住院医师规范化培训质量控制评估体系的构建与应用价值分析

目的分析构建住院医师规范化培训质量控制评估体系的方法及应用价值。方法选取2023年1月至2023年12月在三甲医院接受住院医师规范化培训住培医师113名,针对研究访谈提纲进行设计并选取研究调查问卷,构建、评价和修正结构方程模型,有针... 目的分析构建住院医师规范化培训质量控制评估体系的方法及应用价值。方法选取2023年1月至2023年12月在三甲医院接受住院医师规范化培训住培医师113名,针对研究访谈提纲进行设计并选取研究调查问卷,构建、评价和修正结构方程模型,有针对性地加强管理以不断提升教学质量和水平,对比加强管理前后住培医师对医学教育环境的满意度、住培医师的满意度及住培医师培训能力。结果强化管理后,住培医师医学教育环境量表评分明显高于强化管理前,强化管理前后住培医师教育环境量表评分具有统计学差异(P<0.05)。强化管理后住培医师培训效果满意度评分明显高于强化管理前,强化管理前后住培医师培训效果满意度评分具有统计学差异(P<0.05)。住培医师培训后住培医师理论知识获得了较大幅度的提升。结论构建住院医师规范化培训指导老师教学质量评价体系并实施加强管理有助于提升住培教学质量。 展开更多

Anti-colon cancer activity of water-soluble polysaccharides extracted from Gloeostereum incarnatum via Wnt/β-catenin signaling pathway 被引量:1

The incidence rate of colon cancer ranks the third among malignant tumors worldwide.Gloeostereum incarnatum(GI),a fungus with homology of medicine and food,has multiple pharmacological activities.It was investigated in this study that the anti-colon cancer effect of polysaccharides purifi ed from GI(GIPS)in ApcMinC/Gpt mice(an in situ colon cancer mouse model).Eight-week administration with GIPS at doses of 30 or 90 mg/kg strongly inhibited tumor growth including the reduction on numbers and the suppression of the size without infl uencing the animals’body weight and organ functions.According to the proteomics performing by antibody array,among 308 detected cytokines,GIPS significantly regulated 89 of them.Compared with vehicle-treated mice,GIPS effectively reduced the levels of interleukin(IL)-1β,IL-4,IL-6,IL-17,IL-22,tumor necrosis factor(TNF)-α,matrix metalloproteinase(MMP)-2,and enhanced the levels of IL-15 and IL-18 in serum and/or colon tissues,which suggested its anti-inflammation of GIPS.GIPS suppressed nuclear aggregation of β-catenin,affected the expression of WNT1 and related proteins,thereby regulated the activation of the Wnt signaling.Altogether,GIPS can inhibit the growth of colon cancer,at least partially,via inhibiting the Wnt/β-catenin signaling pathway. 展开更多

Numerical study of aerosol effect on three types of clouds and precipitation in Beijing area

Three types of rainfall (storm, moderate and slight rainfall) in the Beijing area were simulated by the Weather Research and Fore- cast (WRF3.2) model coupled with Milbrandt-two-moment cloud microphysics scheme, to explore the effect of aerosols on clouds and precipitation under continental and maritime aerosol scenarios. Results indicate that an increase of aerosols has various effects on clouds and precipitation. (1) The amount of surface precipitation is obviously affected. With an increase of aerosol con- centration, the 48-hr total precipitation of storm and moderate rainfall decreased by 23% and 16.6%, respectively, and the 24-hr total precipitation of slight rainfall decreased by 14.0%. (2) The distribution of surface precipitation is also clearly affected. The average precipitation for a rain storm increases in most parts of western Beijing and decreases by more than 20 mm in most parts of eastern Beijing with increasing aerosol concentration. The average precipitation of moderate rainfall decreases by 0.1-5 mm in most parts of the Beijing area. The effect of increased aerosol concentration is weak for slight rainfall distribution in the study area. (3) With an increase of aerosol concentration, a narrower width and lower precipitation peak value are found in the storm rainfall, and its duration is prolonged for the high aerosol concentration. An earlier precipitation termination of moderate rainfall is found with increasing aerosol concentration. (4) The upper-air hydrometeors vary with aerosol concentration, For storm and moderate rainfall, significantly higher cloud water concentration and lower rain water were found under the continental aerosol scenario. 展开更多

The interaction among type Ⅱ collagen(CⅡ),human DR4 major histocompatibility complex type Ⅱ molecule(MHC Ⅱ)and T-cell receptor(TCR)is associated with the development of rheumatoid arthritis(RA).The activation of T cells can be reduced through exposure to modified CⅡ(263-272)glycopeptide fragment via competitive inhibition with self-antigen.In this work,30 peptides based on the sequence of CⅡ(263-272)were prepared and evaluated for their binding to DR4 protein by surface plasmon resonance(SPR)assay.The effect on the secretion of pro-inflammatory factors by the spleen cells in collagen induced rheumatoid arthritis(CIA)mouse was also investigated.Two N-glycosylated CⅡ peptides were identified to have strong binding to the human recombinant DR4 protein and weak proinflammatory effect.These glycopeptides could be developed as therapeutic saccharide vaccines for the treatment of rheumatoid arthritis(RA). 展开更多

Rheumatoid arthritis(RA)is an inflammatory autoimmune disease triggered by antigenic peptides with environmental and genetic risk factors.It has been shown that antigen-specific targeting could be a promising therapeutical strategy for RA by restoring immune tolerance to self-antigens without compromising normal immunity.Citrullination of antigens enhances antigenic properties and induces autoimmune responses.Here,we showed that citrullinated antigenic(citAg)vaccine ameliorated collageninduced arthritis with decreased T-helper 1(Th1)and Th17 cells,downregulated proinflammatory cytokines including interlukin-6 and tumor necrosis factor-a,and inhibited antigen recall responses.B cell receptor sequencing further revealed that citAg vaccine could dampen the dysregulated V(D)J recombination,restoring the immune repertoire.Taken together,the results demonstrated that citAg vaccine might have a therapeutic effect on RA. 展开更多

Polypropylene(PP)mesh is commonly used in abdominal wall repair due to its ability to reduce the risk of organ damage,infections and other complications.However,the PP mesh often leads to adhesion formation and does not promote functional tissue repair.In this study,we synthesized one kind of aldehyde Bletilla striata polysaccharide(BSPA)modified chitosan(CS)hydrogel based on Schiff base reaction.The hydrogel exhibited a porous network structure,a highly hydrophilic surface and good biocompatibility.We wrapped the PP mesh inside the hydrogel and evaluated the performance of the resulting composites in a bilateral 1×1.5cm abdominal wall defect model in rats.The results of gross observation,histological staining and immunohistochemical staining demonstrated the positive impact of the CS hydrogel on anti-adhesion and wound healing effects.Notably,the addition of BSPA to the CS hydrogel further improved the performance of the composites in vivo,promoting wound healing by enhancing collagen deposition and capillary rearrangement.This study suggested that the BSPA-modified CS hydrogel significantly promoted the anti-adhesion,anti-inflammatory and pro-angiogenesis properties of PP meshes during the healing process.Overall,this work offers a novel approach to the design of abdominal wall repair patches. 展开更多

No-tillage planters need to be equipped with row cleaners to remove post-harvest plant residue from the seedbed.The two-disc row cleaners cannot effectively remove the plant residue at high speed because the working depth is unstable,which leads to poor seeding quality of the seeder.A straw cleaning unit with double air springs was designed to achieve better straw cleaning performance at high speeds.The analysis of the mechanical characteristics of the double air spring system showed that it enabled separate adjustment of force and stiffness.A dynamic model of the straw cleaning unit was established,and the effectiveness of the double air spring system with adjustable stiffness in stabilizing the working depth of the row cleaners was analyzed.Field experiments were conducted to evaluate the straw cleaning performance and consistency of downforce against the ground of the straw cleaning unit with double air springs at different high speeds.The results showed that the stiffness of the double air spring system for better straw cleaning performance of the straw cleaning unit was different at different working speed,and the required stiffness increased as the working speed increased;When the working speed was 8-12 km/h,the coefficient of variation of cleaned strip width was 6.9%-12.1%,the straw cleaning rate was 81.6%-92.2%and the root mean square error of downforce was 19.93-28.63 N;the coefficient of variation of cleaned strip width was moderately positively correlated with the root mean square error of downforce,and the cleaned strip width consistency was better when the root mean square error of downforce was lower than 25.00 N. 展开更多

Dengue virus (DENV) remains a major public health threat because no vaccine or drugs are available for the prevention and treatment of DENV infection, and the immunopathogenesis mechanisms of DENV infection are not fully understood. Cytotoxic molecules, such as granzyme B (GrzB), may be necessary to control viral infections. However, the exact role of GrzB during DENV infection and the mechanisms regulating GrzB expression during DENV infection are not clear. This study found that miR-27a~, miR-3Oe, and miR-378 were down-regulated in DENV-infected patients, and DENV infection in humans induced a significant up-regulation of GrzB in natural killer (NK) cells and CD8+ T cells. Further investigation indicated that NK cells, but not CD8+ T cells, were the major sources of GrzB, and miR-378, but not miR-27a~ or miR-3Oe, suppressed GrzB expression in NK cells. Notably, we found that overexpression of miR-378 using a miR-378 agomir in DENV-infected mice inhibited GrzB expression and promoted DENV replication. These results suggest the critical importance of miR-378 in the regulation of GrzB expression and a protective role for GrzB in controlling DENV replication in vivo. Therefore, this study provides a new insight into the immunopathogenesis mechanism of DENV infection and a biological basis for the development of new therapeutic strategies to control DENV infection. 展开更多

Distributions of invasive species are commonly predicted with species distribution models that build upon the statistical relationships between observed species presence data and climate data. We used field observations, climate station data, and Maximum Entropy species distribution models for 13 invasive plant species in the United States, and then compared the models with inputs from a General Circulation Model (hereafter GCM-based models) and a downscaled Regional Climate Model (hereafter, RCM-based models). We also compared species distributions based on either GCM-based or RCM-based models for the present (1990-1999) to the future (2046- 2055). RCM-based species distribution models replicated observed distributions remarkably better than GCM- based models for all invasive species under the current climate. This was shown for the presence locations of the species, and by using four common statistical metrics to compare modeled distributions. For two widespread invasive taxa (Bromus tectorum or cheatgrass, and Tamarix spp. or tamarisk), GCM-based models failed miserably to reproduce observed species distributions. In contrast, RCM-based species distribution models closely matched observations. Future species distributions may be significantly affected by using GCM-based inputs. Because invasive plants species often show high resilience and low rates of local extinction, RCM-based species distribution models may perform better than GCM-based species distribution models for planning containment programs for invasive species. 展开更多
