肺癌是全球发病率和死亡率最高的恶性肿瘤。对于肺腺癌而言,识别特定的基因突变并给予相应靶向药物可大大提高患者的生存时间。其中,间变性淋巴瘤激酶(anaplastic lymphoma kinase,ALK)融合发生在3%-7%的非小细胞肺癌(non-small cell lu... 肺癌是全球发病率和死亡率最高的恶性肿瘤。对于肺腺癌而言,识别特定的基因突变并给予相应靶向药物可大大提高患者的生存时间。其中,间变性淋巴瘤激酶(anaplastic lymphoma kinase,ALK)融合发生在3%-7%的非小细胞肺癌(non-small cell lung cancer,NSCLC)中。在临床中,多种检测方法可应用于判断ALK融合状态,但存在检测结果假阴性可能。本文回顾性分析1例肺腺癌患者的诊治经过,经多种检测方式判断ALK融合状态,其中,免疫组化(immunohistochemistry,IHC)(Ventana D5F3)、基于RNA的下一代测序(RNA-based next-generation sequencing,RNA-based NGS)检测证实棘皮类微管关联蛋白样4(echinoderm microtubule associated protein like 4,EML4)-ALK融合阳性,而基于DNA的NGS(DNA-based NGS)检测为阴性。本文比较分析判断ALK融合的检测方法,以明确在不同临床案例中选择何种检测方式最准确、最简便,以便于指导后续治疗。 展开更多

Single zinc oxide nanowires(ZnO NWs)are promising for nanogenerators because of their excellent semiconducting and piezoelectric properties,but characterizing the latter efficiently is challenging.As reported here,an electrical breakdown strategy was used to construct single ZnO NWs with a specific length.With the high operability of a nanomanipulator in a scanning electron microscope,ZnO-NW-based twoprobe and three-probe structures were constructed for fabricating AC/DC nanogenerators,respectively.For a ZnO NW,an AC output of between−15.31 mV and 5.82 mV was achieved,while for a DC nanogenerator,an output of24.3 mV was realized.Also,the three-probe structure’s output method was changed to verify the distribution of piezoelectric charges when a single ZnO NW is bent by a probe,and DC outputs of different amplitudes were achieved.This study provides a low-cost,highly convenient,and operational method for studying the AC/DC output characteristics of single NWs,which is beneficial for the further development of nanogenerators. 展开更多

BACKGROUND Gastric submucosal arterial dilation resulting from splenic artery occlusion represents an exceedingly rare etiology of acute upper gastrointestinal bleeding(UGIB).Although endoscopy is a widely utilized diagnostic and therapeutic modality for gastrointestinal bleeding,it has limitations in detecting arterial abnormalities.CASE SUMMARY This report presents a rare case of massive UGIB in a 57-year-old male with a tortuous left inferior phrenic artery accompanied by splenic artery occlusion.“Gastric varices”was identified during the patient's endoscopy one year before hemorrhage.Despite initial hemostasis by endoscopic clipping,the patient experienced massive rebleeding after one month,requiring intervention with transcatheter arterial embolization(TAE)to achieve hemostasis.CONCLUSION This is the first case to report UGIB due to a tortuous left inferior phrenic artery.This case highlights the limitations of endoscopy in identifying arterial abnormalities and emphasizes the potential of TAE as a viable alternative for the management of arterial bleeding in the gastrointestinal tract. 展开更多

The sticking phenomenon between molten slag and refractory is one of the crucial problems when preparing ferronickel from laterite ore using rotary hearth fulnace or rotary kiln processes. This study aims to ameliorate sticking problems by using silicon dioxide (SiO2) to adjust the melting degree of file briquette during reduction roasting. Thermodynamic analysis indicates that the melting temperature of the slag gradually increases with an increase in the SiO2 proportion (SiO2/(SiO2 + A1203 + MgO) mass ratio). Experimental validations also prove that the briquette retains its original shape when the SiO2 proportion is greater than 75wt%, and sticking problems axe avoided during reduction. A ferronickel product with 8.33wt% Ni and 84.71wt% Fe was prepared via reductive roasting at 1500~C for 90 min with a SiO2 proportion of 75wt% and a C/O molar ratio of 1.0 followed by dry magnetic separation; the corresponding recoveries of Ni and Fe reached 75.70% and 77.97%, respectively. The micro stxucture and phase txmlsformation of reduced briquette reveals that the aggregation and growth of ferroinckel particles were not significantly affected after adding SiO2 to the reduction process. 展开更多

Polycyclic aromatic hydrocarbons(PAHs)are typical persistent organic pollutants(POPs)that are commonly found in the environment.They are carcinogenic,teratogenic,mutagenic and biodegradable obviously.In this paper,the modified mussel shells were used to adsorb and remove anthracene.The results show that the adsorption removal rate of the mussel shells was higher after calcination at 600℃.5%H_(3)PO_(4) solution was more suitable for shell treatment than 3 mol/L ZnCl_(2) solution.As the dosage of the modified shells was 0.5 g/L,the adsorption reached a stable state,and the removal rate of PAHs was about 69.44%;the adsorption efficiency rose with the increase of time.It can be seen that as a new and cheap biological adsorbent,the modified shells can be used to remove PAHs from wastewater. 展开更多

Objective:To study the application effect of the plan-do-check-act(PDCA)cycle management in the hand hygiene management of psychiatric medical staff.Methods:One hundred and twenty medical staff from a psychiatric hospital from May 2023 to December 2023 were selected and divided into two groups.The control group(May 2023 to August 2023)applied the conventional management model,and the observation group(September 2023 to December 2023)applied the PDCA cycle management.The hand hygiene compliance,hand hygiene knowledge,and hygiene qualifications were compared,including the amount of hand sanitizer used.Results:The proportion of medical staff’s hand hygiene compliance and hand hygiene knowledge mastery scores in the observation group were higher than those in the control group(P<0.05);the hand hygiene passing rate in the observation group was higher than that of the control group(P<0.05);the daily amount of hand sanitizer per patient bed and the amount of hand sanitizer used was higher than that of the control group(P<0.05).Conclusion:The PDCA cycle management model for psychiatric medical staff promoted the improvement of hand hygiene compliance and increased their hand hygiene qualifications.It is suitable for further popularization and application in future clinical practice. 展开更多

目的探讨微创肺表面活性物质(LISA)治疗早产儿呼吸窘迫综合征(NRDS)对发育结局的影响。方法回顾性分析2019年12月—2021年5月阜阳市人民医院收治的67例NRDS患儿的临床资料。将采用LISA治疗的30例患儿作为研究组,采用气管插管-肺表面活... 目的探讨微创肺表面活性物质(LISA)治疗早产儿呼吸窘迫综合征(NRDS)对发育结局的影响。方法回顾性分析2019年12月—2021年5月阜阳市人民医院收治的67例NRDS患儿的临床资料。将采用LISA治疗的30例患儿作为研究组,采用气管插管-肺表面活性物质-拔管治疗的37例患儿作为对照组,随访至纠正胎龄12个月。统计两组患儿肺表面活性物质(PS)给药过程中不良反应,分析两组通气、吸氧及再次使用PS情况,比较两组治疗前后血气指标的变化,对比两组并发症发生情况,比较两组随访期间体格发育及听力、视觉、神经系统发育情况。结果对照组与研究组PS给药操作过程中心动过缓、呼吸暂停、经皮血氧饱和度下降、药物反流发生率,72 h内有创机械通气率、总吸氧时间、无创通气时间、再次使用PS率,视网膜病、坏死性小肠结肠炎、脑室内出血、听力损伤、视觉损伤发生率比较,差异均无统计学意义(P>0.05)。研究组支气管肺发育不良发生率低于对照组(P<0.05)。两组治疗前后动脉血氧分压、动脉血二氧化碳分压、动脉血氧分压/吸入气氧浓度的差值比较,差异均无统计学意义(P>0.05)。两组出院时、纠正6个月、纠正12个月体重、头围、身长、宫外生长迟缓(EUGR)发生率比较,差异均无统计学意义(P>0.05)。两组纠正6个月、12个月的EUGR发生率均低于出院时(P<0.05),两组12个月的EUGR发生率均低于纠正6个月(P<0.05)。研究组智力发展指数、精神运动发展指数、发育商优于对照组(P<0.05)。结论LISA治疗NRDS早产儿是一种安全有效的PS给药方法,可降低支气管肺发育不良发生风险,改善神经系统发育情况。 展开更多

目的研究单根锉Wave One镍钛器械治疗弯曲根管的疗效,并与不锈钢K锉、Pro Taper镍钛器械进行比较。方法选取根管弯曲度在20~40°的单根管离体前磨牙60颗,随机分为A、B、C 3组,每组20颗离体牙,3组分别采用Wave One/Crown-down法、K... 目的研究单根锉Wave One镍钛器械治疗弯曲根管的疗效,并与不锈钢K锉、Pro Taper镍钛器械进行比较。方法选取根管弯曲度在20~40°的单根管离体前磨牙60颗,随机分为A、B、C 3组,每组20颗离体牙,3组分别采用Wave One/Crown-down法、K锉逐步后退法及Pro Taper/Crown-down法行根管预备,并采用口腔颌面锥形束计算机体层摄影分别于根管预备前后对离体牙行横向扫描。预备完成后,对3组中的60颗离体牙行冷牙胶侧方加压充填。测量3组中每颗离体牙的根管预备时间、根管充填时间;分析每颗离体牙根管预备前后,根管距根尖孔2、4和6 mm处的根管偏移量和轴中心率,并对以上结果进行统计学分析。结果 A组根管预备时间低于B、C组;在根管充填时间上,A组与C组比较差异无统计学意义(P>0.05),但A组短于B组(P<0.05)。在距根尖孔2、4和6 mm处,A组与B组间的根管偏移量及轴中心率比较,差异有统计学意义(P<0.05);A组与C组比较,差异无统计学意义(P>0.05)。结论与不锈钢K锉及Pro Taper相比,Wave One治疗弯曲根管具有效率高、根管成形效果好的优点,可广泛应用于临床弯曲根管的治疗。 展开更多

The efficacy and applied value of endoscopic hematoma evacuation vs. external ventricular drainage (EVD) in the treatment of severe ventficular hemorrhage (IVH) were explored and compared.From Jan.2015 to Dec.2016,the clinical data of 42 cases of IVH were retrospectively analyzed,including 18 patients undergoing endoscopic hematoma evacuation (group A),and 24patients receiving EVD (group B).The hematoma clearance rate was calculated by 3D Slicer software,and complications and outcomes were compared between the two groups.There were no significant differences in age,sex and Graeb score between groups A and B (P>0.05).The hematoma clearance rate was 70.81%±27.64% in group A and 48.72%±36.58% in group B with a statistically significant difference (P<0.05).The operative time in groups A and B was 72.45±25.26 min and 28.54±15.27min,respectively (P<0.05).The Glasgow Coma Scale (GCS) score increased from 9.28±2.72 at baseline to 11.83±2.91 at 1 week postoperatively in group A,and from 8.25±2.62 at baseline to 10.79±4.12 at 1 week postoperatively in group B (P<0.05).The length of hospital stay was 12.67±5.97 days in group A and 17.33±8.91 days in group B with a statistically significant difference (P<0.05).The GOS scores at 6 months after surgery were 3.83±1.12 in group A,and 2.75±1.23 in group B (P<0.05). These results suggested that endoscopic hematoma evacuation has an advantage of a higher hematoma clearance rate,fewer complications and better outcomes in the treatment of severe IVH,indicating it is a safe,effective and promising approach for severe IVH. 展开更多

Among the scaffolds for tissue-engineering applications, injectable hydrogels have demonstrated great potential for use as three-dimensional cell culture scaffolds in cartilage and bone tissue engineering, owing to their high water content, similarity to the natural extracellular matrix(ECM), porous framework for cell transplantation and proliferation, minimal invasive properties, and ability to match irregular defects. In this review, we describe the selection of appropriate biomaterials and fabrication methods to prepare novel injectable hydrogels for cartilage and bone tissue engineering. In addition, the biology of cartilage and the bony ECM is also summarized. Finally, future perspectives for injectable hydrogels in cartilage and bone tissue engineering are discussed. 展开更多

AIM: To determine the prognostic value of circulating indicators of cell death in acute-on-chronic liver failure (ACLF) patients with chronic hepatitis B virus (HBV) infection as the single etiology. METHODS: Full len... AIM: To determine the prognostic value of circulating indicators of cell death in acute-on-chronic liver failure (ACLF) patients with chronic hepatitis B virus (HBV) infection as the single etiology. METHODS: Full length and caspase cleaved cytokeratin 18 (detected as M65 and M30 antigens) represent circulating indicators of necrosis and apoptosis. M65 and M30 were identified by enzyme-linked immunosorbent assay in 169 subjects including healthy controls (n = 33), patients with chronic hepatitis B (CHB, n = 55) and patients with ACLF (n = 81). According to the 3-mo survival period, ACLF patients were defined as having spontaneous recovery (n = 33) and non-spontaneous recovery which included deceased patients and those who required liver transplantation (n = 48). RESULTS: Both biomarker levels significantly increased gradually as liver disease progressed (for M65: P < 0.001 for all; for M30: control vs CHB, P = 0.072; others: P < 0.001 for all). In contrast, the M30/M65 ratio was significantly higher in controls compared with CHB patients (P = 0.010) or ACLF patients (P < 0.001). In addition, the area under receiver operating characteristic curve (AUC) analysis demonstrated that both biomarkers had diagnostic value (AUC >= 0.80) in identifying ACLF from CHB patients. Interestingly, it is worth noting that the M30/M65 ratio was significantly different between spontaneous and non-spontaneous recovery in ACLF patients (P = 0.032). The prognostic value of the M30/M65 ratio was compared with the Model for End-Stage Liver Disease (MELD) and Child-Pugh scores at the 3-mo survival period, the AUC of the M30/M65 ratio was 0.66 with a sensitivity of 52.9% and the highest specificity of 92.6% (MELD:AUC = 0.71; sensitivity, 79.4%; specificity, 63.0%; Child-Pugh: AUC = 0.77; sensitivity, 61.8%; specificity, 88.9%). CONCLUSION: M65 and M30 are strongly associated with liver disease severity. The M30/M65 ratio may be a potential prognostic marker for spontaneous recovery in patients with HBV-related ACLF. In this study, we hypothesized that an increase in integrin αβand its co-activator vascular endothelial growth factor play important neuroprotective roles in ischemic injury. We performed ischemic preconditioning with bilateral common carotid artery occlusion for 5 minutes in C57BL/6J mice. This was followed by ischemic injury with bilateral common carotid artery occlusion for 30 minutes. The time interval between ischemic preconditioning and lethal ischemia was 48 hours. Histopathological analysis showed that ischemic preconditioning substantially diminished damage to neurons in the hippocampus 7 days after ischemia. Evans Blue dye assay showed that ischemic preconditioning reduced damage to the blood-brain barrier 24 hours after ischemia. This demonstrates the neuroprotective effect of ischemic preconditioning. Western blot assay revealed a significant reduction in protein levels of integrin αβ, vascular endothelial growth factor and its receptor in mice given ischemic preconditioning compared with mice not given ischemic preconditioning 24 hours after ischemia. These findings suggest that the neuroprotective effect of ischemic preconditioning is associated with lower integrin αβand vascular endothelial growth factor levels in the brain following ischemia. 展开更多

AIM To determine the prevalence and diagnostic value of autoantibodies inα-fetoprotein(AFP)-negative hepatocellular carcinoma(HCC).METHODS Fifty-six serum samples from AFP-negative HCC cases,86 from AFP-positive HCC cases,168 from chronic liver disease cases,and 59 from normal human controls were included in this study.Autoantibodies to nucleophosmin(NPM)1,14-3-3zeta and mouse double minute 2 homolog(MDM2)proteins in AFP-negative HCC serum were evaluated by enzymelinked im munosorbent assay.Partially positive sera were further evaluated by western blotting.Immunohistochemistry was used to detect the expression of three tumor-associated antigens(TAAs)in AFP-negative HCC and normal control tissues.RESULTS The frequency of autoantibodies to the three TAAs in AFP-negative HCC sera was 21.4%,19.6%and 19.6%,which was significantly higher than in the chronic liver disease cases and normal human controls(P<0.01)as well as AFP-positive HCC cases.The sensitivity of the three autoantibodies for diagnosis of AFP-negative HCC ranged from 19.6%to 21.4%,and the specificity was approximately 95%.When the three autoantibodies were combined,the sensitivity reached 30.4%and the specificity reached 91.6%.CONCLUSION Autoantibodies to NPM1,14-3-3zeta and MDM2 may be useful biomarkers for immunodiagnosis of AFP-negative HCC. 展开更多

Cisplatin is the core of first-line chemotherapy for patients with advanced ovarian cancer. Many patients eventually become resistant to cisplatin, diminishing its therapeutic effect. MicroRNAs(miRNAs) have critical functions in diverse biological processes. Using miRNA profiling and polymerase chain reaction validation, we identified a panel of differentially expressed miRNAs and their potential targets in cisplatin-resistant SKOV3/DDP ovarian cancer cells relative to cisplatin-sensitive SKOV3 parental cells. More specifically, our results revealed significant changes in the expression of 13 of 663 miRNAs analyzed, including 11 that were up-regulated and 2 that were down-regulated in SKOV3/DDP cells with or without cisplatin treatment compared with SKOV3 cells with or without cisplatin treatment. miRNA array and mRNA array data were further analyzed using Ingenuity Pathway Analysis software. Bioinformatics analysis suggests that the genes ANKRD17, SMC1A, SUMO1, GTF2H1, and TP73, which are involved in DNA damage signaling pathways, are potential targets of miRNAs in promoting cisplatin resistance. This study highlights candidate miRNAmRNA interactions that may contribute to cisplatin resistance in ovarian cancer. 展开更多

This paper proposes a modified centralized shifted Rayleigh filter(MCSRF) algorithm for tracking boost phase of ballistic missile(BM) trajectory with a highly nonlinear dynamical model based on bearings-only.This paper contributes three folds.Firstly,the mathematical model of an MCSRF for multiple passive sensors is derived.Then,minimum entropy based onedimensional optimization search to adaptively adjust the probability of the different filters for real time state estimation is deployed.Finally,the unscented transform(UT) is introduced to resolve the asymmetric state estimation problem.Simulation results show that the proposed algorithm can consecutively track the BM precisely during the boost phase.In comparison with the unscented Kalman filter(UKF) algorithm,the proposed algorithm effectively reduces the tracking position and velocity root mean square(RMS) errors,which will make more sense for early precision interception. 展开更多

AIM:To compare fluoroscopic, endoscopic and guide wire assistance with ultraslim gastroscopy for placement of nasojejunal feeding tubes. METHODS:The information regarding nasojejunal tube placement procedures was retrieved using the gastrointestinal tract database at Tongji Hospital affiliated to Tongji Medical College. Records from 81 patients who underwent nasojejunal tubes placement by different techniques between 2004 and 2011 were reviewed for procedure success and tube-related outcomes. RESULTS:Nasojejunal feeding tubes were successfully placed in 78 (96.3%) of 81 patients. The success rate by fluoroscopy was 92% (23 of 25), by endoscopic technique 96.3% (26 of 27), and by guide wire assistance (whether via transnasal or transoral insertion)100% (23/23, 6/6). The average time for successful placement was 14.9 ± 2.9 min for fluoroscopic placement, 14.8 ± 4.9 min for endoscopic placement, 11.1 ± 2.2 min for guide wire assistance with transnasal gastroscopic placement, and 14.7 ± 1.2 min for transoral gastroscopic placement. Statistically, the duration for the third method was significantly different (P < 0.05) compared with the other three methods. Transnasal placement over a guidewire was significantly faster (P < 0.05) than any of the other approaches. CONCLUSION:Guide wire assistance with transnasal insertion of nasojejunal feeding tubes represents a safe, quick and effective method for providing enteral nutrition. 展开更多
