Based on the NCVEM approximation of state equation and the variational discretization of control variables, we construct a virtual element discrete scheme. For the state, adjoint state and control variable, we obtain the corresponding prior estimate in H<sup>1</sup> and L<sup>2</sup> norms. Finally, some numerical experiments are carried out to support the theoretical results.展开更多

He had bee...In this report, we describe radiation-induced difficult airway management in a patient with nasopharyngeal carcinoma. The patient was presented to receive laparoscopic cholecystectomy for gallbladder stone. He had been diagnosed to have nasopharyngeal cancer about 2 years ago. In operation, after sleeping, the patient was manual controlled ventilation. However, we subsequently found that his neck campaign was limited and mask ventilation was obstructed. We immediately performed oropharyngeal airway, then mask ventilation improved. Fully surface anesthesia with tetracaine atomizing to the root of tongue, larynx wall and piriform recess, the patient was endotracheal intubated with fiberoptic bronchoscope. After intubation, the patient inhaled 2.5% sevoflurane, then esmeron (50 mg) and remifentanyl (0.1 μg/kg every minute) were administrated by intravenous. After the treatment, the patient's life indexes were normal and steady. In conclusion, patients with nasopharyngeal carcinoma (NPC) after radiation therapy should be based on comprehensive evaluation of upper airway and obstructive condition before operation, then perform safe and effective tracheal intubation methods under spontaneous breathing.展开更多

Background:Understanding the clinical implications of autoimmune manifestations associated with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is essential to reduce its consequences. This study was aime...Background:Understanding the clinical implications of autoimmune manifestations associated with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is essential to reduce its consequences. This study was aimed at determining the activities of new-onset primary Sjögren syndrome (pSS) since the emergence of SARS-CoV-2.Methods:This retrospective cohort study included data from 471 participants with dry mouths and eyes who had been attending Nanjing Drum Tower Hospital since December 2019. By April 2023, patients diagnosed with pSS were sequentially assigned to vaccinated group ( n = 24) or vaccinated and infected group based on exposure ( n = 20). Patients diagnosed with pSS within 3 months of vaccination against SARS-CoV-2 were assigned to a vaccinated group, and those who had been vaccinated and then developed pSS within 3 months of follow up after direct exposure to SARS-CoV-2 were assigned to a vaccinated and infected group. The controls comprised age- and sex-matched patients who had not been exposed to SARS-CoV-2 before December 2019 ( n = 21). We then compared data among the three groups. Results:The vaccinated and infected patients had more fever, malaise, splenomegaly, and weight loss before diagnosis and a higher European Alliance of Associations for Rheumatology Sjögren's syndrome disease activity index at the time of onset than the other two groups. Vaccinated patients had a higher frequency of anti-nuclear antibody (ANA) titers > 1:320 ( n = 12;50%) and anti-phospholipid antibodies (aPL) ( n = 7;29%) than the controls. The frequency of anti-Ro/SSA antibodies (13, 65%), ANA titers > 1:320 ( n = 16;80%), and aPLs ( n = 7;29%) ( n = 5;25%) were all significantly higher in vaccinated patients with infection than those in the controls. Higher doses of glucocorticoids, cyclosporin A, and tacrolimus were administered to the vaccinated and infected than the vaccinated and control groups ( p < 0.05, for all). Conclusions:Patients with new-onset pSS and a history of vaccination and SARS-CoV-2 infection might have more active disease. Further strengthening the assessment of people with a clear history of SARS-CoV infection and the monitoring of potential populations for autoimmune screening should not be overlooked.展开更多
Mesenchymal stem cells(MSCs)are critical for immune regulation.Although several microRNAs(miRNAs)have been shown to participate in autoimmune pathogenesis by affecting lymphocyte development and function,the roles of ...Mesenchymal stem cells(MSCs)are critical for immune regulation.Although several microRNAs(miRNAs)have been shown to participate in autoimmune pathogenesis by affecting lymphocyte development and function,the roles of miRNAs in MSC dysfunction in autoimmune diseases remain unclear.Here,we show that patients with systemic lupus erythematosus(SLE)display a unique miRNA signature in bone marrow-derived MSCs(BMSCs)compared with normal controls,among which miR-663 is closely associated with SLE disease activity.MiR-663 inhibits the proliferation and migration of BMSCs and impairs BMSC-mediated downregulation of follicular T helper(Tfh)cells and upregulation of regulatory T(Treg)cells by targeting transforming growth factorβ1(TGF-β1).MiR-663 overexpression weakens the therapeutic effect of BMSCs,while miR-663 inhibition improves the remission of lupus disease in MRL/lpr mice.Thus,miR-663 is a key mediator of SLE BMSC regulation and may serve as a new therapeutic target for the treatment of lupus.展开更多
Although the development of titanium alloys with working temperatures above 600?C faces enormous difficulties and challenges,the related research has not stopped.In the present work,detailed analyses on microstructure...Although the development of titanium alloys with working temperatures above 600?C faces enormous difficulties and challenges,the related research has not stopped.In the present work,detailed analyses on microstructure evolution and hot deformation behavior of a new temperature resistant 650?C titanium alloy Ti65 were investigated from micrometer scale to nanometer scale.The results revealed that lamellarαgrains gradually fragmentized and spheroidized during theα+βphase region compression and the orientation of the c-axis ofαgrains gradually aligned to radial directions,forming two high Schmid factors(SFs)value texture eventually with the increase of strain to 0.7.Moreover,there were some strengthening characters in theα+βphase region such as lenticularαsand nano silicide(TiZr)6 Si3.In theβphase region,fine equiaxed dynamic recrystallized(DRX)βgrains were formed.Besides,the variant selection ofαm′artensite followed Burgers orientation relationship during the compression process.The main deformation mechanisms of theα+βphase region were dislocation slip and orientation dependent spheroidization.Whereas,the deformation process in theβphase region was controlled byβgrain DRX.Interestingly,many nano scale FCC twins were generated at the interface ofαl′ath during deforming in theβphase region,which was firstly observed in Ti65 alloy.展开更多
The effect of Mo additions on the microstructures and mechanical properties of CoCrNi alloys was investigated,meanwhile,ab initio calculations are performed to quantitatively evaluate the lattice distortion and stacki...The effect of Mo additions on the microstructures and mechanical properties of CoCrNi alloys was investigated,meanwhile,ab initio calculations are performed to quantitatively evaluate the lattice distortion and stacking fault energy(SFE).The yield strength,ultimate tensile strength,and elongation of(CoCrNi)_(97)Mo_(3)alloy are 475 MPa,983 MPa and 69%,respectively.The yield strength is increased by~30%and high ductility is maintained,in comparison with CoCrNi alloy.Besides the nano-twins and dislocations,the higher density of stacking faults is induced during the tensile deformation for(CoCrNi)_(97)Mo_(3)alloy.Ab initio calculation results indicate the mean square atomic displacement(MSAD)and SFE value of(CoCrNi)_(97)Mo_(3)alloy is 42.6 pm^(2)and-40.4 mJ/m^(2)at 0 K,respectively.The relationship between mechanical properties and MSAD,SFE for various multiple principal element alloys is discussed.展开更多
