a guideline of acupuncture for Bell's palsy based on principles of evidence-based medicine to optimize acupuncture treating,standardize outcomes evaluating and to improve the quality of acupuncture for patients with Bell's palsy under general circumstances.Objective: To improve the accuracy of diagnosing and managing Bell's palsy, optimize acupuncture treating and outcomes evaluating for patients with Bell's palsy, and to improve the quality of acupuncture for patients with Bell's palsy in most instances.Methods: This guideline was developed using an explicit and transparent a priori protocol based on supporting evidences and experts' consensus. The guideline developing Group followed the protocol through all stages of the development process: proposed clinical questions,searched clinical evidences, evaluated levels of evidences, developed recommendations, peer reviewed and consummated, and finally formed the draft of this guideline.Results:(1)The guideline development group made a Grade A recommendation that ①With a course of Bell's palsy within 3 months, the patients with mild facial palsy may be treated with any one of acupuncture, western drugs, or acupuncture combing with western drugs,whereas the patients with severe facial palsy may be treated with acupuncture or acupuncture combing with western drugs. With a course of more than 3 months, acupuncture is more suitable.②Acupuncture should be applied as early as possible for Bell's palsy.③The principle of selecting acupoints for Bell's palsy is to select local points, points of corresponding meridians and those according to differentiation. Generally,the points of yangming meridians are the main ones. ④The various methods of acupuncture and moxibustion are adopted for Bell's palsy,including filiform needling, moxibustion, electro-acupuncture, etc. Two or more methods are usually used together in clinical practice.(2) The development group formed expert consensus on the principles of acupuncture treatment for Bell' palsy. The rainfall processes during Soudelor's landfall and translation over East China could be separated into four stages based on rainfall characteristics such as distribution, intensity, and corresponding dynamics. The relatively less precipitation in the first stage resulted from interaction between the easterly wind to the north flank of this tropical cyclone(TC) and the coastal topography along the southeast of Zhejiang Province, China. With landfall of the TC in East China during the second stage, precipitation maxima occurred because of interaction between the TC's principal rainbands and the local topography from northeastern Fujian Province to southwestern Zhejiang Province. The distribution of precipitation presented significant asymmetric features in the third stage with maximal rainfall bands in the northeast quadrant of the TC when Soudelor's track turned from westward to northward as the TC decayed rapidly. Finally, during the northward to northeastward translation of the TC in the fourth stage, the interaction between a mid-latitude weather system and the northern part of the TC resulted in transfer of the maximum rainfall from the north of Zhejiang Province to the north of Jiangsu Province,which represented the end of rainfall in Zhejiang Province. Further quantitative calculations of the rainfall rate induced by the interaction between local topography and TC circulation(defined as "orographic effects") in the context of a one-dimensional simplified model showed that orographic effects were the primary factor determining the intensity of precipitation in this case,and accounted for over 50% of the total precipitation. The asymmetric distribution of the TC's rainbands was closely related to the asymmetric distribution of moisture resulted from changes of the TC's structure, and led to asymmetric distribution of local intense precipitation induced by Soudelor. Based on analysis of this TC, it could be concluded that local severe rainfall in the coastal regions of East China is closely related to changes of TC structure and intensity, as well as the outer rainbands. In addition, precipitation intensity and duration will increase correspondingly because of the complex interactions between the TC and local topography, and the particular TC track along large-scale steering flow. The results of this study may be useful for the understanding, prediction, and warning of disasters induced by local extreme rainfall caused by TCs, especially for facilitating forecasting and warning of flooding and mudslides associated with torrential rain caused by interactions between landfalling TCs and coastal topography.展开更多

文摘BACKGROUND Portal hypertension(PHT),primarily induced by cirrhosis,manifests severe symptoms impacting patient survival.Although transjugular intrahepatic portosystemic shunt(TIPS)is a critical intervention for managing PHT,it carries risks like hepatic encephalopathy,thus affecting patient survival prognosis.To our knowledge,existing prognostic models for post-TIPS survival in patients with PHT fail to account for the interplay among and collective impact of various prognostic factors on outcomes.Consequently,the development of an innovative modeling approach is essential to address this limitation.AIM To develop and validate a Bayesian network(BN)-based survival prediction model for patients with cirrhosis-induced PHT having undergone TIPS.METHODS The clinical data of 393 patients with cirrhosis-induced PHT who underwent TIPS surgery at the Second Affiliated Hospital of Chongqing Medical University between January 2015 and May 2022 were retrospectively analyzed.Variables were selected using Cox and least absolute shrinkage and selection operator regression methods,and a BN-based model was established and evaluated to predict survival in patients having undergone TIPS surgery for PHT.RESULTS Variable selection revealed the following as key factors impacting survival:age,ascites,hypertension,indications for TIPS,postoperative portal vein pressure(post-PVP),aspartate aminotransferase,alkaline phosphatase,total bilirubin,prealbumin,the Child-Pugh grade,and the model for end-stage liver disease(MELD)score.Based on the above-mentioned variables,a BN-based 2-year survival prognostic prediction model was constructed,which identified the following factors to be directly linked to the survival time:age,ascites,indications for TIPS,concurrent hypertension,post-PVP,the Child-Pugh grade,and the MELD score.The Bayesian information criterion was 3589.04,and 10-fold cross-validation indicated an average log-likelihood loss of 5.55 with a standard deviation of 0.16.The model’s accuracy,precision,recall,and F1 score were 0.90,0.92,0.97,and 0.95 respectively,with the area under the receiver operating characteristic curve being 0.72.CONCLUSION This study successfully developed a BN-based survival prediction model with good predictive capabilities.It offers valuable insights for treatment strategies and prognostic evaluations in patients having undergone TIPS surgery for PHT.

文摘Stipa breviflora seeds,collected from Inner Mongolia,were sowed on MS medium containing 0,50,100,150,200 mmol/L NaC l,to investigate the effects of salt stress on the physiological phenotype and antioxidant enzyme activity of S.breviflora seedlings.The results showed the germination rate,root length and fresh weight of seedlings were all influenced by salinity.Germination rate,root length and fresh weight of seedlings were significantly inhibited by 100,150 and50 mmol/L NaC l,and were 62%,1.30 cm,0.299 g,respectively.With NaC l content increasing,the trend of superoxide dismutase( SOD) and peroxidase( POD) activity was rising at first and then falling,and the activity of SOD and POD reached the maximum at 150 and 100 mmol/L,48.6 and 6 766 U/g,respectively.The activity of catalase( CAT) continued to increase and reached the maximum at 150 mmol/L,299 U/g.The isozyme zymogram showed that SOD 2-4,POD 1-2,and CAT 1-2 were positively correlated with salinity tolerance of S.breviflora seedlings.

文摘Among the numerous transition metal catalysts,manganese-based compounds are considered as promising peroxymonosulfate(PMS)catalysts due to their low cost and environmental friendliness,such as cryptomelane manganese oxide(K_(2-x)Mn_(8)O_(16):abbreviation KMnO).However,the limited catalytic performance of KMnO limits its practical application.In this work,iron-doped KMnO(Fe-KMnO)was prepared by one-step hydrothermal method to optimize its catalytic performance.Compared with KMnO/PMS system,Fe-KMnO/PMS system possessed more excellent removal efficiency of tetracycline(TC).Meanwhile,the Fe-KMnO/PMS system also exhibited good practical application potential and excellent stability.The mechanism of Fe-KMnO activation of PMS was further analyzed in detail.It was found that Fe participated in the redox of high-valent Mn,which promoted the activation of PMS.Moreover,The Fe site as an adsorption site enhanced the TC enrichment ability of the catalyst,reducing the mass transfer resistance and further enhancing the TC removal ability of Fe-KMnO/PMS system.This work not only provides an excellent PMS catalyst,but also offers new insights into the mechanism of PMS activation by bimetallic manganese-based catalysts.

文摘BACKGROUND During the coronavirus disease 2019(COVID-19)epidemic,the fever clinic is an important link for screening and diagnosing whether a patient is infected with the novel coronavirus.Blood collection from children’s fingertips is a commonly used detection method;however,in children,the blood collection process may cause discomfort and resistance.To address this problem,the use of heating gloves combined with hand swinging can be considered for fingertip blood collection in children.AIM To explore the application of fever gloves with the handshaking method for fingertip blood collection from children in fever clinics during the COVID-19 epidemic.METHODS A total of 100 children were selected for fingertip blood collection at the fever clinic of our hospital from June 2022 to June 2023 and were divided into two groups using a randomized numerical table method,with 50 cases in each group,including the control and observation groups.The patients in the control group followed the doctor's instructions to cooperate with the routine fingertip blood collection method,and the patients in the observation group followed the doctor's instructions to cooperate with the static fever gloves with the shaking hands method of children's fingertip blood collection.The level of the six blood routine and collection indexes,and the satisfaction of the examination of the patients in the peripheral blood group and the fever gloves with the shaking hands method of the children's fingertip blood collection group were compared.RESULTS The red and white blood cell count,hemoglobin,and red blood cell pressure volume in the observation group were higher than those in the control group(P<0.05);the platelet count in the control group was lower than that in the observation group(P<0.05);the number of times of squeezing the fingertip,the average time of blood collection,and the score of puncture pain in the observation group were significantly better than those in the control group(P<0.05);and satisfaction with the routine blood examination in the observation group was greater than that in the control group.CONCLUSION The application value of the fever gloves with shaking hands method for children's fingertip blood collection was better,the accuracy of examination indexes was higher,and patient satisfaction with the examination was greater.

文摘Objective:This paper aims to analyze the analgesic effect of ultrasound-guided transversus abdominis plane block(TAPB)combined with non-steroidal anti-inflammatory analgesic drug lornoxicam on abdominal surgery in patients with a history of drug addiction.Methods:32 patients aged 18-60 who underwent lower abdominal surgery in the First People's Hospital of Liangshan Yi Autonomous Prefecture and Butuo County People's Hospital of Liangshan Yi Autonomous Prefecture from January 2022 to March 2023 were selected,the patients must have drug abuse history for more than 1 year,with a history of drug withdrawal and relapse.The patients were divided into observation group and control group by the envelope method,with 16 cases in the observation group and 16 cases in the control group.Two groups of patients underwent ultrasound-guided bilateral transversus abdominis plane block after the operation.The observation group was treated with dexmedetomidine hydrochloride 1μg/kg+0.25%ropivacaine hydrochloride 40ml,and the control group was treated with 40ml 0.9%sodium chloride injection,the two groups of patients returned to the ward after operation and given intravenous infusion of lornoxicam for relieving the pain.The visual analogue scale(VAS)score of postoperative pain,the times of rescue analgesia,the time of postoperative anal exhaust,the time of ambulation,nausea and vomiting,withdrawal symptoms,related adverse reactions,and hospitalization days were compared between the two groups.Results:The VAS score of postoperative pain in the observation group was significantly lower than that in the control group,P<0.05.Patients in the observation group used less postoperative rescue analgesics than those in the control group,P<0.05.For postoperative anal exhaust time,the difference between the two groups of patients was relatively small,and the time in the observation group was shorter,P>0.05.The time to get out of bed and the length of hospital stay were not significantly different between the control group and the observation group,P>0.05.The withdrawal symptoms of the patients in the observation group were better,P<0.05,nausea and vomiting,and other adverse reactions were lower in the control group,P<0.05.Conclusion:Ultrasound-guided transversus abdominis plane block combined with lornoxicam can be used to relieve pain in abdominal surgery for patients with drug addiction,which can effectively improve the therapeutic effect of patients and reduce the number of postoperative rescue analgesia.Thus,it has high clinical application value.

文摘AIM: To evaluate the effect of long weightlessness on retina and optic nerve in suspension (TS) rats. -term tail METHODS: A stimulated weightlessness model was established by suspending rats tail. After 12wk, the ultrastructure and the number of optic nerve axons were observed by transmission electron microscope. The number of survival retinal ganglion cells (RGCs) was calculated by fluorescent gold retrograde labeling. Retina cells apoptosis was detected by TUNEL staining. The function of optic nerve and retina was evaluated by the visual evoked potential (VEP) and oscillatory potentials (Ops). RESULTS: The optic nerve axons were swollen and sparsely aligned, and the lamellar separation and myelin disintegration occurred after 12wk in TS rats. The density of optic nerve axons was 32.23±3.92 ( ys 37.43±4.13, P= 0.0145), the RGCs density was 1645 ±46 cells/mm^2 (vs 1867±54 cells/mm^2 F=0.0000), the incidence rate of retinal cells apoptosis was 5.38%±0.53% ( vs 4.75%±0.54%, P= 0.0238), the amplitude of VEP-P100 was 15.43±2.14 μV (vs 17.67±2.17 μV, P=0.0424), the latency of VEP-P100 was 69.05v5.34ms (vs 62.43±4.87ms P=0.0143) and the sum amplitude of Ops was 81.05±8.34 μV (Ys 91.67± 10.21 μV, P=0.0280) in TS group and the control group, respectively. CONCLUSION: Long-term weightlessness can induce the ultrastructural changes and functional depress of the optic nerve, as well as retinal cell damages in TS rats.

文摘AIM To clarify the mechanisms of connexin 32 (Cx32) downregulation by potential transcriptional factors (TFs) in Helicobacter pylori (H. pylori)-associated gastric carcinogenesis. METHODS Approximately 25 specimens at each developmental stage of gastric carcinogenesis [non-atrophic gastritis, chronic atrophic gastritis, intestinal metaplasia, dysplasia and gastric carcinoma (GC)] with H. pylori infection [H. pylori (+)] and 25 normal gastric mucosa (NGM) without H. pylori infection [H. pylori (-)] were collected. After transcriptional factor array analysis, the Cx32 and PBX1 expression levels of H. pylori-infected tissues from the developmental stages of GC and NGM with no H. pylori infection were measured by real-time polymerase chain reaction (RT-PCR) and Western blot analysis. Regarding H. pylori-infected animal models, the Cx32 and PBX1 mRNA expression levels and correlation between the gastric mucosa from 10 Mongolian gerbils with long-term H. pylori colonization and 10 controls were analyzed. PBX1 and Cx32 mRNA and protein levels were further studied under the H. pylori-infected condition as well as PBX1 overexpression and knockdown conditions in vitro. RESULTS Incremental PBX1 was first detected by TF microarray in H. pylori-related gastric carcinogenesis. The identical trend of PBX1 and Cx32 expression was confirmed in the developmental stages of H. pylori-related clinical specimens. The negative correlation of PBX1 and Cx32 was confirmed in H. pylori-infected Mongolian gerbils. Furthermore, decreased PBX1 expression was detected in the normal gastric epithelial cell line GES-1 with H. pylori infection. Enforced overexpression or RNAi-mediated knockdown of PBX1 contributed to the diminished or restored Cx32 expression in GES-1 and the gastric carcinoma cell line BGC823, respectively. Finally, dual-luciferase reporter assay in HEK293T cells showed that Cx32 promoter activity decreased by 30% after PBX1 vector co-transfection, indicating PBX1 as a transcriptional downregulator of Cx32 by directly binding to its promoters. ONCLUSION PBX1 is one of the determinants in the Cx32 promoter targeting site, preventing further damage of gap junction protein in H. pylori-associated gastric carcinogenesis.
文摘AIM:To develop a useful diabetic retinopathy(DR)screening tool for patients with type 2 diabetes mellitus(T2DM).METHODS:A DR prediction model based on the Logistic regression algorithm was established on the development dataset containing 778 samples(randomly assigned to the training dataset and the internal validation dataset at a ratio of 7:3).The generalization capability of the model was assessed using an external validation dataset containing 128 samples.The DR risk calculator was developed through We Chat Developer Tools using Java Script,which was embedded in the We Chat Mini Program.RESULTS:The model revealed risk factors(duration of diabetes,diabetic nephropathy,and creatinine level)and protective factors(annual DR screening and hyperlipidemia)for DR.In the internal and external validation,the recall ratios of the model were 0.92 and 0.89,respectively,and the area under the curve values were 0.82 and 0.70,respectively.CONCLUSION:The DR screening tool integrates education,risk prediction,and medical advice function,which could help clinicians in conducting DR risk assessments and providing recommendations for ophthalmic referral to increase the DR screening rate among patients with T2DM.

文摘Fetal bovine serum(FBS) is widely used in cell cultures due to its high stability and easy access. It was also used as a substitute for porcine follicular fluid(PFF) in previous studies. However, FBS components are unclear, and the presence of FBS in culture media may introduce a variation from batch to batch. This study aimed to establish an effective method to screen FBS in place of PFF in the culture media for porcine oocytes in vitro. We screened FBS from different sources by using porcine fetal fibroblast cells. The effects of six FBS samples on porcine fetal fibroblast cell growth were tested via frozen cell survival assay, cell clone formation assay, cell growth curve, and cell passage activity assay. The best serum that we called GFBS(heat-inactivated FBS, cat. no. 10500-64;Gibco) showed a similar effect on the maturation and development of porcine oocytes to that of PFF and can be used as a good substitute for PFF. These results suggested that the porcine fetal fibroblast cell culture test can be used as a valuable method to screen FBS for porcine oocyte maturation and embryonic development in vitro.
文摘Objective:To study the correlation of PCT, SAA and CD64 levels in peripheral blood with the inflammatory response and viscera function in neonate with sepsis.Methods: A total of 35 neonates diagnosed with sepsis in our hospital between March 2014 and March 2016 were selected as the sepsis group of the study, 60 healthy neonates born in our hospital during the same period were selected as the control group, peripheral blood samples were collected to determine the levels of PCT, SAA, CD64 as well as inflammatory factors, myocardial injury marker molecules and liver injury marker molecules.Results:PCT, SAA, CD64, sTREM-1, TNF-α, IL-6, IL-8, cTnI, cTnT, H-FABP, CK-MB, AST, ALT, GGT, TBIL and DBIL levels in peripheral blood of sepsis group were significantly higher than those of control group;PCT, SAA and CD64 levels in peripheral blood were positively correlated with sTREM-1, TNF-α, IL-6, IL-8, cTnI, cTnT, H-FABP, CK-MB, AST, ALT, GGT, TBIL and DBIL levels. Conclusion: PCT, SAA and CD64 levels in peripheral blood of neonatal sepsis significantly increase and are associated with the degree of inflammation, myocardial injury and liver injury.
文摘Objective:To investigate the effect of Dingjifumai Decoction(DJFM)on Electrocardiogram(ECG)and sodium potassium pump in rats with ventricular arrhythmia.Methods:Forty healthy male SD rats(200±20g)were randomly divided into blank group,model group,Metoprolol group and DJFM group.Ten rats in each group were fed with normal diet and free drinking water.Each group was given gavage,and the amount of gavage in each group was calculated according to body weight.In the model group,0.001%Aconitine was injected into the tail vein at 30ug/kg.In the Metoprolol group,Metoprolol suspension was given according to the standard of 5.2mg/kg per day.In the DJFM group,DJFM was given at 17.6g/kg per day.After 2 weeks of administration,the biologic experiment system BL-420F was used to monitor the II lead ECG curve,and the ECG changes were observed and recorded.Then,the left ventricle of the rat was taken,and part of the heart tissue sodium potassium pump was detected.Results:(1)The effect of DJFM on ECG of rats with ventricular arrhythmia:After intravenous injection of aconitine,the incidence of Ventricular Premature beat(VP),Ventricular Tachycardia(VT),Ventricular Fibrillation(VF)in the model group was 100%,suggesting that the model building of rats with ventricular arrhythmia was successful.(2)VP,VT,and VF time:Compared with model group,DJFM group and Metoprolol group can significantly delay the VP,VT and VF,the difference was statistically significant(P<0.05).The effect of DJFM group and Metoprolol group on delaying the appearance of VP,VT and VF was the same,there was no significant difference(P>0.05).(3)The effect of DJFM on sodium potassium pump in rat ventricular arrhythmia heart tissues:Compared with the blank group,the sodium potassium pump value in the model group was significantly decreased,and the difference was statistically significant(P<0.05).Compared with the model group,the sodium potassium pump value of the tissues in the Metoprolol group and the DJFM group increased,and the difference was statistically significant(P<0.05).There was no significant difference in sodium potassium pump between the Metoprolol group and the DJFM group(P>0.05).Conclusion:1.The rat model of ventricular arrhythmia can be successfully prepared by intravenous injection of Aconitine.2.DJFM can prolong the occurrence time of cardiac arrhythmias caused by aconitine in rats,such as VP,VT,VF,et al.The mechanism may be related to fast Na^+channel,and it may prevent and control arrhythmias by inhibiting Na^+influx and reducing the fast response cellular self-discipline.3.DJFM can protect the myocardial tissue sodium potassium pump,which can protect the myocardial cells and improve the myocardial metabolism.

People can easily achieve success by collaborative learning. So this paper proposes a common model of adaptive mobile collaborative learning system (AMCLS). The AMCLS has six parts which are Learner Model, Collaborative Learning Model, Domain Model,Adaptive Recommend Model based on Context-Aware, Evaluation Model and Presentation Model. The AMCLS can provide the appropriate learning contents and learning paths to learners according to their characteristics, learning context-aware, devices, time and location. Besides, it also provides better collaborative learning mode to help learners to accomplish their collaborative learning more efficiently.

文摘During recovery from heat stress,plants clear away the heat-stress-induced misfolded proteins through the ubiquitin-proteasome system(UPS).In the UPS,the recognition of substrate proteins by E3 ligase can be regulated by the N-terminal acetyltransferase A(NatA)complex.Here,we determined that Arabidopsis STRESS-RELATED UBIQUITIN-ASSOCIATED-DOMAIN PROTEIN FACTOR 1(SUF1)interacts with the NatA complex core subunit NAA15 and positively regulates NAA15.The suf1 and naa15 mutants are sensitive to heat stress;the NatA substrate NSNC1 is stabilized in suf1 mutant plants during heat stress recovery.Therefore,SUF1 and its interactor NAA15 play important roles in basal thermotolerance in Arabidopsis.

文摘Disease progression prediction and therapeutic drug target discovery for Coronavirus disease 2019(COVID-19)are particularly important,as there is still no effective strategy for severe COVID-19 patient treatment.Herein,we performed multi-platform omics analysis of serial plasma and urine samples collected from patients during the course of COVID-19.Integrative analyses of these omics data revealed several potential therapeutic targets,such as ANXA1 and CLEC3B.Molecular changes in plasma indicated dysregulation of macrophage and suppression of T cell functions in severe patients compared to those in non-severe patients.Further,we chose 25 important molecular signatures as potential biomarkers for the prediction of disease severity.The prediction power was validated using corresponding urine samples and plasma samples from new COVID-19 patient cohort,with AUC reached to 0.904 and 0.988,respectively.In conclusion,our omics data proposed not only potential therapeutic targets,but also biomarkers for understanding the pathogenesis of severe COVID-19.

文摘Backgroud: Acupuncture is common used for Bell's palsy in clinic, however, recent systematic reviews all shows that there is no sufficient evidence to support the effectiveness of acupuncture for Bell's palsy because ofthe poor quality and heterogeneity. It's urgently necessary to develop a guideline of acupuncture for Bell's palsy based on principles of evidence-based medicine to optimize acupuncture treating,standardize outcomes evaluating and to improve the quality of acupuncture for patients with Bell's palsy under general circumstances.Objective: To improve the accuracy of diagnosing and managing Bell's palsy, optimize acupuncture treating and outcomes evaluating for patients with Bell's palsy, and to improve the quality of acupuncture for patients with Bell's palsy in most instances.Methods: This guideline was developed using an explicit and transparent a priori protocol based on supporting evidences and experts' consensus. The guideline developing Group followed the protocol through all stages of the development process: proposed clinical questions,searched clinical evidences, evaluated levels of evidences, developed recommendations, peer reviewed and consummated, and finally formed the draft of this guideline.Results:(1)The guideline development group made a Grade A recommendation that ①With a course of Bell's palsy within 3 months, the patients with mild facial palsy may be treated with any one of acupuncture, western drugs, or acupuncture combing with western drugs,whereas the patients with severe facial palsy may be treated with acupuncture or acupuncture combing with western drugs. With a course of more than 3 months, acupuncture is more suitable.②Acupuncture should be applied as early as possible for Bell's palsy.③The principle of selecting acupoints for Bell's palsy is to select local points, points of corresponding meridians and those according to differentiation. Generally,the points of yangming meridians are the main ones. ④The various methods of acupuncture and moxibustion are adopted for Bell's palsy,including filiform needling, moxibustion, electro-acupuncture, etc. Two or more methods are usually used together in clinical practice.(2) The development group formed expert consensus on the principles of acupuncture treatment for Bell' palsy. Bell's palsy is suitably treated according to the stages, differentiation and symptoms.

文摘The characteristics and dynamics associated with the distribution, intensity, and triggering factors of local severe precipitation in Zhejiang Province induced by Super Typhoon Soudelor(2015) were investigated using mesoscale surface observations, radar reflectivity, satellite nephograms, and the final(FNL) analyses of the Global Forecasting System(GFS) of the National Center for Environmental Prediction(NCEP). The rainfall processes during Soudelor's landfall and translation over East China could be separated into four stages based on rainfall characteristics such as distribution, intensity, and corresponding dynamics. The relatively less precipitation in the first stage resulted from interaction between the easterly wind to the north flank of this tropical cyclone(TC) and the coastal topography along the southeast of Zhejiang Province, China. With landfall of the TC in East China during the second stage, precipitation maxima occurred because of interaction between the TC's principal rainbands and the local topography from northeastern Fujian Province to southwestern Zhejiang Province. The distribution of precipitation presented significant asymmetric features in the third stage with maximal rainfall bands in the northeast quadrant of the TC when Soudelor's track turned from westward to northward as the TC decayed rapidly. Finally, during the northward to northeastward translation of the TC in the fourth stage, the interaction between a mid-latitude weather system and the northern part of the TC resulted in transfer of the maximum rainfall from the north of Zhejiang Province to the north of Jiangsu Province,which represented the end of rainfall in Zhejiang Province. Further quantitative calculations of the rainfall rate induced by the interaction between local topography and TC circulation(defined as "orographic effects") in the context of a one-dimensional simplified model showed that orographic effects were the primary factor determining the intensity of precipitation in this case,and accounted for over 50% of the total precipitation. The asymmetric distribution of the TC's rainbands was closely related to the asymmetric distribution of moisture resulted from changes of the TC's structure, and led to asymmetric distribution of local intense precipitation induced by Soudelor. Based on analysis of this TC, it could be concluded that local severe rainfall in the coastal regions of East China is closely related to changes of TC structure and intensity, as well as the outer rainbands. In addition, precipitation intensity and duration will increase correspondingly because of the complex interactions between the TC and local topography, and the particular TC track along large-scale steering flow. The results of this study may be useful for the understanding, prediction, and warning of disasters induced by local extreme rainfall caused by TCs, especially for facilitating forecasting and warning of flooding and mudslides associated with torrential rain caused by interactions between landfalling TCs and coastal topography.
