单纯Roux-en-Y胃旁路术治疗超重或肥胖合并食管裂孔疝4例报告

目的:探讨腹腔镜Roux-en-Y胃旁路术对超重或肥胖合并食管裂孔疝患者的治疗效果。方法:回顾分析2012年11月至2019年12月为4例肥胖或超重合并食管裂孔疝患者行减重及代谢手术的临床资料及随访资料。结果:4例患者中2例超重合并2型糖尿病,2... 目的:探讨腹腔镜Roux-en-Y胃旁路术对超重或肥胖合并食管裂孔疝患者的治疗效果。方法:回顾分析2012年11月至2019年12月为4例肥胖或超重合并食管裂孔疝患者行减重及代谢手术的临床资料及随访资料。结果:4例患者中2例超重合并2型糖尿病,2例肥胖合并糖尿病或糖耐量异常,均合并高血压及血脂异常。3例术前胃镜提示食管裂孔疝,其中2例合并反流性食管炎(洛杉矶分级B级),诊断为胃食管反流病。4例术中均探查明确食管裂孔疝。4例患者均行单纯Roux-en-Y胃旁路术,未对食管裂孔疝进行修补。术后随访2~11.5年。肥胖患者术后1年内体重下降明显,多余体重减少百分比分别为63.64%与70.59%。2例超重合并糖尿病患者分别于术后第6个月、5年时血糖升高,另1例肥胖伴糖尿病患者完全缓解。4例患者术后均未出现胃食管反流症状;1例轻度贫血;高血压均缓解,肝功、血脂未见异常。结论:单纯Roux-en-Y胃旁路术治疗肥胖合并食管裂孔疝患者可有效减轻体重,对于超重或肥胖伴随疾病有明显改善,是安全、有效的。 展开更多

Improvement of fabrication efficiency and part performance was the main challenge for the large-scale powder bed fusion(PBF)process.In this study,a dynamic monitoring and feedback system of powder bed temperature field using an infrared thermal imager has been established and integrated into a four-laser PBF equipment with a working area of 2000 mm×2000 mm.The heat-affected zone(HAZ)temperature field has been controlled by adjusting the scanning speed dynamically.Simultaneously,the relationship among spot size,HAZ temperature,and part performance has been established.The fluctuation of the HAZ temperature in four-laser scanning areas was decreased from 30.85℃to 17.41℃.Thus,the consistency of the sintering performance of the produced large component has been improved.Based on the controllable temperature field,a dynamically adjusting strategy for laser spot size was proposed,by which the fabrication efficiency was improved up to 65.38%.The current research results were of great significance to the further industrial applications of large-scale PBF equipment. 展开更多

The high-speed on/off valve(HSV)serves as the fundamental component responsible for generating discrete fluids within digital hydraulic systems.As the switching frequency of the HSV increases,the properties of the generated discrete fluid approach those of continuous fluids.Therefore,a higher frequency response characteristic of HSV is the key to ensure the control accuracy of digital hydraulic systems.However,the current research mainly focuses on its dynamic performance,but neglect its FRC.This paper presents a theoretical analysis demonstrating that the FRC of the HSV can be enhanced by minimizing its switching time.The maximum switching frequency(MSF)is mainly determined by opening dynamic performance when HSV operates with low switching duty ratio(SDR),whereas the closing dynamic performance limits the MSF when HSV operates with high SDR.Building upon these findings,the pre-excitation control algorithm(PECA)is proposed to reduce the switching time of the HSV,and consequently enhance its FRC.Experimental results demonstrate that PECA shortens the opening delay time of HSV by 1.12 ms,the closing delay time by 2.54 ms,and the closing moving time by 0.47 ms in comparison to the existing advanced control algorithms.As a result,a larger MSF of 417 Hz and a wider controllable SDR range from 20%to 70%were achieved at a switching frequency of 250 Hz.Thus,the proposed PFCA in this paper has been verified as an effective and promising approach for enhancing the control performance of digital hydraulic systems. 展开更多

In the intelligent medical diagnosis area,Artificial Intelligence(AI)’s trustworthiness,reliability,and interpretability are critical,especially in cancer diagnosis.Traditional neural networks,while excellent at processing natural images,often lack interpretability and adaptability when processing high-resolution digital pathological images.This limitation is particularly evident in pathological diagnosis,which is the gold standard of cancer diagnosis and relies on a pathologist’s careful examination and analysis of digital pathological slides to identify the features and progression of the disease.Therefore,the integration of interpretable AI into smart medical diagnosis is not only an inevitable technological trend but also a key to improving diagnostic accuracy and reliability.In this paper,we introduce an innovative Multi-Scale Multi-Branch Feature Encoder(MSBE)and present the design of the CrossLinkNet Framework.The MSBE enhances the network’s capability for feature extraction by allowing the adjustment of hyperparameters to configure the number of branches and modules.The CrossLinkNet Framework,serving as a versatile image segmentation network architecture,employs cross-layer encoder-decoder connections for multi-level feature fusion,thereby enhancing feature integration and segmentation accuracy.Comprehensive quantitative and qualitative experiments on two datasets demonstrate that CrossLinkNet,equipped with the MSBE encoder,not only achieves accurate segmentation results but is also adaptable to various tumor segmentation tasks and scenarios by replacing different feature encoders.Crucially,CrossLinkNet emphasizes the interpretability of the AI model,a crucial aspect for medical professionals,providing an in-depth understanding of the model’s decisions and thereby enhancing trust and reliability in AI-assisted diagnostics. 展开更多

Elucidating the genetic basis of natural variation in grain size and weight among rice varieties can help breeders develop high-yielding varieties.We identified a novel gene,GW3a(Grain Weight 3a)(LOC_Os03g27350),that affects rice grain size and weight.gw3a mutants showed higher total starch content and dry matter accumulation than the wild type(WT),Nipponbare,suggesting that GW3a negatively regulates grain size and weight.Moreover,our study found that GW3a interacted with OsATG8 by cleaving it,suggesting that GW3a may be involved in the assembly of autophagosomes and starch degradation in plants.The haplotype analysis of GW3a showed functional differences between indica and japonica rice.Taken together,we conclude that GW3a is expressed in the autophagosome pathway regulating starch metabolism in rice,affecting yield-related traits,such as grain size,grain weight and thousand grain weight(TGW).Our findings also shed new light on autophagy-mediated yield trait regulation,proposing a possible strategy for the genetic improvement of high-yield germplasm in rice. 展开更多

Information steganography has received more and more attention from scholars nowadays,especially in the area of image steganography,which uses image content to transmit information and makes the existence of secret information undetectable.To enhance concealment and security,the Steganography without Embedding(SWE)method has proven effective in avoiding image distortion resulting from cover modification.In this paper,a novel encrypted communication scheme for image SWE is proposed.It reconstructs the image into a multi-linked list structure consisting of numerous nodes,where each pixel is transformed into a single node with data and pointer domains.By employing a special addressing algorithm,the optimal linked list corresponding to the secret information can be identified.The receiver can restore the secretmessage fromthe received image using only the list header position information.The scheme is based on the concept of coverless steganography,eliminating the need for any modifications to the cover image.It boasts high concealment and security,along with a complete message restoration rate,making it resistant to steganalysis.Furthermore,this paper proposes linked-list construction schemeswithin theproposedframework,which caneffectively resist a variety of attacks,includingnoise attacks and image compression,demonstrating a certain degree of robustness.To validate the proposed framework,practical tests and comparisons are conducted using multiple datasets.The results affirm the framework’s commendable performance in terms of message reduction rate,hidden writing capacity,and robustness against diverse attacks. 展开更多

With a further increase in energy flexibility for customers,short-term load forecasting is essential to provide benchmarks for economic dispatch and real-time alerts in power grids.The electrical load series exhibit periodic patterns and share high associations with metrological data.However,current studies have merely focused on point-wise models and failed to sufficiently investigate the periodic patterns of load series,which hinders the further improvement of short-term load forecasting accuracy.Therefore,this paper improved Autoformer to extract the periodic patterns of load series and learn a representative feature from deep decomposition and reconstruction.In addition,a novel multi-factor attention mechanism was proposed to handle multi-source metrological and numerical weather prediction data and thus correct the forecasted electrical load.The paper also compared the proposed model with various competitive models.As the experimental results reveal,the proposed model outperforms the benchmark models and maintains stability on various types of load consumers. 展开更多

目的观察低分子肝素对结直肠癌患者术后静脉血栓栓塞(VTE)的治疗效果。方法收集2016年1月至2023年1月于首都医科大学附属北京天坛医院收治的72例结直肠癌术后发生VTE的患者的临床资料,根据治疗方法的不同将患者分为常规组(接受常规治疗... 目的观察低分子肝素对结直肠癌患者术后静脉血栓栓塞(VTE)的治疗效果。方法收集2016年1月至2023年1月于首都医科大学附属北京天坛医院收治的72例结直肠癌术后发生VTE的患者的临床资料,根据治疗方法的不同将患者分为常规组(接受常规治疗)和观察组(在常规治疗的基础上接受低分子肝素治疗),每组36例。比较两组患者的手术相关指标,包括手术时间、术中出血量、下床活动时间、总住院时间。比较两组患者治疗前(确诊VTE时)、治疗后(治疗14 d后)的凝血功能指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)水平、D-二聚体(D-D)水平]和下肢静脉系统的平均血流速度。结果观察组患者的下床活动时间、总住院时间均明显短于常规组患者,差异均有统计学意义(P﹤0.01)。治疗后,观察组患者的PT、APTT均长于常规组患者,FIB、D-D水平均低于常规组患者,差异均有统计学意义(P﹤0.05)。治疗后,观察组患者股总静脉、股深静脉、股浅静脉、胫后静脉、胫前静脉的平均血流速度均快于常规组患者,差异均有统计学意义(P﹤0.05)。结论低分子肝素能够提高对结直肠癌患者术后VTE的治疗效果,提高患者下肢静脉血流速度,改善患者的凝血功能。 展开更多

目的探讨长链非编码RNA HOTAIR在急性肝损伤中的表达及其对细胞铁死亡及炎症发生的干预作用。方法选取无特定病原体级C57BL/6小鼠,将小鼠分为对照组、急性肝损伤组、急性肝损伤+HOTAIR对照质粒腺病毒干预组(NC组)、急性肝损伤+HOTAIR过... 目的探讨长链非编码RNA HOTAIR在急性肝损伤中的表达及其对细胞铁死亡及炎症发生的干预作用。方法选取无特定病原体级C57BL/6小鼠,将小鼠分为对照组、急性肝损伤组、急性肝损伤+HOTAIR对照质粒腺病毒干预组(NC组)、急性肝损伤+HOTAIR过表达腺病毒干预组(OV组)、急性肝损伤+HOTAIR短发夹RNA干预组(KD组)、急性肝损伤+HOTAIR过表达腺病毒+Fer-1(一种铁死亡抑制剂)干预组(OV+Fer-1组),急性肝损伤模型通过1%四氯化碳溶液5 ml/(kg·d)的量灌胃得到,对照组小鼠灌胃用等量0.9%氯化钠溶液。比较各组小鼠血清和肝组织中HOTAIR、炎症因子[肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)]、肝损伤标志物(丙氨酸转氨酶、天冬氨酸转氨酶)、铁死亡标志物(Fe^(3+)、谷胱甘肽、丙二醛)水平,观察小鼠肝组织病理变化。结果急性肝损伤组小鼠血清HOTAIR表达水平高于对照组[(2.9±0.6)比(1.1±0.3)](P<0.001)。OV组小鼠血清和肝组织中TNF-α、IL-1β及丙氨酸转氨酶、天冬氨酸转氨酶水平均高于NC组,而KD组上述指标均低于NC组(均P<0.05)。OV组小鼠血清和肝组织中丙二醛和Fe^(3+)水平均高于NC组、谷胱甘肽水平低于NC组,而KD组小鼠血清和肝组织中丙二醛和Fe^(3+)水平低于NC组、谷胱甘肽水平高于NC组(均P<0.05)。OV+Fer-1组小鼠血清和肝组织中TNF-α、IL-1β及丙氨酸转氨酶、天冬氨酸转氨酶水平均低于OV组(均P<0.05)。与NC组相比,OV组小鼠肝组织中炎性细胞浸润程度显著增加,而这种作用在OV+Fer-1组中被显著回调。结论HOTAIR可上调急性肝损伤引起的炎症反应,铁死亡信号通路可能介导了这一作用机制。 展开更多

目的了解吉林省农村地区居民健康生活方式与行为素养水平及其影响因素,为进一步做好吉林省居民健康素养水平提升干预工作提供建议。方法采用分层多阶段及PPS法抽取吉林省居民健康素养监测点及脱贫县监测点,以入户调查的方式对15~69岁常... 目的了解吉林省农村地区居民健康生活方式与行为素养水平及其影响因素,为进一步做好吉林省居民健康素养水平提升干预工作提供建议。方法采用分层多阶段及PPS法抽取吉林省居民健康素养监测点及脱贫县监测点,以入户调查的方式对15~69岁常住居民进行面对面调查,抽取其中3249份农村居民调查问卷进行统计分析,计数资料采用率或构成比表示,组间比较采用χ^(2)检验,率的趋势变化采用趋势χ^(2)检验,调查对象健康生活方式与行为素养影响因素采用Logistic回归分析,以P<0.05为差异有统计学意义。结果农村地区居民健康生活方式与行为素养水平为12.10%。咳嗽、打喷嚏应正确遮掩口鼻行为具备率最低,为31.15%。多因素Logistic回归结果显示,健康生活方式与行为素养水平,相对于30岁以下人群,60~69岁居民素养水平较低(OR=0.520,95%CI:0.305~0.888),相对于文化程度在小学及以下人群,大专及以上文化程度居民素养具备率较高(OR=10.544,95%CI:7.165~15.518),相对于家庭常住人口数为1人,家庭人口数在3人及以上的居民健康生活方式与行为素养具备率较高(OR=1.855,95%CI:1.289~2.669)。结论调查的农村地区居民健康生活方式与行为素养水平较低,应进一步加强农村地区健康生活方式与行为的普及与推广。 展开更多

Walled cells,such as in plants and fungi,compose an important part of the model systems in biology.The cell wall primarily prevents the cell from over-expansion when exposed to water,and is a porous material distributed with nanosized pores on it.In this paper,we study the deformation of a membrane patch by an osmotic pressure through a nanopore on the cell wall.We find that there exists a critical pore size or a critical pressure beyond which the membrane cannot stand against the pressure and would inflate out through the pore and further expand.The critical pore size scales linearly with the membrane tension and quadratically with the spontaneous curvature.The critical pressure is inversely proportional to the pore radius.Our results also show that the fluid membrane expansion by pressure is mechanically different from the solid balloon expansion,and predict that the bending rigidity of the membrane in walled cells should be much larger than that of the mammalian cells so as to prevent membrane inflation through the pores on the cell wall. 展开更多

Fusion-born alpha particles in burning plasmas are usually regarded as have a slowing-down distribution,which differs significantly from the Maxwellian distribution of thermal particles in velocity space.A generalized multi-point average method has been developed for gyrokinetic Poisson equation with slowing-down equilibrium distribution using optimization in Fourier space.Its accuracy is verified in both long and short wavelength limits.The influence of changing equilibrium distribution from Maxwellian to slowing-down on gyrokinetic Poisson equation is analyzed to illustrate the significance of the new method.The effect of critical speed in the slowingdown distribution on the field solver is also presented.This method forms an important basis for global gyrokinetic simulation of low-frequency drift Alfvénic turbulence in burning plasmas. 展开更多

目的:探讨小剂量右美托咪定(dexmedetomidine,DEX)静脉输注对胸腔镜下肺癌根治术老年患者血浆β-淀粉样蛋白和术后认知功能的影响。方法:选取2016年6月至2018年3月我院择期全麻下拟行胸腔镜下肺癌根治术老年患者60例作为研究对象,按随... 目的:探讨小剂量右美托咪定(dexmedetomidine,DEX)静脉输注对胸腔镜下肺癌根治术老年患者血浆β-淀粉样蛋白和术后认知功能的影响。方法:选取2016年6月至2018年3月我院择期全麻下拟行胸腔镜下肺癌根治术老年患者60例作为研究对象,按随机数字表法分为两组,右美托咪定组(DEX组)和生理盐水组(NS组),每组患者各30例。DEX组患者支气管插管成功后经静脉持续泵注右美托咪定0.4μg·(kg·h)^(-1)输注至手术结束,NS组患者泵注等量的NS。两组患者分别于麻醉开始前(T1)及手术开始后2 h(T2)、4 h(T3)、24 h(T4)、7 d(T5)采集外周静脉血标本,采用酶联免疫吸附试验(ELISA)法测定β-淀粉样蛋白含量。两组患者分别于术前1 d和术后7 d行神经心理测验评估认知功能,比较两组患者术后认知功能障碍(post operative cognitive dysfunction,POCD)的发生率。结果:两组患者组间比较,DEX组T2~T5时点血浆β-淀粉样蛋白含量较NS组均降低(P<0.05)。两组患者组内比较,与术前T1时点比较,两组患者T2~T5时血浆β-淀粉样蛋白含量均逐渐升高,于T4时点达到峰值,DEX组T5时点与T1时点比较差异无统计学意义(P>0.05),而NS组T5时点仍高于T1时点(P<0.05)。DEX组POCD发生率为6.7%,低于NS组的26.7%(P<0.05)。结论:右美托咪定用于全麻下老年患者胸腔镜肺癌根治术对患者的认知功能具有保护作用,可以降低POCD的发生率,其作用机制可能与右美托咪定能降低患者血浆β-淀粉样蛋白有关。 展开更多

重型和危重型COVID-19患者实施气管插管的临床经验 被引量:10

目的对18例实施气管插管的重型和危重型新型冠状病毒肺炎(COVID-19)患者进行临床分析。方法采用回顾性研究方法,对这18例COVID-19患者的气管插管情况进行分析。记录患者的年龄、性别、插管前意识状态、给氧方式等基本信息。由实施气管... 目的对18例实施气管插管的重型和危重型新型冠状病毒肺炎(COVID-19)患者进行临床分析。方法采用回顾性研究方法,对这18例COVID-19患者的气管插管情况进行分析。记录患者的年龄、性别、插管前意识状态、给氧方式等基本信息。由实施气管插管的麻醉医生评估并记录患者的插管条件。记录患者麻醉诱导前及插管后的基本生命体征变化。结果患者平均年龄70.39±8.02岁,插管前15例(83.33%)患者进行了无创呼吸机治疗,13例患者(72.22%)插管前意识状态是清醒的。麻醉诱导后,患者的血压、心率有显著下降(P<0.05)。患者气管插管条件的优良率为94.44%,一次插管成功率为100%。目前有死亡5例,有1例已行气管切开,有12例患者仍在ICU行气管插管机械通气治疗,其中1例患者机械通气效果差,目前已行ECMO辅助。共16名临床经验丰富的麻醉医生参与了气管插管操作,插管时均采取三级防护,目前尚未发生医务人员感染的情况。结论对于有气管插管指征的重型和危重型新冠肺炎患者,建议早期插管,给予有创呼吸支持,增加患者的救治机会。插管过程中需谨慎使用麻醉药物,加强医务人员的防护,最大化保障患者和医务人员的安全。 展开更多

针对电力系统数据来自多个源头、现场运行维护人员日常巡检工作量大、人工核查出错率高的问题,研究了一种模糊匹配技术,模糊匹配多种数据源的数据并导入为基准值,定时巡检与基准值进行比对,对巡检周期内的巡检信息进行自动整理、智能分... 针对电力系统数据来自多个源头、现场运行维护人员日常巡检工作量大、人工核查出错率高的问题,研究了一种模糊匹配技术,模糊匹配多种数据源的数据并导入为基准值,定时巡检与基准值进行比对,对巡检周期内的巡检信息进行自动整理、智能分析,形成多种巡检报告并以Web方式展示。该技术具有匹配速度快,准确率高,混淆率低的特点,在广东电网获得了成功应用,有效替代了运行维护人员对电网安全稳定装置的日常巡检、定检及年检等工作,减少了人为参与,降低了现场运维人员核对的工作量及人工核查出错率,提高了工作效率。 展开更多
