Diabetes,commonly known for its metabolic effects,also critically affects the enteric nervous system(ENS),which is essential in regulating gastrointestinal(GI)motility,secretion,and absorption.The development of diabetes-induced enteric neuropathy can lead to various GI dysfunctions,such as gastroparesis and irregular bowel habits,primarily due to disruptions in the function of neuronal and glial cells within the ENS,as well as oxidative stress and inflammation.This editorial explores the pathophysiological mechanisms underlying the development of enteric neuropathy in diabetic patients.Additionally,it discusses the latest advances in diagnostic approaches,emphasizing the need for early detection and intervention to mitigate GI complications in diabetic individuals.The editorial also reviews current and emerging therapeutic strategies,focusing on pharmacological treatments,dietary management,and potential neuromodulatory interventions.Ultimately,this editorial highlights the necessity of a multidisciplinary approach in managing enteric neuropathy in diabetes,aiming to enhance patient quality of life and address a frequently overlooked complication of this widespread disease. 展开更多

Diabetes mellitus(DM)and Alzheimer's disease(AD)are two major health concerns that have seen a rising prevalence worldwide.Recent studies have indicated a possible link between DM and an increased risk of developing AD.Insulin,while primarily known for its role in regulating blood sugar,also plays a vital role in protecting brain functions.Insulin resistance(IR),especially prevalent in type 2 diabetes,is believed to play a significant role in AD's development.When insulin signalling becomes dysfunctional,it can negatively affect various brain functions,making individuals more susceptible to AD's defining features,such as the buildup of beta-amyloid plaques and tau protein tangles.Emerging research suggests that addressing insulin-related issues might help reduce or even reverse the brain changes linked to AD.This review aims to explore the relationship between DM and AD,with a focus on the role of IR.It also explores the molecular mechanisms by which IR might lead to brain changes and assesses current treatments that target IR.Understanding IR's role in the connection between DM and AD offers new possibilities for treatments and highlights the importance of continued research in this interdisciplinary field. 展开更多

Diabetes mellitus,characterized by chronic hyperglycemia due to insulin deficiency or resistance,poses a significant global health burden.Central to its pathogenesis is the dysfunction or loss of pancreatic beta cells,which are res-ponsible for insulin production.Recent advances in beta-cell regeneration research offer promising strategies for diabetes treatment,aiming to restore endogenous insulin production and achieve glycemic control.This review explores the physiological basis of beta-cell function,recent scientific advan-cements,and the challenges in translating these findings into clinical applications.It highlights key developments in stem cell therapy,gene editing technologies,and the identification of novel regenerative molecules.Despite the potential,the field faces hurdles such as ensuring the safety and long-term efficacy of regen-erative therapies,ethical concerns around stem cell use,and the complexity of beta-cell differentiation and integration.The review highlights the importance of interdisciplinary collaboration,increased funding,the need for patient-centered approaches and the integration of new treatments into comprehensive care strategies to overcome these challenges.Through continued research and collab-oration,beta-cell regeneration holds the potential to revolutionize diabetes care,turning a chronic condition into a manageable or even curable disease. 展开更多

Hepatocellular carcinoma(HCC),the predominant type of liver cancer,is a major contributor to cancer-related fatalities across the globe.Diabetes has been identified as a significant risk factor for HCC,with recent research indicating that the hormone resistin could be involved in the onset and advancement of HCC in diabetic individuals.Resistin is a hormone that is known to be involved in inflammation and insulin resistance.Patients with HCC have been observed to exhibit increased resistin levels,which could be correlated with more severe disease stages and unfavourable prognoses.Nevertheless,the exact processes through which resistin influences the development and progression of HCC in diabetic patients remain unclear.This article aims to examine the existing literature on the possible use of resistin levels as a biomarker for HCC development and monitoring.Furthermore,it reviews the possible pathways of HCC initiation due to elevated resistin and offers new perspectives on comprehending the fundamen-tal mechanisms of HCC in diabetic patients.Gaining a better understanding of these processes may yield valuable insights into HCC’s development and progression,as well as identify possible avenues for prevention and therapy. 展开更多

The global burden of diabetic foot ulcers(DFUs)is a significant public health concern,affecting millions of people worldwide.These wounds cause considerable suffering and have a high economic cost.Therefore,there is a need for effective strategies to prevent and treat DFUs.One promising therapeutic approach is the use of adiponectin,a hormone primarily produced and secreted by adipose tissue.Adiponectin has demonstrated anti-inflammatory and antiatherogenic properties,and researchers have suggested its potential therapeutic applications in the treatment of DFUs.Studies have indicated that adiponectin can inhibit the production of pro-inflammatory cytokines,increase the production of vascular endothelial growth factor,a key mediator of angiogenesis,and inhibit the activation of the intrinsic apoptotic pathway.Additionally,adiponectin has been found to possess antioxidant properties and impact glucose metabolism,the immune system,extracellular matrix remodeling,and nerve function.The objective of this review is to summarize the current state of research on the potential role of adiponectin in the treatment of DFUs and to identify areas where further research is needed in order to fully understand the effects of adiponectin on DFUs and to establish its safety and efficacy as a treatment for DFUs in the clinical setting.This will provide a deeper understanding of the underlying mechanisms of DFUs that can aid in the development of new and more effective treatment strategies. 展开更多

The growing worldwide burden of insulin resistance(IR) emphasizes the importance of early identification for improved management.Obesity,particularly visceral obesity,has been a key contributing factor in the development of IR.The obesity-associated chronic inflammatory state contributes to the development of obesity-related comorbidities,including IR.Adipocytokines,which are released by adipose tissue,have been investigated as possible indicators of IR.Visfatin was one of the adipocytokines that attracted attention due to its insulinmimetic activity.It is released from a variety of sources,including visceral fat and macrophages,and it influences glucose metabolism and increases inflammation.The relationship between visfatin and IR in obesity is debatable.As a result,the purpose of this review was to better understand the role of visfatin in glucose homeostasis and to review the literature on the association between visfatin levels and IR,cardiovascular diseases,and renal diseases in obesity. 展开更多

The escalating global burden of type 2 diabetes mellitus necessitates the implementation of strategies that are both more reliable and faster in order to improve the early identification of insulin resistance(IR)in high-risk groups,including overweight and obese individuals.The use of salivary biomarkers offers a promising alternative to serum collection because it is safer,more comfortable,and less painful to obtain saliva samples.As obesity is the foremost contributory factor in IR development,the adipocytokines such as leptin,adiponectin,resistin,and visfatin secreted from the adipose tissue have been studied as potential reliable biomarkers for IR.Measurement of salivary adipokines as predictors for IR has attracted widespread attention because of the strong correlation between their blood and salivary concentrations.One of the adipokines that is closely related to IR is resistin.However,there are conflicting findings on resistin’s potential role as an etiological link between obesity and IR and the reliability of measuring salivary resistin as a biomarker for IR.Hence this study reviewed the available evidence on the potential use of salivary resistin as a biomarker for IR in order to attempt to gain a better understanding of the role of resistin in the development of IR in obese individuals. 展开更多

BACKGROUND Recently,there have been several findings that showed intestinal colonisation of Blastocystis hominis(Blastocystis)as a risk factor to the worsening of colorectal cancer(CRC).However,studies have shown controversial results in the pathogenicity of Blastocystis.AIM To review systematically the evidence available on the association between CRC and Blastocystis and the prevalence of Blastocystis in CRC patients and to investigate cytopathic and immunological effects of Blastocystis in in vitro and in vivo studies.METHODS PRISMA guidelines were utilised in conducting this systematic review.Original articles published before February 2,2020 were included.PubMed,Science Direct,Scopus and Google scholar databases were searched.Manual searching was carried out to find articles missed during the online search.RESULTS Out of 12 studies selected for this systematic review,seven studies confirmed the prevalence of Blastocystis and found it to be between 2%-28%in CRC patients,whereby subtype 1 and subtype 3 were predominantly seen.A total of four studies employing in vitro human colorectal carcinoma cell line study models showed significant cytopathic and immunological effects of Blastocystis.In addition,one in vivo experimental animal model study showed that there was a significant effect of infection with Blastocystis on exacerbation of colorectal carcinogenesis.CONCLUSION Blastocystis is a commonly identified microorganism in CRC patients.These studies have provided supportive data that Blastocystis could exacerbate existing CRC via alteration in host immune response and increased oxidative damage.Future studies of CRC and Blastocystis should attempt to determine the various stages of CRC that are most likely to be associated with Blastocystis and its relationship with other intestinal bacteria. 展开更多
