It is associated with a maternal and foetal mortality rate of around 27%. In view of the grim picture painted by this condition for both mother and newborn, studies have been carried out into the early detection of patients at risk of developing pre-eclampsia. These make it possible to introduce pregnancy-specific monitoring and preventive strategies to reduce the incidence of the condition. Objective: To establish the link between placental ultrasound characteristics and the onset of pre-eclampsia. Methodology: A multicentre prospective cohort study was conducted in two hospitals in Yaoundé, namely the Yaoundé Gynaecological Obstetrics and Paediatrics Hospital and the Nkolndongo Health and Social Animation Centre, in the gynaecology and radiology departments over a period of 11 months, from October 2022 to August 2023. It included pregnant women who had undergone obstetric ultrasound between 12 and 18 weeks’ gestation. In addition to routine obstetric ultrasound, we performed obstetric Doppler measurements in these patients. The resistance index of the left and right uterine arteries, the umbilical artery and the placental volume were the characteristics sought. At the end of this examination, two groups were formed: cases (pathological Doppler group) and controls (normal Doppler group). Blood pressure and urine dipstick were taken at each antenatal visit until delivery, then during the immediate postpartum period and finally at 7 days, 21 days and 42 days after delivery. Results: Sixty-seven (67) patients were included. Of these, 35 (47.8%) had pathological Dopplers. Sixteen patients had arterial hypertension associated with proteinuria and were therefore labelled as pre-eclampsia (PE). This gives a prevalence of 23.9%. Fifteen (42.8%) of these patients belonged to the exposed group and one to the unexposed group. The mean IR of the pre-eclampsia patients was significantly higher than that of the patients without pre-eclampsia;respectively 0.74 ± 0.096 and 0.49 ± 0.097 for the right uterine arteries and 0.71 ± 0.13 and 0.52 ± 0.089 for the left uterine arteries. We found that an increase in the uterine artery resistance index was significantly associated with the onset of pre-eclampsia, with a relative risk of 13.7 and a p value Conclusion: Abnormal Doppler ultrasound between 12 and 18 weeks of amenorrhoea had good overall sensitivity for predicting pre-eclampsia. Among the Doppler indices, the uterine artery resistance index was the only one significantly associated with pre-eclampsia.展开更多

Imaging tools such as ultrasound, can be used to differentiate between the types of breast lumps. This study aimed to determine the value of ultrasound as a first-line examination in the diagnosis of breast masses. Methods: This was a retrospective cross-sectional study of women presenting with breast lumps from November 2022 to June 2023 at the Yaounde General Hospital, which lasted seven months, from November 2022 to June 2023. The sampling was exhaustive and consecutive. Association between variables was studied using the x<sup>2</sup> test and concordance between ultrasound and histopathological findings was assessed using the Kappa correlation coefficient. Results: 234 women were included in the study. Their mean age was 46.3 ± 11.4 years. Overall, 15 (6.4%) lumps were benign while 219 (93.6%) were malignant. Triple negative (5.6%) was the most recurrent genomic classification. The correlation between the ultrasound and histopathological findings was significant, with an observed concordance rate at 85.1%, kappa = 0.322 and a p-value Conclusion: The performance of ultrasound in differentiating benign and malignant lesion was high. However, the discordant cases highlight the need for a diagnosis system which blends histopathological and radiological findings for an improved management of patients with breast lumps.展开更多

However, it is often requested by non experienced practitioners. We aimed to describe the utility and the appropriateness of CT Scan request forms and results in the diagnosis of SN pathologies. <strong>Method:</strong> We conducted a retrospective cross-sectional study in four health facilities in Yaoundé, Cameroun, from October 2018 to September 2019. All patients to whom SN CT Scan was performed were called for an appointment. After informed consent, data on the appropriateness and conformity of the request form were collected and compared to the guidelines of the French society of radiology. CT Scan results were also collected and the quality of the report was evaluated. <strong>Results:</strong> A total of 206 SN CT Scan request forms and reports were included, with a mean patient age of 37.7 ± 16.1 years. The request form was conformed to guidelines in 8.3% of cases and was found appropriate in 62.6% of cases. No CT Scan report had all the items required, and 159 (77.2%) reports were useful. The most common diagnosis in patients with abnormal results was chronic sinusitis (49.5%) and SN polyp or polyposis (19.4%). <strong>Conclusion:</strong> Most of SN CT Scan requested were appropriate and useful. However, the conformity of request forms and reports to the guidelines is low. Suggesting the need for capacity building for the diagnosis of SN pathologies especially for non specialists like general practitioners and nurses.展开更多

Several intrinsic and extrinsic factors are linked to this lesion. Anatomical factors increase the anterior translation of the knee and thus promote the stretching and rupture of the anterior cruciate ligament. Objectives: To determine the anatomical risk factors favouring ACL rupture by comparing morphometric parameters of patients with knee ACL rupture to patients without ACL rupture, as well as the intercondylar notch index and the tibial slopes of the two groups and describing the ruptured ACLs and associated signs. Materials and Methods: We conducted a case-control descriptive analytical study in imaging centres of the General Henri Mondor of Aurillac Hospital Centre in France (CHM) and the Jordan Medical Centre of Yaoundé in Cameroon (CMJ). MRI exam protocols included T1 SE, T2 SE, proton density and Fat Sat sequences, with slices in all three planes. Morphometry knee variables measured in our study were: intercondylar notch index and the tibial slopes. These measurements were obtained from images stored in DICOM format and post processing software OsiriX MD®for CMJ patients and Explore®for CHM patients. Results: The study included 92 individuals, 38 in the case group and 54 in the control group. The mean age was 36.6 years for both groups;35.5 years for the cases and 37.4 years for the controls. The sex ratio was 1.87 men for 1 woman in both groups;2.16 men for 1 woman for the case group. 53% of ACL ruptures are partial, with the predominant direct sign being morphologic and signal abnormalities of the ACL. The most significant indirect sign of ACL rupture was mirror-image bone contusion that was observed in 47.3% of cases. In the case group, the mean lateral tibial slope was 4.003°, whereas it was 2.92°in the control group. The comparison of means was estimated at approximately 0.039 (p Conclusion: The increase in the lateral tibial slope was a risk factor for anterior cruciate ligament rupture in our study population. The ...Objective: Bleeding during the first trimester of pregnancy is common and can be a sign of complication that often necessitates obstetric ultrasound for the assessment of the haemorrhage, and of fetal well being. The aim of this study was to determine the causes of first trimester bleeding on obstetrical ultrasound in our area. Method: It was a cross-sectional descriptive study during a six-month period from May to October 2017. All women who referred to the Regional center of medical imaging of Ngaoundere who agreed to participate in the study with first trimester bleeding were evaluated with clinical history and ultrasonography (US). Ultrasonographic exam was done via transabdominal or endovaginal approaches using GE LOGIQ 7 scanner brand. Microsoft Office Excel 2010 and SPHINX V 4.0 were used for data analysis. Results: A total of 121 of pregnant women who presented vaginal bleeding during the first trimester were enrolled into the study. These Ultrasound examinations represented 37.93% of all obstetric ultrasounds in the first trimester. The patients in this study ranged in age from 16 to 47 years with an average of 30 years. 50 (41.32%) were pregnant for the first time and 24 (19.83%) were primiparous. Mean gestational age was 9 weeks with the extremes of 5 and 12 weeks. 97 (80.2%) reported spontaneous bleeding while in 24 (18.8%) bleeding was mostly caused by sexual intercourse 11 (45.8%). Pelvic pain 91 (75.2%) was the most associated sign. 36 (29.8%) patients were diagnosed as threatened abortions. 27 (22.3%) were diagnosed with incomplete abortions. 4 (3.3%) cases showed an ectopic gestation. 9 (7.4%) cases showed complete abortions. Concordance between clinical diagnosis and ultrasound results was 84%. Conclusions: Bleeding in the first trimester of pregnancy is common in our area. Ultrasonography occupies a prominent place in the etiological diagnosis of bleeding in the first trimester of pregnancy;it is easy to find in most cases a responsible abnormality of bleeding.展开更多

Chest X-ray is readily available and usually the first chest investigation. Thoracic CT scan constitutes an alternative and complimentary chest investigation. It is currently the most efficient investigation for the chest and its contents. Objectives: To evaluate the relevance of chest CT Scan requests in two university teaching hospitals in Cameroon. Material and Methods: We conducted a cross-sectional, retrospective and descriptive study at the Radiology and imaging units of the Yaounde Central Hospital and the Yaounde University Teaching Hospital Centre. Included in our study were files of patients who had a chest CT scan investigation during three years. Results: We had a study population of 323 subjects. The age interval was 23 months to 91 years old. Pulmonologist were the most prescribers with 27.2%. We had 80% conformity of indications with French Society of Radiology (FSR) standards. 50 over of 323 indications were not recommended by the FSR. Conclusion: There is a conformity rate of 80% between indications and the FSR recommendations.展开更多

These research studies led to recommendations meant for result amelioration. However, the orthopantomogram (OPT) field remains less studied in that regard in Cameroon. <strong>Aim:</strong> Evaluate the relevance and compliance of examination requests for panoramic dental radiography. <strong>Setting and Design:</strong> It is a descriptive cross-sectional study with a consecutive sample of dental panoramic requests identified during the study period in 4 radiology centres of Yaoundé. <strong>Material and Methods:</strong> Variables studied here were the validity criteria of imaging examination. <strong>Request:</strong> Five of which are of administrative order (date of the request, requesting department, patient’s identity, patient’s age, applicant’s identity) and three of clinical order (anatomic region, reason for the examination and the purpose of the examination). <strong>Statistical Analysis:</strong> Chi-squared test was used with confidence interval of 95%. <strong>Results:</strong> Patient’s identity (name) was the criteria carrying the highest information (98.9%), followed by the name of the applicant physician (91.6%). Out of 179 requests analysed, 8.6% had complete information. 46.4% of requests had no indications. Dental surgeon was the top prescriber with a total of 112 (62.6%). <strong>Conclusion:</strong> The quality of dental panoramic requests was not optimal in Yaounde, with lack of precision mostly noted at the level of clinical criteria.展开更多

Their specific needs and preferences have to be ascertained by healthcare providers to ameliorate service delivery. Objective: To assess the pregnant woman’s expectations during routine prenatal US scan and her perspective ofUSsafety during pregnancy in a Central African obstetric population. Methods: A cross-sectional descriptive survey of consenting pregnant women who reported for routine prenatal US scan using an anonymous questionnaire. A convenient sample of 200 participants was adopted. Results: Thirty-three (16.8%) respondents (on a total of 196) declared they had never done anUSscan. One hundred and eleven (58.4%) on a total of 190 stated that they had not received any information on what ultrasonography is all about. Before theUSscan the respondents would like to receive information on the aim or purpose of ultrasonography, possible inconveniences or risks, and on how to prepare before the scan. The most reported expectations were assurance of the wellbeing of the fetus (58%), gender determination (44.5%) and information on fetal position (20.5%). Thirty-four respondents considered ultrasonography as not perfectly safe for the mother or the “baby”, with the relevant reasons being the use or production of some potentially harmful “rays”. Conclusion: Pregnant women would want to be assured of the wellbeing of the fetus, the gender and position during routine prenatal US. They however need to be informed of its purpose and safety.展开更多

Objectiv...Background: Urethral stricture is a disabling condition that remains prevalent due to the upsurge of sexually transmitted infections and traumatic lesions of the urethra during trauma of the pelvis in humans. Objective: Describe the epidemiological, clinical and radiological presentations of male urethral strictures. Method: It was a cross-sectional and descriptive study during a one-year period from January to December 2017 including all male patients presenting for a urethrocystography examination, at the Ngaoundere Radiology and Medical Imaging Center who agreed to participate. The urethrocystography technique depended on patient’s condition and was either retrograde, anterograde, or following intravenous urography. The examinations were performed with GE X-ray equipment using Kodak Dry View CRs. The interpretation was performed by a radiologist with at least five years of experience. The variables studied were age, clinic, history, uretrocystography technique and results. The data were collected using a previously established data sheet and processed with Sphinx Plus2 V5 and Microsoft Excel 2010 software. Results: 39 patients were enrolled in this study;the average age of our patients was 43.8 years with extremes of 4 and 76 years. The 50 - 60 age group (30.7%) was the most affected. Infection (53.9%) and trauma (23.1%) were the most common etiologies;dysuria (38.5%) and diminution of urinary flow (30.8%) were the most patient’s complaint. Retrograde urethrocystography (66.7%) was the most performed technic. Minimal bleeding in 46.2% of patients was the most common incident during urethrocystography;76.9% of patients had a difficult urination and 7.7% an impassable urethral meatus. After interpretation, 84.6% of patients had urethral stenosis, which was associated with bladder diverticula (23%) and fistulas (15.3%). The stenosis was seated on membranous urethra (81.8%), tight type (45.4%), short (66.7%) and single (81.8%). The post-void residue was significant in 84.6% of cases and not significant in 15.4% of cases. 15.3% of the urethrocystographies were normal. Conclusion: Urethral stenosis in Ngaoundere mainly affects young adult;the main etiology is infectious, very often unique, short, and tight and the membranous urethra is the seat of predilection. Hence, to reduce the extent of pathology with its functional consequences and its impact on the socio-professional life in our context, the focus should be on sexually transmitted infectious.展开更多

Materials and Methods: It was a descriptive and retrospective study ...Objective: To assess the importance of incidental extraspinal findings on Magnetic Resonance Imaging of the lumbar spine in two hospital facilities. Materials and Methods: It was a descriptive and retrospective study from November 2015 to March 2016. The records of patients who had done a Magnetic Resonance Imaging (MRI) scan of the lumbar spine were re-read in search of incidental findings. The incidental findings found were classified using Colonography Reporting and Data System(C-RADS) classification of extracolonic lesions to assess clinical significance. The prevalence of incidental findings was calculated for each facility, as well as the distribution according to age, the organs involved and the clinical importance. A non-detection rate was calculated by confronting the findings of the study with the original reports. Results: The prevalence of incidental findings was respectively 33% (19 out of 36) in Jordan Medical Center (JMC) in Yaounde and 27.74% (106 out of 292) in Jacques Monod Hospital. The extraspinal incidental findings were classified mainly as extracolonic 2 (E2): 58% in each facility. The percentage detection of incidental findings was 5% at JMS and 1.7% at Jacques Monod Hospital. Conclusion: Extraspinal incidental findings are frequent in both hospitals. However, the rate of detection remains very low.展开更多

Pelvic pain 91 (75.2%) was the most associated sign. 36 (29.8%) patients were diagnosed as threatened abortions. 27 (22.3%) were diagnosed with incomplete abortions. 4 (3.3%) cases showed an ectopic gestation. 9 (7.4%) cases showed complete abortions. Concordance between clinical diagnosis and ultrasound results was 84%. Conclusions: Bleeding in the first trimester of pregnancy is common in our area. Ultrasonography occupies a prominent place in the etiological diagnosis of bleeding in the first trimester of pregnancy;it is easy to find in most cases a responsible abnormality of bleeding.
文摘Chest investigation is common in hospital practice. Chest X-ray is readily available and usually the first chest investigation. Thoracic CT scan constitutes an alternative and complimentary chest investigation. It is currently the most efficient investigation for the chest and its contents. Objectives: To evaluate the relevance of chest CT Scan requests in two university teaching hospitals in Cameroon. Material and Methods: We conducted a cross-sectional, retrospective and descriptive study at the Radiology and imaging units of the Yaounde Central Hospital and the Yaounde University Teaching Hospital Centre. Included in our study were files of patients who had a chest CT scan investigation during three years. Results: We had a study population of 323 subjects. The age interval was 23 months to 91 years old. Pulmonologist were the most prescribers with 27.2%. We had 80% conformity of indications with French Society of Radiology (FSR) standards. 50 over of 323 indications were not recommended by the FSR. Conclusion: There is a conformity rate of 80% between indications and the FSR recommendations.
文摘<strong>Background:</strong> Research studies made in Cameroon on compliance and relevance in the various areas of imaging, show the failure to comply with administrative and clinical criteria. These research studies led to recommendations meant for result amelioration. However, the orthopantomogram (OPT) field remains less studied in that regard in Cameroon. <strong>Aim:</strong> Evaluate the relevance and compliance of examination requests for panoramic dental radiography. <strong>Setting and Design:</strong> It is a descriptive cross-sectional study with a consecutive sample of dental panoramic requests identified during the study period in 4 radiology centres of Yaoundé. <strong>Material and Methods:</strong> Variables studied here were the validity criteria of imaging examination. <strong>Request:</strong> Five of which are of administrative order (date of the request, requesting department, patient’s identity, patient’s age, applicant’s identity) and three of clinical order (anatomic region, reason for the examination and the purpose of the examination). <strong>Statistical Analysis:</strong> Chi-squared test was used with confidence interval of 95%. <strong>Results:</strong> Patient’s identity (name) was the criteria carrying the highest information (98.9%), followed by the name of the applicant physician (91.6%). Out of 179 requests analysed, 8.6% had complete information. 46.4% of requests had no indications. Dental surgeon was the top prescriber with a total of 112 (62.6%). <strong>Conclusion:</strong> The quality of dental panoramic requests was not optimal in Yaounde, with lack of precision mostly noted at the level of clinical criteria.
文摘Background: Pregnant women are important stakeholders regarding prenatal ultrasound (US) scanning. Their specific needs and preferences have to be ascertained by healthcare providers to ameliorate service delivery. Objective: To assess the pregnant woman’s expectations during routine prenatal US scan and her perspective ofUSsafety during pregnancy in a Central African obstetric population. Methods: A cross-sectional descriptive survey of consenting pregnant women who reported for routine prenatal US scan using an anonymous questionnaire. A convenient sample of 200 participants was adopted. Results: Thirty-three (16.8%) respondents (on a total of 196) declared they had never done anUSscan. One hundred and eleven (58.4%) on a total of 190 stated that they had not received any information on what ultrasonography is all about. Before theUSscan the respondents would like to receive information on the aim or purpose of ultrasonography, possible inconveniences or risks, and on how to prepare before the scan. The most reported expectations were assurance of the wellbeing of the fetus (58%), gender determination (44.5%) and information on fetal position (20.5%). Thirty-four respondents considered ultrasonography as not perfectly safe for the mother or the “baby”, with the relevant reasons being the use or production of some potentially harmful “rays”. Conclusion: Pregnant women would want to be assured of the wellbeing of the fetus, the gender and position during routine prenatal US. They however need to be informed of its purpose and safety.
文摘Background: Urethral stricture is a disabling condition that remains prevalent due to the upsurge of sexually transmitted infections and traumatic lesions of the urethra during trauma of the pelvis in humans. Objective: Describe the epidemiological, clinical and radiological presentations of male urethral strictures. Method: It was a cross-sectional and descriptive study during a one-year period from January to December 2017 including all male patients presenting for a urethrocystography examination, at the Ngaoundere Radiology and Medical Imaging Center who agreed to participate. The urethrocystography technique depended on patient’s condition and was either retrograde, anterograde, or following intravenous urography. The examinations were performed with GE X-ray equipment using Kodak Dry View CRs. The interpretation was performed by a radiologist with at least five years of experience. The variables studied were age, clinic, history, uretrocystography technique and results. The data were collected using a previously established data sheet and processed with Sphinx Plus2 V5 and Microsoft Excel 2010 software. Results: 39 patients were enrolled in this study;the average age of our patients was 43.8 years with extremes of 4 and 76 years. The 50 - 60 age group (30.7%) was the most affected. Infection (53.9%) and trauma (23.1%) were the most common etiologies;dysuria (38.5%) and diminution of urinary flow (30.8%) were the most patient’s complaint. Retrograde urethrocystography (66.7%) was the most performed technic. Minimal bleeding in 46.2% of patients was the most common incident during urethrocystography;76.9% of patients had a difficult urination and 7.7% an impassable urethral meatus. After interpretation, 84.6% of patients had urethral stenosis, which was associated with bladder diverticula (23%) and fistulas (15.3%). The stenosis was seated on membranous urethra (81.8%), tight type (45.4%), short (66.7%) and single (81.8%). The post-void residue was significant in 84.6% of cases and not significant in 15.4% of cases. 15.3% of the urethrocystographies were normal. Conclusion: Urethral stenosis in Ngaoundere mainly affects young adult;the main etiology is infectious, very often unique, short, and tight and the membranous urethra is the seat of predilection. Hence, to reduce the extent of pathology with its functional consequences and its impact on the socio-professional life in our context, the focus should be on sexually transmitted infectious.
文摘Objective: To assess the importance of incidental extraspinal findings on Magnetic Resonance Imaging of the lumbar spine in two hospital facilities. Materials and Methods: It was a descriptive and retrospective study from November 2015 to March 2016. The records of patients who had done a Magnetic Resonance Imaging (MRI) scan of the lumbar spine were re-read in search of incidental findings. The incidental findings found were classified using Colonography Reporting and Data System(C-RADS) classification of extracolonic lesions to assess clinical significance. The prevalence of incidental findings was calculated for each facility, as well as the distribution according to age, the organs involved and the clinical importance. A non-detection rate was calculated by confronting the findings of the study with the original reports. Results: The prevalence of incidental findings was respectively 33% (19 out of 36) in Jordan Medical Center (JMC) in Yaounde and 27.74% (106 out of 292) in Jacques Monod Hospital. The extraspinal incidental findings were classified mainly as extracolonic 2 (E2): 58% in each facility. The percentage detection of incidental findings was 5% at JMS and 1.7% at Jacques Monod Hospital. Conclusion: Extraspinal incidental findings are frequent in both hospitals. However, the rate of detection remains very low.
