目的:调查广州市耐多药肺结核患者自付医疗费用和影响因素,为优化耐多药肺结核患者医疗报销政策提供依据。方法:对2020年1月1日至2022年12月31日在广州市胸科医院耐多药专科诊室接受治疗的患者284例,采用自行设计的调查问卷通过面对面... 目的:调查广州市耐多药肺结核患者自付医疗费用和影响因素,为优化耐多药肺结核患者医疗报销政策提供依据。方法:对2020年1月1日至2022年12月31日在广州市胸科医院耐多药专科诊室接受治疗的患者284例,采用自行设计的调查问卷通过面对面或电话进行耐多药肺结核相关因素调查。通过医院信息系统收集患者自付医疗费用(包括实验室检查费、抗结核药品费、非抗结核药品费、住院费等)。采用描述性统计分析、卡方检验分析不同年龄、性别、家庭收入、职业及教育程度等情况下耐多药肺结核患者的自付医疗费用。采用逐步多重线性回归分析耐多药肺结核患者自付医疗费用的影响因素;检验水准α=0.05。结果:284例耐多药肺结核患者自付医疗费用中位数(四分位数)为99927.25(40136.45,202526.91)元,前3位分别为抗结核药品费、实验室检查费和非抗结核药品费,中位数(四分位数)分别为46676.27(5590.92,150869.86)元、22130.59(15609.23,31417.06)元和12751.68(7977.07,19418.64)元;多因素分析模型中,耐多药肺结核患者自付医疗费用的保护因素是广州市户籍[OR(95%CI)=0.95(0.84~0.99)]、全程面视服药管理[OR(95%CI)=0.73(0.65~0.87)],危险因素是药物不良反应[OR(95%CI)=1.33(1.19~1.46)]、使用利奈唑胺[OR(95%CI)=2.23(1.92~2.59)]、住院治疗[OR(95%CI)=1.57(1.32~1.91)]。结论:广州市耐多药肺结核患者自付医疗费用较高,应规范耐多药患者服药管理,减少药物不良反应,优化医疗报销政策。 展开更多

目的 探究重症监护病房(ICU)患者家属疾病不确定感与领悟社会支持、心理弹性的相关性。方法 选取508名2020年1月—2022年1月在成都市第三人民医院ICU住院患者的家属进行调查,采用问卷调查法调查患者一般资料、家属疾病不确定感(MUIS-FM... 目的 探究重症监护病房(ICU)患者家属疾病不确定感与领悟社会支持、心理弹性的相关性。方法 选取508名2020年1月—2022年1月在成都市第三人民医院ICU住院患者的家属进行调查,采用问卷调查法调查患者一般资料、家属疾病不确定感(MUIS-FM)评分、心理弹性(RS-14)评分和领悟社会支持(PSSS)评分;采用线性回归分析自变量RS-14和PSSS评分对MUIS-FM评分的影响;采用Pearson相关性分析MUIS-FM各维度评分与RS-14和PSSS评分的相关性。结果 ICU患者家属疾病不确定感总分为(70.58±10.53)分;线性回归分析结果显示,RS-14评分(β=-1.493)和PSSS评分(β=-0.153)对ICU患者家属的疾病不确定感具有负向预测作用(P<0.05);Pearson相关性分析结果显示,MUIS-FM各维度评分与RS-14和PSSS评分均分别呈现负相关(P<0.05)。心理弹性在ICU患者家属领悟社会支持和疾病不确定感中的中介效应值为0.29,占总效应比例为39.19%。结论ICU患者家属疾病不确定感处于较高水平,领悟社会支持和心理弹性处于中等水平,且患者家属疾病不确定感与领悟社会支持和心理弹性具有显著相关性。 展开更多

Transcription factors(TFs)play essential roles in transcriptional reprogramming during activation of plant immune responses to pathogens.OsSPL10(SQUAMOSA promoter binding protein-like10)is an important TF regulating trichome development and salt tolerance in rice.Here we report that knockout of OsSPL10 reduces whereas its overexpression enhances rice resistance to blast disease.OsSPL10 positively regulates chitin-induced immune responses including reactive oxygen species(ROS)burst and callose deposition.We show that OsSPL10 physically associates with OsJAmyb,an important TF involved in jasmonic acid(JA)signaling,and positively regulates its protein stability.We then prove that OsJAmyb positively regulates resistance to blast.Our results reveal a molecular module consisting of OsSPL10 and OsJAmyb that positively regulates blast resistance. 展开更多

In the reversed field pinch(RFP),plasmas exhibit various self-organized states.Among these,the three-dimensional(3D)helical state known as the“quasi-single-helical”(QSH)state enhances RFP confinement.However,accurately describing the equilibrium is challenging due to the presence of 3D structures,magnetic islands,and chaotic regions.It is difficult to obtain a balance between the available diagnostic and the real equilibrium structure.To address this issue,we introduce KTX3DFit,a new 3D equilibrium reconstruction code specifically designed for the Keda Torus eXperiment(KTX)RFP.KTX3DFit utilizes the stepped-pressure equilibrium code(SPEC)to compute 3D equilibria and uses polarimetric interferometer signals from experiments.KTX3DFit is able to reconstruct equilibria in various states,including axisymmetric,doubleaxis helical(DAx),and single-helical-axis(SHAx)states.Notably,this study marks the first integration of the SPEC code with internal magnetic field data for equilibrium reconstruction and could be used for other 3D configurations. 展开更多

The trajectory of the compact torus(CT)within a tokamak discharge is crucial to fueling.In this study,we developed a penetration model with a vacuum magnetic field region to accurately determine CT trajectories in tokamak discharges.This model was used to calculate the trajectory and penetration parameters of CT injections by applying both perpendicular and tangential injection schemes in both HL-2A and ITER tokamaks.For perpendicular injection along the tokamak's major radius direction from the outboard,CTs with the same injection parameters exhibited a 0.08 reduction in relative penetration depth when injected into HL-2A and a 0.13reduction when injected into ITER geometry when considering the vacuum magnetic field region compared with cases where this region was not considered.In addition,we proposed an optimization method for determining the CT's initial injection velocity to accurately calculate the initial injection velocity of CTs for central fueling in tokamaks.Furthermore,this paper discusses schemes for the tangential injection of CT into tokamak discharges.The optimal injection angle and CT magnetic moment direction for injection into both HL-2A and ITER were determined through numerical simulations.Finally,the kinetic energy loss occurring when the CT penetrated the vacuum magnetic field region in ITER was reduced byΔEk=975.08 J by optimizing the injection angle for the CT injected into ITER.These results provide valuable insights for optimizing injection angles in fusion experiments.Our model closely represents actual experimental scenarios and can assist the design of CT parameters. 展开更多

A multi-channel polarimeter-interferometer has been developed on the Keda Torus eXperiment(KTX)for the study of equilibrium dynamics and internal magnetic fluctuations.A three-wave technique based on terahertz solid-state sources(-650 GHz)is applied for simultaneous measurements of electron density and Faraday rotation angle.The output power of the microwave source is 2 mW.Faraday rotation effect using a rotating wave plate is tested with phase noise less than 0.8°,and the density phase noise is less than 0.9°.Measurement of Faraday rotation angle and density for discharges on KTX have demonstrated high sensitivity to internal MHD activities. 展开更多

Density limit has long been a widely studied issue influencing the operating range of tokamaks.The rapid growth of the m/n=2/1(where m and n are poloidal and toroidal mode numbers,respectively)tearing mode is generally regarded as a primary precursor to the density limit disruption.In this experiment,the coupling of the m/n=1/1 mode and the m/n=2/1 mode in highdensity plasma was observed.During a sawtooth cycle,the frequencies of the two modes gradually converge until they become equal.After that,toroidal coupling occurs between the 1/1 and 2/1 modes,resulting in a mutually fixed phase relationship.With the occurrence of toroidal coupling,the 2/1 mode is stabilized.Prior to the disruption,the cessation of the 1/1 and 2/1 mode coupling,along with the rapid growth in the amplitude of the 2/1 mode,can be observed.Additionally,under the same parameters,comparing discharges with or without the 1/1 mode,it is found that the presence of the 1/1 mode leads to higher plasma density and temperature parameters. 展开更多

Many experiments have demonstrated that resonant magnetic perturbation(RMP) can affect the turbulent transport at the edge of the tokamak. Through the Experimental Advanced Superconducting Tokamak(EAST) density modulation experiment, the particle transport coefficients were calculated using the experimental data, and the result shows that the particle transport coefficients increase with RMP. In this study, the six-field two-fluid model in BOUT++ is used to simulate the transport before and after density pump-out induced by RMP,respectively referred as the case without RMP and the case with RMP. In the linear simulations,the instabilities generally decreases for cases with RMP. In the nonlinear simulation, ELM only appears in the case without RMP. Additionally, the particle transport coefficient was analyzed,and the result shows that the particle transport coefficient becomes larger for the case with RMP,which is consistent with the experimental conclusion. Moreover, its magnitude is comparable to the results calculated from experimental data. 展开更多

The optical design of near-infrared phase contrast imaging(NI-PCI)diagnosis on HL-2A is introduced in this paper.This scheme benefits from the great progress of near-infrared laser technology and is a broadening of traditional phase contrast technology.This diagnostic can work as a keen tool to measure plasma wavenumber spectra by inferring string-integrated plasma density fluctuations.Design of both the front optical path which is the path before the laser transmitting into the tokamak plasma and the rear optics which is the path after the laser passing through the plasma is detailed.The 1550 nm laser is chosen as the probe beam and highprecision optical components are designed to fit the laser beam,in which a phase plate with a 194-nm-deep silver groove is the key.Compared with the conventional 10.6μm laser-based PCI system on HL-2A,NI-PCI significantly overcomes the unwanted phase scintillation effect and promotes the measurement capability of high-wavenumber turbulence with an increased maximal measurable wavenumber from 15 cm^(-1)to 32.6 cm^(-1). 展开更多

目的双源CT虚拟单能量成像(VMI)技术在消除椎弓螺钉金属伪影方面具有显著的应用价值,本研究选择伪影最严重层面上四个不同的位置,从伪影指数和对比度噪声比方面探讨双源CT虚拟单能量成像技术消除椎弓螺钉金属伪影的应用价值。方法纳入4... 目的双源CT虚拟单能量成像(VMI)技术在消除椎弓螺钉金属伪影方面具有显著的应用价值,本研究选择伪影最严重层面上四个不同的位置,从伪影指数和对比度噪声比方面探讨双源CT虚拟单能量成像技术消除椎弓螺钉金属伪影的应用价值。方法纳入48例行双源CT检查椎弓根螺钉植入情况的患者,获得14组(60~190keV,间隔10keV)虚拟单能量图像和1组(140kVp)混合能量图像,测量金属植入物周围的4个兴趣区和同一层面1个脂肪兴趣区的CT值和标准差,并算出伪影指数和对比度噪声比。针对伪影严重程度及金属周围组织清晰度进行主观评分。比较不同图像伪影指数、对比度噪声比和主观评分差异,评估VMI去除金属伪影的价值。结果随着keV的增加,各感兴趣区的伪影指数减小,而对比度噪声比增加。能级110~190keV时伪影指数均低于140kVp图像,且差异有统计学意义(P<0.05)。能级110~190keV时各个感兴趣区的对比度噪声比均高于140kVp混合式能量图像,且差异有统计学意义(P<0.05)。主观评分方面,110~150keV图像评分高于140kVp图像,且差异有统计学意义(P<0.05)。结论双源CT单能量成像技术可有效减少脊柱螺钉内固定后金属伪影的影响,能级110~150keV的单能量图像伪影指数低和对比度噪声比高,并且主观评价显示伪影消除效果良好,是消除椎弓根螺钉金属伪影的最佳单能量值。 展开更多

Compact torus(CT)injection is a highly promising technique for the central fueling of future reactor-grade fusion devices since it features extremely high injection velocity and relatively high plasma mass.Recently,a CT injector for the EAST tokamak,EAST-CTI,was developed and platform-tested.In the first round of experiments conducted with low parameter settings,the maximum velocity and mass of the CT plasma were 150 km·s^(-1)and 90μg,respectively.However,the parameters obtained by EAST-CTI were still very low and were far from the requirements of a device such as EAST that has a strong magnetic field.In future,we plan to solve the spark problem that EAST-CTI currently encounters(that mainly hinders the further development of experiments)through engineering methods,and use greater power to obtain a more stable and suitable CT plasma for EAST. 展开更多

Plant trichomes are a specialized cellular tissue that functions in resistance to biotic and abiotic stresses.In rice,three transcription-factor genes:OsWOX3B,HL6,and OsSPL10,have been found to control trichome development.Although studies have shown interactions between the three genes,their full relationship in trichome development is unclear.We found that the expression levels of OsWOX3B and HL6 were both reduced in OsSPL10-knockout plants but increased in OsSPL10-overexpression plants,suggesting that OsSPL10 positively regulates their expression.Physical interaction between OsSPL10 and OsWOX3B was found both in vivo and in vitro and attenuated their abilities to bind to the promoter of HL6 to activate its transcription.This mechanism may regulate trichome length by adjusting the expression of HL6.A rice gene network regulating trichome development is proposed. 展开更多

The Doppler reflectometer(DR),a powerful diagnostic for the plasma perpendicular velocity(u⊥)and turbulence measurement,has been widely used in various fusion devices.Many efforts have been put into extracting the Doppler shift from the DR signal.There are several commonly used methods for Doppler shift extraction,such as the phase derivative,the center of gravity,and symmetric fitting(SFIT).However,the strong zero-order reflection component around 0 kHz may interfere with the calculation of the Doppler shift.To avoid the influence of the zerofrequency peak,the asymmetric fitting(AFIT)method was designed to calculate the Doppler shift.Nevertheless,the AFIT method may lead to an unacceptable error when the Doppler shift is relatively small compared to the half width at half maximum(HWHM).Therefore,an improved method,which can remove the zero-frequency peak and fit the remaining Doppler peak with a Gaussian function,is devised to extract the Doppler shift.This method can still work reliably whether the HWHM is larger than the Doppler shift or not. 展开更多
